The rigorous gravity of satisfaction and safety are factors that affect the return of satisfied patients and payment by the Centers for Medicare and Medicaid (CMS) (Hagland, 2011). Because nurses are seen as key providers and are the ones that are typically perceived as providers of timely pain management, ones who develop healthy, trusting relationships and communicators of much of the information that is provided in a hospital setting, many satisfaction scores are based on the quality of nurse care delivered. Due to the importance of nursing and their overall impact on patient's as well as hospital's bottom line, it imperative that the nurse excel at meeting and exceeding patient expectations. When nursing staff is overwhelmed and understaffed, the patient suffers. The suffering can cause longer stays and increased financial burden on the patient as well as the hospital. One way to improve outcomes is to design and implement an hourly or intentional rounding program to ensure that the patient's needs are being met and that patients feel well taken care of. There are different situations in a variety of facilities that can benefit from hourly rounding. The need to develop an program that is tailored to the necessities of a specific floor or unit is crucial in the success of the program. Based on research, hourly rounding can make a positive impact on patient and nurse satisfaction.

Patient safety and fall prevention are a top priority for nursing. Hourly or purposeful rounding has been suggested as an intervention to combat patient falls. Research shows that hourly rounding reduces patient falls, decreases call light use and helps to improve patient satisfaction scores. A systematic review of literature was conducted to determine the effectiveness of hourly rounding in preventing patient falls as oppose to no rounding or current clinical practices. Fifteen peer-reviewed research articles on the subject were examined, including 13 quantitative or qualitative studies and two literature review studies. Overall, based on the research, hourly rounding helped to reduce patient falls. Therefore, hourly rounding should be incorporated into nursing work flow as an evidence-based best practice.

There have been various methods to prevent falls from happening. Bed alarms and sitters are common methods used to prevent falls. One way to increase safety in hospitals is to implement hourly rounds. Hourly rounds allow the nurse to address patient needs before something can happen such as a fall. Hourly rounds also give the patient and nurse to discuss any concerns they may have about the plan of care. The process of hourly rounds would require educating the nursing staff about the process. Education materials such as pamphlets and in services would be needed to inform everyone of the new process. Step by step instructions on how to do hourly rounds will be presented. To evaluate the effectiveness of the hourly rounding process patients will fill out questionnaires about the hourly rounds. That information can be used to determine if hourly rounds increase patient satisfaction and safety. Nurses will also be interviewed about the hourly rounding process. The process will be reviewed and determined the method is effective I increasing patient satisfaction and safety.

Patients must be monitored and cared well to improve their health conditions and to avoid further complications during hospital stay. Risk of hospitalized patients fall is the most concern for nurses. Hospital acquired new incidents like patient fall and pressure ulcers has a negative impact on patient recovery, causes lengthy hospital stay, financial burden for hospitals and increases nursing burnouts. Patients with stroke are at high risk for fall. In earlier studies it is found that 14% of the stroke patients fell at least once during their hospital admission (Tutuarima, Meulen, and Limburg, 2012). Frequent monitoring of the patients by nurses can prevent never events. But nurses attempt to be proactive on avoiding these issues are impaired by factors like high patient ratios, lack of time , frequent call lights and stress from meeting the patients demands. Planned hourly rounding is a proactive approach to patient care. Hourly rounding reduces patient fall, skin break downs, call light use, noise level and also improve patient satisfaction. Studies on hourly rounding to increase the patient safety and patient satisfaction showed a 52% decline in call light use after implementing hourly rounding and unit was quiet and nurses could spend more time for patient care and logs (Ford, 2010). Intentional rounding researches showed significant reduction in fall rate with one hour rounding. With the implementation of hourly rounding on the stroke and cardiac inpatient unit of the Texas Health Resources Hospital, the purpose is to prevent unwanted events and to improve patient satisfaction and safety.

