Book excerpt: Studies conducted over the last 25 years have suggested that a substantial fraction of hospital use is inappropriate. This report examines the relationship between patient and provider (e.g., physician and hospital) characteristics and inappropriate hospital use, in terms of both admissions and days. The study examines adult nonpregnancy hospitalizations from a randomized trial of health insurance, the RAND Health Insurance Experiment. The data include evaluations of the appropriateness of inpatient treatment based on medical record review, patient characteristics (sociodemographic, economic, and health status), and provider characteristics (specialty and practice patterns for physicians, accreditations and facilities for hospitals). The findings indicate that women and patients of older physicians are more likely to have been inappropriately admitted to the hospitals. The authors conclude that patient and provider characteristics influence the likelihood that an admission is inappropriate; however, differences in provider and patient characteristics do not account for geographic differences in inappropriate hospitalization.

Book excerpt: This Note analyzes the literature regarding appropriateness of acute care provided to the elderly. The authors identified 17 articles that explicitly cited appropriate or inappropriate care (including undercare, overcare, and misuse) provided in hospital and ambulatory settings and for procedures, and 19 articles that presented data on the appropriateness of medication use in the elderly. Virtually every study found at least double-digit levels of inappropriate care. Perhaps as much as one-fifth to one-quarter of acute hospital services or procedures were felt to be used for equivocal or inappropriate reasons, and two-fifths to one-half of the medications studied were overused in outpatients. The few studies that examined underuse or misuse of services also documented the existence of these phenomena. This was especially true for the ambulatory care of chronic physical and mental conditions and concerned the use of low-cost technologies (visits, preventive services, some medications). Thus, the authors conclude that there appears to be a substantial problem in matching acute services to the needs of elderly patients. This mismatch occurs both in terms of overuse and underuse, at least for areas where research has been conducted.

Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.
