Book excerpt: The purpose of this study is to address the non compliance and decreased awareness by health care professionals in the importance of thromboembolism prevention. A venous thromboembolism (VTE) "occurs for the first time in 100 persons per 100,000 each year in the United States, and rises from 5 cases per 100,000 persons,15 years old to 500 cases (0.5%) per 100,000 persons at age 80 years" (White, 2014). On average "one third of patients with symptomatic VTE will get a pulmonary embolism (PE), whereas two thirds get a deep vein thrombosis (DVT) alone (White, R.H, 2014). Venous thromboembolism prophylaxis is primarily the responsibility of the healthcare professional. The patient is dependent on healthcare professionals to provide the needed care to prevent the occurrence of VTE and to educate the patient, because the patient is not knowledgeable about what is needed to ensure the non occurrence or recurrence of a VTE. It is the responsibility of the health care professional to ensure that the patient is receiving the "best evidenced based practice" possible and care available. Therefore; this study was conducted to bring to the attention, on the need to increase compliance with thromboembolism prevention procedures; and to raise the awareness on the need to educate healthcare professionals on VTE prophylaxis protocols and the need for compliance. Studies have shown that non compliance may stem from, physicians and nurses not understanding or knowing the research on evidenced based practice, thus; this study serves to encouraging the need for educating and for compliance with using prophylactic measures against thromboembolisms and to decrease VTE hospital readmittance, especially in patients that are at risk. With providing the evidenced based education and following compliance measures, setting goals, anticipating milestones, and with setting and using evidence based frameworks, VTE prevalence in at risk patients can be minimized and hospital readmittance costs can be eliminated.

Book excerpt: There are nearly 900,000 cases of venous thromboembolism yearly in the United States with over 300,000 deaths, a figure that exceeds that of most other diseases. This incidence rate is echoed in global studies, with an acknowledged poor adherence to clinical practice guideline recommendations for the prevention of this serious hosptial-acquired condition. Venous thromboembolism is the primary patient safety concern for many of the health care regulating bodies, including the Centers for Medicare and Medicaid, National Quality Forum, and The Joint Commission. It has been recognized as the most preventable cause of hospital deaths; therefore, many insurance payers will limit reimbursement for the added cost of care required in the event of this complication. Arizona hospitals are in the median range when evalutated for compliance with the guideline recommendations and for initiating venous thromboembolism prophylaxis, with a moderate improvement noted in those partiicpating in more closely monitored programs. This capstone project was initiated at a small community hospital that initially had an abysmal rate of adherence with the prevention protocol, and seldom acknowledged the disease risk during patient assessments, with the goal of improvement in regulatory comliance and amelioration of patients safety with a reduction in morbidity and mortality. The theoretical foundation for this project was the Rosswurm and Larrabee conceptual model and the constructivism adult learner theory; additionally, protocols and an audit-feedback system were used as a reinforcement tool. This project was initiated as part of the admission process for inpatients, with each patient expected to be assessed for venous thromboembolism potential within 24 hours using the Caprini risk assessment model. The hospitalist clinicians were provded copies of the model along with the accompanying instruction tool during an educational briefing. Once they were aprised of the disease risks, patient benefits, regulatory requirements, and financial incentives, copies of the risk assessment tool were placed on each medical chart and at each dictation station as a reminder of the protocol. During the first month of this project evaluation, an improvement was noted in guideline adherence from 23% to over 59% of patients assessed for risk and of those found to be at risk, the percentage of patients actually receiving treatment increased from 31% to 77%, demonstrating significant improvement. The expected improvement outcome is 25% to 40% with the use of similar methods as this project. The primary adverse effect of chemoprophylaxis is hemorrhage; however there were no incidences of increased bleeding noted during this evaluation and three incidents of deep vein thrombosis in untreated patients, clearly illustrating the importance of this therapy. This project demonstrated the critical importance of recognizing the risk for thromboembolism, determining the appropriate therapy with consideration for confounding factors, and administering that therapy for an adequate time period. This project supports the recommendations of the clinical practice guidelines and the governmental regulations surrounding the issue, and provides the hospital with a framework for compliance with accreditation and regulating core measures.

Book excerpt: "The purpose of the evidence-based practice (EBP) change project was to improve knowledge of deep vein thrombosis (DVT) prophylaxis, improve provider adherence to standardized prophylaxis guidelines, and decrease the incident rate of DVTs. Despite the known health risk, established DVT healthcare standards are often neglected by healthcare providers (Agency for Healthcare Research and Quality, 2014). The project focused on providers implementing established clinical guidelines to decrease the incidence rate of DVTs in a 115 bed acute rehabilitation hospital. A literature review and established guidelines for antithrombotic therapies yielded an automated tool which assigned risk values to patients and prophylaxis prescription guidance. After a one hour education session, providers consistently used an electronic DVT risk assessment tool to assess new patients and guide prescription decision making. Pre and post education tests measured provider knowledge and education effectiveness. Descriptive statistics evaluated test results and tool usage, as well as DVT benchmark and project comparison incidence rates from hospital electronic medical records diagnostic data. Three outcomes were attained. Provider knowledge increase in mean score was 43% with a t-test result of 2.364 (p=0.003). The compliance rate using the assessment tool was 97.6%. There was reduction in incidence rates of DVTs from the benchmark of 25% to the project rate of 1.6% of patients. Assessment aided by automation enabled appropriate DVT prophylaxes. The DVT risk assessment tool remains in place for providers. Automated DVT risk management results in shorter hospital stays and improved patient outcomes.
