Book excerpt: Transportation Planning and Public Health worked closely together a century ago but drifted apart over the decades. Today, both groups face enormous challenges. Transportation planning needs to replace aging infrastructures while preparing for population growth and the demands that come with it. Resources are already taxed and new ways of doing business are needed. Public Health successfully addressed communicable diseases during the last century only to be confronted with the high cost of treating chronic diseases including obesity in this century. Prevention in the form of increased physical activity as a key strategy led Public Health to the built environment and alternative forms of transportation. A call for both groups to collaborate was issued by national and international leaders and yet far too little collaboration has occurred at the local level. The problem addressed in this study is the disconnect between Transportation Planning and Public Health. The research question is "what are the barriers to collaboration between Transportation Planning and Public Health?" It is expected that local-level government collaboration, organizational incentives and objectives that encourage collaboration, positive history of collaboration with other organizations, personal and professional contacts with other organizations, and effective leadership will be important factors in support of collaboration. It is also expected that no political will, slower process, lack of trust, lack of funding, different mission, different motivation, different culture, and uneven playing field due to power will be top barriers to collaboration. The theoretical framework used is the Network Model which is compared to the Traditional Model. In addition to a literature review regarding public health, transportation planning, collaboration, and the Network and Traditional Models, this study includes a focus group and a survey. The focus group included twelve senior level participants from public health and transportation planning in the North Central Texas Council of Governments (NCTCOG) Region. The survey included a broader segment of the same group totaling 127 respondents. Findings included information about organization practices internally and externally regarding collaboration and Network Model values. The top three supportive factors for collaboration included local level government cooperation, effective leadership of the collaboration, and personal and professional contacts. The top barriers to collaboration included lack of funding, no political will, slower process, different mission and motivation, and uneven playing field due to power. Almost 80% of the survey participants indicated that collaboration between transportation planning and public health was important. Eight action items were identified to enhance collaboration. The action items included 1.) Make the compelling argument to stakeholders as to the importance of collaboration between transportation planning and public health. 2.) Learn together 3.) Identify/develop/utilize tools 4.) Engage leaders 5.) Provide incentives 6.) Build trust 7.) Include local government and 8.) Work within the Network Model for Action.

The Eco-Logical approach is a potential solution designed to advance transportation planning through an integrated planning process that includes other stakeholders. However, since the Eco-Logical approach provides no guidance on forming collaborations, network theory will be used to fill this hole. Both the Eco-Logical approach and network theory place heavy emphasis on information sharing and data exchange. This study will examine the current status of data sharing between transportation planning and public health through an analysis of available literature and case studies, and a survey of transportation planners and public health professionals in the Dallas Fort-Worth Metropolitan Area. Dallas-Fort Worth is area is significant for its growing population and lack of state or regional controls over planning. The purposes of the research are to identify opportunities and barriers to data sharing between the two communities and to make recommendations to improve data sharing.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: Performance measurement and management have been evolving at state Departments of Transportation (DOTs) in recent years, and a variety of performance data is being utilized in different ways to guide decision-making processes. However, health considerations beyond air quality and safety are not yet being incorporated into performance management programs at state DOTs. Concurrently, Health Impact Assessments (HIAs) and other public health tools have seen increasing use among Metropolitan Planning Organizations (MPOs) and their stakeholders through collaboration with public health professionals. With the 2012 reauthorization of the surface transportation bill - Moving Ahead for Progress in the 21st Century (MAP-21) - state DOTs and MPOs have the opportunity to consider health proactively in transportation decision making to address the environmental sustainability requirements of MAP-21. This study investigates the possibility for integration between transportation performance measurement and management and the HIA approach, identifying and explaining the linkages between the two previously isolated processes. The study draws from best practices in performance measurement/management at state DOTs and various examples of health-related activities among MPOs and other planning to inform a suggested approach for incorporating health considerations and metrics in transportation decision making. The suggested approach recognizes common goals of health and transportation agencies, which are well-aligned with national objectives, and emphasizes the role of multidisciplinary interagency collaboration and partnership. This approach is intended to be a resource for state DOTs and MPOs that are interested in extending their performance measurement/management activities to formally include health considerations, as it can ease many of the implementation issues currently faced when considering broader health impacts of transportation.

It not only directly shapes the social and physical environments but also determines the type of places where people can live, learn, work, and play in their everyday lives. This project develops a conceptual framework for collaborative and equitable health and transportation planning by extending the social determinants of health framework to include three major pathways through which transportation factors operate on health and equity outcomes. The three major pathways are behavioral health, environmental health, and social exclusion, which are identified via a thorough review of the academic literature and gray resources on health and transportation connections. Of the three pathway mechanisms, social exclusion and environmental health are intrinsically linked to social equity and justice issues. We further review state-level initiatives linking transportation to health and interview six state departments of transportation that are pioneers in advocating and implementing integrated health and transportation planning. The interview results are summarized and discussed in the report. Based on the interviews, we make recommendations for the Minnesota Department of Transportation to help further its efforts on integrated health and transportation planning.
