The application of this information is ever more important as the limits of transplantation continue to be reduced, including the recent success of bone marrow transplantation between unrelated but closely matched individuals. In addition, the need to transfuse platelets in the face of immunologic barriers continues to challenge transfusion services. Thus, the serologic information summarized in this volume is essential for optimal patient care. At the same time, recombinant DNA technology has led to a revolution in our understanding of many aspects of basic biology. Among the advances has been the initial characterization of the structure of some HLA loci. While this will ultimately improve clinical services, constant reference to serologic data is essential so that the powerful new techniques can be applied in the most effective ways. The timing of the First Red Cross International Histocompatibility Workshop is fortunate as it brings together experts from around the world to address the state of the art. We are all grateful to Dr. John Lee and his colleagues for organizing the workshop, and for bringing together in this volume the material to be presented in Beijing during October 17-23, 1990. Book excerpt: The Major Histocompatibility Complex (MHC) was discovered as a con sequence of the chronic problem encountered by cancer biologists in the early years of this century: the failure to maintain tumor lines by serial passage in outbred mice. A number of observations pointed to genetic similarity being a prerequisite for successful transplantation and they were incorporated into a genetic theory of transplantation by C.C. Little. This prompted scientists like Little to initiate breeding experiments designed to test his hypothesis and produce genetically identical mice which would permit the growth of trans planted tumors. Most inbred strains of mice commonly used in immunology derive from those efforts. Transplantation of normal tissues obeyed the same rules found for malignant tissues and rejection was shown to be an immunological phenomenon. G.D. Snell showed that a single genetic locus determined rapid rejection of skin grafts. This was initially called the Major Histocompatibility Locus but was subsequently shown to include many functionally related genes and renamed the Major Histocompatibility Complex (MHC). Book excerpt: The aim of MHC Protocols is to document protocols that can be used for the analysis of genetic variation within the human major histocompatibility complex (MHC; HLA region). The human MHC encompasses approximately 4 million base pairs on the short arm of chromosome 6 at cytogenetic location 6p21. 3. The region is divided into three subregions. The telomeric class I region contains the genes that encode the HLA class I molecules HLA-A, -B, and -C. The centromeric class II region contains the genes encoding the HLA class II molecules HLA-DR, -DQ, and -DP. In between is the class III region, originally identified because it contains genes encoding components of the complement pathway. The entire human MHC has recently been sequenced (1) and each subregion is now known to contain many other genes, a number of which have immunological functions. The study of polymorphism within the MHC is well established, because the region contains the highly polymorphic HLA genes. HLA polymorphism has been used extensively in solid organ and bone marrow transplantation to match donors and recipients. As a result, large numbers of HLA alleles have been identified, a process that has been further driven by recent interest in HLA gene diversity in ethnic populations. The extreme genetic variation in HLA genes is believed to have been driven by the evolutionary response to infectious agents, but relatively few studies have analyzed associations between HLA genetic variation and infectious disease, which has been difficult to demonstrate.
