The interaction of HIV/AIDS with tuberculosis, malaria and bacterial infections have increased HIV-related morbidity and mortality, and in turn, the HIV pandemic has brought about devastating increases in tuberculosis. Understanding the population impact and the dynamics of infection diseases in the most affected region is critical to efforts to reduce the morbidity and mortality of such infections, and for decisions on where to use limited resources in the fight against infections. This book aims to contribute to these efforts by offering a demographic and epidemiological perspective on emerging and reemerging infections in sub-Saharan Africa.

Whilst developed economies already have the basic infrastructure in place through which to support their ageing populations, developing nations frequently do not, and it should not be assumed that their best course of action is to attempt to duplicate the supportive infrastructures of developed countries. In developing nations these may be culturally inappropriate, geographically inaccessible, economically or politically unsustainable, or all of these. Effective and sustainable support services must be designed with reference to the circumstances of the client group, and it is increasingly evident that knowledge of the lives of the aged in developing countries is currently very limited. This book aims to inform the reader on the livelihoods of elders in developing countries and to stimulate a discussion of appropriate methods of supporting them in maintaining their quality of life during and beyond the coming decades of demographic change. It does so through reporting the lives and livelihoods of the aged population of Kikole (a pseudonym), a highly impoverished village in Uganda. Individual livelihoods are explored from a lifecourse perspective, with present day quality of life being shown often to be the result of earlier enforced changes in circumstances arising in economic, social or cultural marginalization, political or physical insecurity, or macro-economic change, rather than in the physical or mental changes that may accompany advancing age.

Over 16,000 new infections occur every day. Yet we are not helpless in the face of disaster, as shown by the rich prevention experience analyzed in this valuable new compendium. “Best pr- tice” exists—a set of tried and tested ways of slowing the spread of HIV, of persuading and enabling people to protect themselves and others from the virus. Individually, features of best practice can be found almost everywhere. The tragedy, on a world scale, is that prevention is spotty, not comprehensive; the measures are not being applied on anywhere near the scale needed, or with the right focus or synergy. The national response may concentrate solely on sex workers, for example. Elsewhere, efforts may go into school education for the young, but ignore the risks and vulnerability of men who have sex with men. Action may be patchy geographically. AIDS prevention may not benefit from adequate commitment from all parts and sectors of society, compromising the sustainability of the response. In some countries matters are still worse—there is still hardly any action at all against AIDS and scarcely any effort to make HIV visible. It is no wonder that the epidemic is still emerging and in some places is altogether out of control.
