Book excerpt: The emergence of severe acute respiratory syndrome (SARS) in late 2002 and 2003 challenged the global public health community to confront a novel epidemic that spread rapidly from its origins in southern China until it had reached more than 25 other countries within a matter of months. In addition to the number of patients infected with the SARS virus, the disease had profound economic and political repercussions in many of the affected regions. Recent reports of isolated new SARS cases and a fear that the disease could reemerge and spread have put public health officials on high alert for any indications of possible new outbreaks. This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. The report provides an illuminating survey of findings from the epidemic, along with an assessment of what might be needed in order to contain any future outbreaks of SARS or other emerging infections.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

The public health emergency preparedness and response (PHEPR) system, with its multifaceted mission to prevent, protect against, quickly respond to, and recover from public health emergencies, is inherently complex and encompasses policies, organizations, and programs. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.

Our families are more fragmented, more people are childless and we are living longer, which all increase our chances of being socially isolated in later life. A growing number of older people can be expected to be housebound with minimum social contact. Many of these risk factors are associated with social progress and independence, particularly for younger people. But they make it harder for us to sustain the social networks on which we may have to rely for support as we grow older. Policy-makers have tried to protect older people against isolation through efforts to integrate health and social care services, and to involve older users more actively in making decisions about their own care. Yet more must be done to improve the quality of life of those living housebound lives. Over 30 people die alone and unnoticed in their own homes each year, and one in six older people living alone rate themselves as ‘often or always lonely’.Unless significant changes occur, by 2021 nearly 2.2 million over-65s will be socially isolated. The benefits of promoting independent living for older people are enormous, for individuals, our public services and wider society. Extending years of active life will allow older people to play a full role in family life and in their local communities. Home Alone argues that users need to become ‘co-producers’ of personalised services. Policy-makers must learn from the voluntary and community sectors to help older housebound people develop their own networks of support.

Originally conceived as a digital campaign the project in collaboration with DISTANZ is now spanning across six continents. STILL HERE is rooted in the still life, with its iconic depictions of inanimate objects, finding 'beauty' in banality. The book presents a selection of one hundred submissions for the project and illustrates that, despite the current restrictions, a profound sense of community and creativity is still alive and pulsing in the private spaces of many. With contributions by the artists such as Monica Bonvicini, Tosh Basco fka Boychild, Katherine Bernhardt, Simon Denny, Marcel Dzama, Issy Wood, Shirin Neshat, Adam Pendleton, Laure Prouvost, Wolfgang Tillmans, Raphaela Vogel, Paul Mpagi Sepuya, among many more, alongside authors such as Chris Kraus, ballet dancer David Hallberg, neuroscientist Mendel Kaelen, auctioneer Simon de Pury and sexual anthropologist Betony Vernon. The vast majority of people in wealthy, highly connected, or digitally privileged societies may have crossed the digital divide, but being online does not mean that everyone is equally connected—and digital inequality reflects experience both online and off. In Connected in Isolation Eszter Hargittai looks at how this digital disparity played out during the unprecedented isolation imposed in the early days of the coronavirus pandemic. During initial COVID-19 lockdowns the Internet, for many, became a lifeline, as everything from family get-togethers to doctor’s visits moved online. Using survey data collected in April and May of 2020 in the United States, Italy, and Switzerland, Hargittai explores how people from varied backgrounds and differing skill levels were able to take advantage of digital media to find the crucial information they needed—to help loved ones, procure necessities, understand rules and risks. Her study reveals the extent to which long-standing social and digital inequalities played a critical role in this move toward computer-mediated communication—and were often exacerbated in the process. However, Hargittai notes, context matters: her findings reveal that some populations traditionally disadvantaged with technology, such as older people, actually did better than others, in part because of the continuing importance of traditional media, television in particular. The pandemic has permanently shifted how reliant we are upon online information, and the implications of Hargittai’s groundbreaking comparative research go far beyond the pandemic. Connected in Isolation informs and expands our understanding of digital media, including how they might mitigate or worsen existing social disparities; whom they empower or disenfranchise; and how we can identify and expand the skills people bring to them.
