Book excerpt: Research has shown that rural water programs benefit communities by promoting women's empowerment, and improving children's education and the health of residents. However, the tendency for such programs to be short-lived (as water pumps break down and villages are unable to repair them) erodes any benefits and sets villages back on the path toward using unsafe and inconvenient water sources. The incidence of failed rural water projects has prompted calls for a more holistic approach to addressing rural water supply issues. Two development organizations, the Nyarkoa Foundation (NF) and the Rural Education and Development Program (REDEP), implemented a water program in villages located in the Ajumako-Enyan-Essiam District of Central Region, Ghana, where earlier water programs had been unsuccessful. Using a new approach, the program focused on gender-sensitive planning, financing for maintenance and participatory governance. Through interviews, focus group discussions and participant observation, this study evaluates the impacts of the NF/REDEP water program in two villages, Ofosu and Awordo. Findings showed that improved access to clean water enhanced economic opportunities for women and children's education in both villages. There was also evidence that equitable and participatory decision-making engendered cooperation and efficient management of the water program, while exclusionist policy making led to apathy and noncompliance. However, the combined usage of the water pumps with unprotected water sources threatened to negate its health benefits, while the absence of effective women's involvement in the management of the program raised questions about its capacity to truly empower women. These findings reveal the need for increased sensitization on the risks of using unsafe water and a review of the program management approach.

Along with the numerous failures there are also successes in this sector. From these successes a new view has begun to emerge of what the guiding principles of rural water supply strategies should be. This book brings together and spells out the constituents of this emerging view. The central message is that it is the local people themselves, not those trying to help them, who have the most important role to play. The community itself must be the primary decisionmaker, the primary investor, the primary organizer, and the primary overseer. The authors examine the implications of this primary principle for the main policy issues - the level of service to be provided in different settings, the level and mechanisms for cost recovery, the roles for the private and public sectors, and the role of women. The potential advantages of proceeding from this outlook, instead of the older top-down approaches, are considerable. Improvement efforts are more likely to meet felt needs, new facilities are more likely to be kept in service, and more communities are more likely to get safe water sooner.

A dedicated Sustainable Development Goal (SDG 6) declares a commitment to "ensure availability and sustainable management of water and sanitation for all." Monitoring progress toward this goal will be challenging: direct measures of water and sanitation service quality and use are either expensive or elusive. However, reliance on household surveys poses limitations and likely overstated progress during the Millennium Development Goal period. In Innovations in WASH Impact Measures: Water and Sanitation Measurement Technologies and Practices to Inform the Sustainable Development Goals, we review the landscape of proven and emerging technologies, methods, and approaches that can support and improve on the WASH indicators proposed for SDG target 6.1, "by 2030, achieve universal and equitable access to safe and affordable drinking water for all," and target 6.2, "by 2030, achieve access to adequate and equitable sanitation and hygiene for all and end open defecation, paying special attention to the needs of women and girls and those in vulnerable situations." Although some of these technologies and methods are readily available, other promising approaches require further field evaluation and cost reductions. Emergent technologies, methods, and data-sharing platforms are increasingly aligned with program impact monitoring. Improved monitoring of water and sanitation interventions may allow more cost-effective and measurable results. In many cases, technologies and methods allow more complete and impartial data in time to allow program improvements. Of the myriad monitoring and evaluation methods, each has its own advantages and limitations. Surveys, ethnographies, and direct observation give context to more continuous and objective electronic sensor data. Overall, combined methodologies can provide a more comprehensive and instructive depiction of WASH usage and help the international development community measure our progress toward reaching the SDG WASH goals.

Nearly one billion people currently lack access to an adequate water supply, and more than twice as many lack access to basic sanitation services. It is projected that by 2025 water scarcity will affect nearly two-thirds of all people on the planet. Recognizing that water availability, water quality, and sanitation are fundamental issues underlying infectious disease emergence and spread, the Institute of Medicine held a two-day public workshop, summarized in this volume. Through invited presentations and discussions, participants explored global and local connections between water, sanitation, and health; the spectrum of water-related disease transmission processes as they inform intervention design; lessons learned from water-related disease outbreaks; vulnerabilities in water and sanitation infrastructure in both industrialized and developing countries; and opportunities to improve water and sanitation infrastructure so as to reduce the risk of water-related infectious disease.

To focus needed attention on this crisis, the United Nations' Sustainable Development Goals call for universal, equitable, safe, and affordable water service. This global challenge has proved to be particularly difficult in a rural context. Governments in developing regions often lack the resources, capital, and capabilities to provide and manage public services, to include drinking water. Many have turned to the private sector, foreign donors, and aid organizations to supply safe water. Historically, these programs have struggled to do so sustainably, over-prioritizing short-term infrastructure development at the expense of the long-term operation of the systems. Many water providers and their donors have focused on the number of facilities built and the number of people with access, driving the performance of the Water, Sanitation, and Hygiene (WASH) sector by these metrics alone. Sustainability; however, cannot be described by system functionality or population coverage exclusively. The definition of the term should be expanded to include a wider variety of metrics that describe the long term operation and resiliency of the system, from the perspective of both the water suppliers and the communities they serve. A more nuanced view of sustainability is especially needed in rural communities where weak governance, low per capita resources, and high per capita costs of obtaining water access combine to present a substantial barrier to water service delivery. In recent years there has been recognition in the development sector that in order to improve sustainability, water providers should shift from a paradigm that relies on external support yet operates independently from other WASH actors, to one that combines grants and a strong enough revenue base to recover operational and maintenance costs in collaboration with WASH actors. Water providers must achieve a difficult balance that increases impact (increasing the amount of impoverished communities using safe water) with a model that will continue to perform. A broad, systematic review of WASH programs was conducted to develop a comprehensive summary of considerations for rural communities in developing regions to proactively develop their own water provision. The primary recommendations for community authorities seeking water service delivery are applied to a case study on the island of Île-à-Vache, Haiti."--from abstract.