This project is a review of literature regarding the impact of hourly rounding on call light use, patient satisfaction, and safety with a primary focus on falls in an acute care setting. It is proposed that use of purposeful hourly rounding will increase both patient safety and decrease use of call lights if implementation includes the use of theory of planned behavior, which would impact both patient and nursing staff behavior. The implementation plan set forth in this project is not one of new design. It is one of modified design which relies heavily on implementation research studies allowing for an understanding of what can be improved so that professional behaviors already in place will be more streamlined. The evaluation plan has reasoning as well as tools to be used in the process of piloting the program allowing for improvement before dissemination to other facilities for a larger scale impact.

Hourly rounding consists of asking patients important questions each hour to help keep patients more comfortable while also maintaining safety during the hospital stay. In order to assist hourly rounding in becoming an even bigger part of the health care routine, data needs to be collected from studies to see how well it really works. By doing a study and tracking the results over a six-month time frame, it will not only help hourly rounding become implemented on that particular floor, but it could also lead to the implementation of hourly rounding in many other places. Evidence based practice runs the health care profession. Once a theory is proven to work or proven to be the safer method, it will be implemented in a larger setting. Hourly rounding is hypothesized to decrease patient falls, patient call lights, and increase patient safety and satisfaction. Before the study begins, the staff and supervisors will be educated on the topic as well as what is expected from hourly rounding. There will be a staff meeting with pamphlets, power point presentations, and additional time to ask any questions that may arise. Once the information from the study has been collected and interpreted, we should see that patient satisfaction rises because patients feel a bigger sense of responsibility in their own care. Some patients have commented that hourly rounding made them feel more important. Patients need to feel like the center of their care and feel like their opinions truly matter. Hourly rounding is going to help hospitals achieve that goal.

Hourly rounding has been shown to decrease call light usage, falls and pressure sores as well as increase patient satisfaction. 'Margo A. Halm, RN, PhD, CNS-BC, director of nursing research and quality at Salem Hospital in Oregon, reviewed 11 hourly rounding studies and reported in a 2009 American Journal of Critical Care article that 83 percent of studies examined showed a reduction in call lights, 77 percent a decrease in fall rates and 88 percent an improvement in patient satisfaction' (Wood, 2014, para. 8). 'Swanson's Nursing Care Theory supports the purposed use of hourly rounding as it centers on practices that promote patient satisfaction and safety by focusing on the connection between caring processes and patient well-being and outcomes' (Tongues, Ray, 2011, pages374). Hourly rounding was introduced previously at this facility but the implementation was poorly received by nurses. In an effort to reintroduce hourly rounding, a team will be formed to champion the process, a presentation to the stakeholders will be done outlining the plan as well as the costs involved, and ongoing education will be provided for nursing as well as tools to aid the nurse in incorporating hourly rounding into their daily routines. Weekly meetings with a specific agenda will be held encouraging staff feedback on how the hourly rounding trial is proceeding. At the end of the trial period an assessment will be given to each nurse seeking their ideas and any new problems that need to be addressed. The team will evaluate data from the meetings, patient's comments and any occurrence reports concerning falls and pressure sores and call light usage information obtained from the facilities call light system. This information will be shared with the stake holders as well as nursing staff. As the nursing shortage increases, hospitals as well as nursing leaders are searching for ways to provide quality care, enhance patient safety and satisfaction and to increase nurse's satisfaction. 'Hourly patient rounding proposes to be one answer to both patient and nurse satisfaction' (Wood, 2014, para.6).

Strong patient satisfaction promotes trust in the health care giver, and gives patients confidence in referring their loved ones, friends, and families to the same facility for care. It was observed that the role that nurses and other direct care givers play in giving the best care to the patient plays a huge role in patient satisfaction as well as willingness to recommend the facility to others. Safety is also an important subject observed to be a concern for every patient admitted into the hospital for care, and nurses are direct care givers who spend the most time with the patient which forms the basis of patient trust and confidence. The need to improve patient satisfaction, and prevent harm was identified in the medical/surgical unit of the hospital with the largest volume of hospital admissions. A pilot study to last for two months was proposed for one arm of the unit on the south side. The purpose of the study was to test the efficacy of hourly rounding, and its effect on patient satisfaction due to the presence of staff in the room to proactively meet with patient needs of Pain, Potty, Position, Possession, and Parting (5P's) every hour. Hourly rounding is designed to reduce the use of call lights, prevent falls, and reduce long hospital stays. This is in contrast to the current system of waiting for the sound of the bed alarm, or a patient to call before entering the room, which has resulted in many falls, delayed responses to call lights, and long hospital stays. Research evidence shows that in many hospitals across the nation where hourly rounding was done intentionally to meet with patient needs there was great improvement in several patient related factors. These factors include higher patient satisfaction scores, reduced harm such as falls, increased prevention of pressure ulcers, and reduced use of call lights.

