Book excerpt: The institute of Medicine (IOM) has called upon nursing's three million-plus members to become healthcare leaders. With the IOM's call for the advancement of nursing leadership, it is important to examine the reasons why nurses remain underrepresented in top executive healthcare positions such as that of CEO. The hospital or health system CEO directs strategic planning that leads to the implementation of healthcare sevices, and impacts healthcare policy. Since personality affects individuals' attraction to and success in leadership through its impact on behavior and affect, it is important to consider personality characteristics as foundational to the development of succession planning practices. A knowledge gap regarding inherent qualities, such as the personality traits, was found to exist in current nursing leadership literature. Using a comparative descriptive survey design, this study examined personality traits of nurse CEOs and CNOs based on executive job title and gender. To answer four questions, two survey instruments were utilized to collect data for analysis. The NEO Five-Factor Inventory 3 (NEO-FFI-3) and a demographic survey were mailed to 427 nurse CEOs and 408 CNOs identified by the American Hospital Associations Resource Center. Data analysis was conducted using descriptive statistics, an independent samples t-test, and one-way Manova. Results revealed that the personality composition of nurse CEOs and CNOs is similar to that found in CEOs in corporate America, and that title alone had little effect on personality variance. Gender produced variance on the domain of Neuroticism, while gender and title combined created a variance on the domain of Openness. In addition, male CEOs in this study exhibited higher levels of Neuroticism than female CEOs. Demographic data revealed that nearly 60% of executive nursing leaders are at or near retirement age, suggesting a demand for aggressive succession planning. Additionally, a lack of ethnic diversity was revealed as well as an overrepresentation of males in nurse executives roles. It is hoped that these findings will stimulate similar nursing leadership research.

Secondarily, the study examines the influence of selected demographic variables on role performance. Role theory serves as the framework for the study. A convenience sample of 30 nurse executives in the acute care setting was obtained from a list generated by the Organization of Nurse Executives. The 46-item Managerial Work Survey (MWS) developed by McCall and Segrist in 1980 based on work roles identified by Mintzberg (1973) was utilized to collect the data. Descriptive statistics were used to describe the extent to which female nurse executives engage in the work roles operationalized in the MWS. The influence of the demographic variables of age, education, and institutional size on the work roles of leader and entrepreneur were considerable. The role "entrepreneur" was found to be the most frequently performed role. "Planning and implementing change" had the highest measure of central tendency. The investigator concluded that the "entrepreneur" and "leader" work roles had the highest level of participation among the female nurse executives surveyed. Through the research process, the role of the nurse executive can be understood and recognized for the important place it holds in contemporary healthcare organizations. Implications for theory, research, and practice are identified.
