Some organizations have been deeply affected by fatal incidents. Even routine injuries and illnesses affecting all organizations can result in lost productivity, increased worker’s compensation cost, and poor employee morale. Although a number of organizations are interested in preventing on-the-job injuries and illness, none focus on the water industry. Book excerpt: In 2004, the WHO Guidelines for Drinking Water Quality recommended that water suppliers develop and implement "Water Safety Plans" (WSPs) in order to systematically assess and manage risks. Since this time, governments and regulators, water suppliers and practitioners have increasingly embraced this approach, but they have also requested further guidance. This much-anticipated workbook answers this call by describing how to develop and implement a WSP in clear and practical terms. Stepwise advice is provided through 11 learning modules, each representing a key step in the WSP development and implementation process: 1. Assemble the WSP team; 2. Describe the water supply system; 3. Identify hazards and hazardous events and assess the risks; 4. Determine and validate control measures, reassess and prioritise the risks; 5. Develop, implement and maintain an improvement/upgrade plan; 6. Define monitoring of the control measures; 7. Verify the effectiveness of the WSP; 8. Prepare management procedures; 9. Develop supporting programmes; 10. Plan and carry out periodic review of the WSP; 11. Revise the WSP following an incident ; Every Module is divided into three sections: 'Overview', 'Examples and Tools', and 'Case studies'. The overview section provides a brief introduction to the Module, including why it is important and how it fits into the overall WSP development and implementation process. It outlines key activities that should be carried out, lists typical challenges that may be encountered, and summarizes the essential outputs to be produced. The examples and tools section provides resources which could be adapted to support the development and implementation of WSPs. These resources include example tables and checklists, template forms, diagrams, or practical tips to help a WSP team address specific challenges. These are often example outputs and methodologies adapted from recent WSP experiences. Each Module concludes with case studies so the reader can benefit from lessons-learned from real-life experiences. They are intended to make WSP concepts more concrete and to help readers anticipate issues and challenges that may arise. The descriptions were drawn from WSP initiatives in Australia, the Latin American and the Caribbean region (LAC), and the United Kingdom.

The Roundtable on Environmental Health Sciences, Research, and Medicine of the Institute of Medicine convened a workshop in October 2007, summarized in this volume, to address objectives related to Sustainable Water, Sanitation, and Hygiene Services. One of the objectives of the workshop was to think about the interdependence of environmental health and human health as connected through water. Organizations cannot discuss water without considering the interrelationship of sanitation and hygiene. It is the convergence of these strategies that promotes healthy outcomes for both individuals and the environment. A second objective of the workshop was to consider how planning, management, and interdisciplinary approaches-including technology, social behavioral issues, gender, health, environment, economic, and political aspects-can be integrated to arrive at sustainable solutions. Many organizations and agencies are trying to forge a path toward sustainable practices in water, but the various sectors utilizing and governing water services are not interconnected. More integration and a greater understanding of holistic approaches are needed.

Book excerpt: Safety has traditionally been defined as a condition where the number of adverse outcomes was as low as possible (Safety-I). From a Safety-I perspective, the purpose of safety management is to make sure that the number of accidents and incidents is kept as low as possible, or as low as is reasonably practicable. This means that safety management must start from the manifestations of the absence of safety and that - paradoxically - safety is measured by counting the number of cases where it fails rather than by the number of cases where it succeeds. This unavoidably leads to a reactive approach based on responding to what goes wrong or what is identified as a risk - as something that could go wrong. Focusing on what goes right, rather than on what goes wrong, changes the definition of safety from ’avoiding that something goes wrong’ to ’ensuring that everything goes right’. More precisely, Safety-II is the ability to succeed under varying conditions, so that the number of intended and acceptable outcomes is as high as possible. From a Safety-II perspective, the purpose of safety management is to ensure that as much as possible goes right, in the sense that everyday work achieves its objectives. This means that safety is managed by what it achieves (successes, things that go right), and that likewise it is measured by counting the number of cases where things go right. In order to do this, safety management cannot only be reactive, it must also be proactive. But it must be proactive with regard to how actions succeed, to everyday acceptable performance, rather than with regard to how they can fail, as traditional risk analysis does. This book analyses and explains the principles behind both approaches and uses this to consider the past and future of safety management practices.
