Book excerpt: Invasion is a crucial component of the complex process of metastasis that marks the transition of breast cancer from local to life threatening disease. The approach we have taken to identify the molecular pathology underlying the onset of invasion, is to apply a combined microdissection and molecular approaches to a unique tissue bank resource. This enables us to isolate mRNA and directly compare gene expression profiles from pathologically defined regions of DCIS and early invasive tumor cells. We have microdissected and identified a number of genes that show differential expression between DCIS and invasive components (in 5 tumors by subtraction hybridization and 8 tumors by membrane filter cDNA array techniques), as well as between low and high grade DCIS. We have also pursued 3 specific genes, previously unexplored in relation to breast cancer, that show patterns of differential expression consistent with a role in the process of invasion - Psoriasin (S100A7), Lumican, (a small leucine-rich proteoglycan) and Angio-Associated Migratory Protein (AAMP). We conclude that our approach is productive in identifying novel genes that are previously unexplored in relation to early breast tumor progression and believe that these may provide markers of progression to invasive disease.

From a clinical perspective, the discovery of genetic markers of risk in a proportion of familial breast cancer cases has opened up new vistas for understanding and ultimately preventing this disease. On the other hand, aggressive - even daring - therapies are being proven to be effective against advanced breast cancer. For the breast cancer experimentalist, this is also a time of great advance. Although animal and cell culture breast cancer models have proven to be of great use, there are now increasing opportunities to test the concepts developed in these models in actual clinical samples and cases. It is gratifying to see how well these concepts "translate" into the clinical setting. A very active area of research that is linking the laboratory to the clinic is the dissection of the biology and elucidation of the significance of proliferate breast disease and the identification of true, "high risk" or "preneoplastic" legions within the previously ill-defined spectrum of fibrocystic or benign breast disease. One anticipates that discoveries made here will also lead to earlier detection, intervention and prevention of life-threatening cancer.Even, however, as we look with optimism to the eventual eradication of breast cancer, we are once again forced to face the reality that we have not yet achieved our goal. Thus, we are saddened by the much too premature death of Dr. Helene Smith from breast cancer. Helena's work was at the forefront of efforts to understand the biology of human breast cancer at the molecular level. Her insight, open-mindedness, and refusal to sacrifice relevance for convenience will continue to set the standard for all breast cancer researchers. This volume is dedicated to her memory.

It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

These results were obtained by loss of heterozygosity (LOM) studies in breast tumors. Metastasis and poor outcome are the cancer phenotypes associated with lip' 5.5. Since breast cancer patients usually do not die as a result of the primary tumor, but from the progression to a metastatic phenotype, we have embarked on a study to identify% the gene(s) responsible on lip15.5. We have constructed a detailed physical map across the region, participated in sequencing the region, generated a transcript map across the region and have begun to access the genes in this region as candidates for tumor suppressor genes. The candidate region has been reduced to 400 kb and contains about 4-6 potential genes. We have cloned the region and the molecular characterization has begun. Interestingly, the 11p15.5 .5 region has been shown to be located in an imprinted domain. We have evidence to suggest preferential 11pl 5.5 maternal loss in breast tumors suggesting that breast cancer in this region has an imprinting component. We have cloned the homologous imprinted region from the mouse and are testing for imprinted expression in fetal and adult tissues including mammary gland.

With the on-going debate about the requirement of EMT for cancer metastasis, an emerging focus on intermediate states of EMT and its reverse process mesenchymal-epithelial transition (MET) offer new ideas for metastatic requirements and the dynamics of EMT/MET during the entire metastatic cascade. Therefore, we would like to initiate discussions on viewing EMT and its downstream signaling networks as a fulcrum of cellular plasticity, and a facilitator of the adaptive responses of cancer cells to distant organ microenvironments and various therapeutic assaults. We hereby invite scientists who have prominently contributed to this field, and whose valuable insights have led to the appreciation of epithelial-mesenchymal plasticity as a more comprehensive mediator of the adaptive response of cancer cells, with huge implications in metastasis, drug resistance, tumor relapse, and patient survival.
