Rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) are two prototypic female-predominant autoimmune diseases. Despite different clinical manifestations, similarities exist. Both are characterized by a dysregulated immune response to self-tissues. For both diseases, the strongest genetic risk factors are linked to human leukocyte antigen (HLA) alleles. Pregnancy affects their course and may also contribute to risk. This dissertation investigates mother-child HLA relationships and risk of developing RA or SLE. Chapter 1 provides background on RA and SLE and a literature review outlining the rationale for the dissertation. The chapter states the hypotheses of the project and the specific aims addressed in each chapter. Chapter 2 addresses the question of mother-child histocompatibility and risk of RA or SLE. The study tests the hypothesis that having any children who are compatible from the perspective of the mother increases the risk of developing RA or SLE. Results show that having any histocompatible children at HLA-B and DPB1 increase risk of RA in the mother. Increased structural similarity as quantified by a sequence similarity matching score at DPB1 was associated with decreased risk of SLE. Mother-child compatibility at minor histocompatibility antigen ZAPHIR was likewise associated with decreased risk of SLE but compatibility at LB-WNK1 increased risk for mothers who were also HLA-compatible at HLA-A with their children. This is the first study investigating the relationship between minor histocompatibility antigen compatibility and risk of SLE. This study supports the hypothesis that mother-child HLA relationships are associated with risk of RA and SLE. Chapter 3 addresses the question of whether having children who carry HLA alleles associated with increased risk of SLE contributes to its development among mothers. The alleles investigated include DRB1*03:01, *15:01, *08:01. In addition, the study tests the hypothesis that molecular mimicry by means of exposure to DRB1*04:01 that codes for a similar amino acid sequence to the Epstein-Barr virus, a known SLE risk factor, increases risk among mothers. There was no association between SLE risk-associated alleles. However, increased risk of SLE was associated with having at least one child who carried the DRB1*04:01 allele among women negative for the allele but with at least one of the predisposing alleles. This study supports the hypothesis that a child's alleles inherited from the father influence a mother's subsequent risk of SLE. Chapter 4 investigates the association between having children who carry alleles encoding amino acids (AA) associated with RA including the "shared epitope", the sequence DERAA at AA positions 70-74, AA valine, lysine, alanine at positions 11, 71, 74 of the HLA-DRB1 molecule, aspartic acid at position 9 of HLA-B and phenylalanine at position 8 of DPB1. The study finds increased risk of RA among mothers who had any child with SE, DERAA, valine, lysine and alanine encoding alleles, independent of maternal carrier status. Among non-carrier mothers, increased risk of RA was associated with having children who carried DERAA. Chapter 5 summarizes the key findings and results from each study and the conclusions of this work. Future directions are also addressed.

Book excerpt: [Truncated abstract] The dramatic increase in the expression of allergic diseases such as asthma and allergy over the last 20-30 years has highlighted the urgent need to identify causative factors. It was hypothesised that direct immune interactions between mother and fetus contribute to the cytokine milieu of pregnancy, thus influencing immune maturation after birth. Further it was speculated that the cytokine responses produced as a result of maternalfetal interactions are Th-2 skewed in women allergic disease, which programmes their offspring towards developing an allergic phenotype after birth. To test this hypothesis a cohort of 169 pregnant women were recruited at 20 weeks gestation and defined as allergic or non-allergic based on both clinical history and skin prick test sensitisation. These women and their infants were followed up throughout pregnancy (20 weeks, 30 weeks, 36 weeks gestation and 6 weeks post-partum) and up to 2.5 years of age. Mixed lymphocyte reactions (MLR) were used to measure maternal cytokine (IL-6, IL-10, IL-13 and IFN-) and lymphoproliferative responses to fetal alloantigens at each pregnancy time-point. Human leukocyte antigen (HLA) typing of mothers and infants were performed to assess the effect of HLA mismatch on maternal MLR responses to their fetus. After delivery, mononuclear cells (MNC) were isolated from cord blood (CB) and stimulated with allergens, mitogen and toll-like receptor (TLR) ligands. .... As IL-6 also participates in adaptive immunity by promoting Th-2 differentiation it is proposed that the production of IL-6 as a results of maternal encounters with paternal antigens during pregnancy, contribute to the Th-2 skewed responses observed universally in most infants at birth. Associations between maternal-fetal interaction and clinical outcomes in infancy: Although clinical signs of allergy in infancy were not the main outcome measure of this thesis, there were interesting, yet complex relationships between the production of these maternal cytokines towards the fetus and allergic disease at infant follow-ups. Increased maternal IFN-y to fetal alloantigen was associated with asthma at 2.5 years and a trend towards recurrent wheeze at 12 months. In contrast decreased maternal IL-13 production was associated with IgE mediated food allergy at 12 months. Adjusting for maternal allergy and other potential confounders including infant gender, method of delivery, HLA mismatch, and paternal allergy did not account for these relationships. Further follow-ups of these infants are required to determine if these relationship last in to early childhood. In conclusion, the findings of this thesis provides further support for the hypothesis that immune responses at birth are programmed prenatally, and that this programming has implications later in life. Importantly, the placenta is the immunologically active interface between mother and fetus during pregnancy. Therefore it is emphasised that there is a crucial need for future research to focus on early immune programming at the placental level before the aetiological pathways of immune mediated diseases can be fully elucidated.

The central objective of the symposium was to throw light on the regulation of cellular and molecular events underlying blastocyst formation. It was particularly appropriate that the date of the symposium marked the 20th anniversary of the publication of the classic volume Biology of the Blastocyst, the proceedings of an international workshop held in 1970. This book, which summarized most of the information then available on this topic in mammals, was edited by the pioneer in blastocyst research, Dr. Richard B1andau, who was the guest speaker at the symposium.

Book excerpt: The non-classical HLA class I molecule HLA-G is different from classical HLA class I molecules because of the low polymorphism in the coding region, the fact that HLA-G primary transcript is alternatively spliced in seven isoforms, and the inhibitory action on immune cells. Although HLA-G is low polymorphic, variants in both promoter and 3’ un-translated region (UTR) of HLA-G locus regulate its expression. In healthy conditions, a basal level of HLA-G gene transcription is observed in most cells and tissues; however, translation into HLA-G protein is restricted to trophoblasts in the placenta, where it participates in promoting tolerance at the fetal-maternal interface. HLA-G is also expressed by thymic epitelial, cornea, mesenchymal stem cells, nail matrix, pancreatic beta cells, erythroid, and endothelial precursors. HLA-G can be neo-expressed in adult tissues in pathological conditions, and its expression has been documented autoimmune disorders, viral infections, and cancer. In the latter setting de novo HLA-G expression is associated with the capability of tumor cells to evade the immune control. In the last decade it has become evident that HLA-G expression on T cells and antigenpresenting cells confers to these cells tolerogenic properties. This Research Topic focused on i) summarizing updated clinical and immunological evidences that HLA-G expression is associate with beneficial or detrimental tolerance, ii) gathering new insights into the mechanisms governing the expression of HLA-G in healthy and pathological conditions, such as pre-eclampsia, and iii) examining the mechanisms underlying HLA-G mediated tolerance.