The problem exists because patients continue to fall and often times are seriously injured. There are also concerns of patients not being turned and ending up with pressure sores. These problems result in poor patient satisfaction. Studies have shown that hospitals and rehabilitation facilities that have taken part in trials of hourly rounding by nurses and nursing personnel, have shown a decrease in the number of patient falls and an increase in patient satisfaction numbers. Implementing a plan for hourly rounding is necessary to improve patient safety, involving nurses checking on patients and observing patient's rooms and surroundings. After the plan is implemented, an evaluation of the process is required to determine if the process or plan has been effective. Evaluation is often skipped over in EBP research, however it is necessary to determine the effectiveness of the plan and to let others know the results of the trial. Shortly after the plan is evaluated, it should be disseminated to other nurses, stakeholders and the public in order for the information to be useful. Dissemination is also important because it allows nurses to see results of studies in a quick, efficient way without having to spend hours looking up the research themselves.

Research has demonstrated hourly rounding to be a cost-effective, nurse led intervention that positively influences patient satisfaction (Petras, Dudjak and Bedner, 2013; Sherrod, Brown, Vroom and Sullivan, 2012; Brosey and March, 2015). Hourly rounding is a proactive approach to patient care where basic patient needs are anticipated and fulfilled through regularly scheduled checks by healthcare professionals. This evidence based practice (EBP) project proposes implementation of hourly rounding to improve patient satisfaction at Canyon Vista Medical Center's (CVMC) emergency department (ED). A thorough description of the problem and proposed solution will be discussed. A detailed implementation plan including rationale, evidence, approval, implementation logistics and resources will be provided. An evaluation plan consisting of evaluation tools and data collection methods will be presented in addition to a strategy for dissemination of findings to key stakeholders and the greater nursing community. Finally, a literature review including analysis and appraisal of fifteen peer-reviewed research articles on the topic of hourly rounding and patient satisfaction will be presented.

Book excerpt: The purpose of this practice improvement project was to determine if a relationship exists between purposeful hourly rounding and incidence of patient falls and patient satisfaction with information received from nurses for male and female patients over 18 years in one 60-bed acute rehabilitation setting. The first question that guided this project was: Are patient falls in an acute rehabilitation setting reduced when purposeful hourly rounding occurs? The second question was: Do patient satisfaction scores improve when purposeful hourly rounding was implemented in acute rehabilitation setting? This project implemented purposeful hourly rounding by nursing staff over a three-week intervention period in one rehabilitation hospital with an average daily census of 40 male and female patients. Employee education occurred in February, 2017. Fall rates were gathered from the hospital's internal incident reporting system. Patient satisfaction scores were obtained from Press Ganey surveys. The patient satisfaction results and the fall rates of 39 patients who did not experience purposeful hourly rounding were compared to the patient satisfaction and fall rates of 68 patients who experienced the purposeful hourly rounding. This project used a quasi-experimental correlational design. An independent t-test analysis did not demonstrate a difference between the patient satisfaction scores on 10 surveys received from the first group of patients and the 5 surveys received from the patients who experienced rounding. A chi-square analysis of the fall rates did not demonstrate a statistical difference between the groups. Although the results were not statistically significant, the project provided a baseline for future projects such as identifying barriers for staff to perform purposeful hourly rounding and improving the preparation for staff to perform hourly rounding.
