Book excerpt: The ongoing plight of refugees and displaced persons worldwide is one of today' greatest tragedies. The groups can include refugees, civilians who have returned home but still need help, persons displaced internally within their own countries, asylum seekers and stateless people. These people are vulnerable and their ability to cope is left severely compromised. Because the normal social safety nets are often absent, women and girls can be subject to sexual violence and rape, and generally drug and alcohol abuse are often rife. At the same time, health care service are often only minimal or non-existant. The variety of complex factors within refugee camps or displaced settlements makes the people living there enormously vulnerable to HIV. Addressing HIV-related needs in the context of refugee situations requires a change in the thinking of the authorities in many countries of asylum. Failure to provide the necessary interventions could be very harmful to both refugees and the surrounding host populations. In order to meet the HIV-related needs in this context, the UN High Commissioner for Refugees and UNAIDS advocate for the implementation of best practices. Suggested best practices include: Integrating refugee issues into national health and HIV programmes; implementing subregional initiatives; and combining funding streams.

Book excerpt: The mini-dissertation analyses the international law obligations of the government and nonstate actors regarding the protection of internally displaced children living with HIV in the Democratic Republic of the Congo. The war and armed conflicts in the Eastern DRC have exacerbated the vulnerability of children, causing them to be separated from their families, to experience sexual violence and forced conscription into armed groups, to experience the violent deaths of a parent or friend, resulting in insufficient adult care. They further are subject to a lack of safe drinking water and food, insufficient access to health care services, discrimination and stigmatisation, and so on. These factors increase their risk of contracting HIV and, if they are already living with HIV, they adversely affect their welfare. The mini-dissertation illustrates that international, regional and domestic human rights instruments protecting children can be applied in situations of armed conflicts to supplement humanitarian law instruments. It demonstrates that the government of the DRC has not implemented and fulfilled its international obligations to ensure these children adequate access to health services and to humanitarian assistance for displaced persons living with HIV: security and protection within displaced persons camps: and that children are protected from abuse and human rights violations. The dissertation recommends the prosecution of perpetrators of crimes tied to the conflicts which have targeted children, as well as the ratification by the DRC of regional instruments such as the African Union Convention on the Protection and Assistance of Internally Displaced Persons in Africa, and the African Charter on the Rights and the Welfare of the Child, as this may enhance the legal protection of displaced children in the DRC.

Book excerpt: Human Immunodeficiency Virus infection (HIV) and Acquired Immunodeficiency Syndromes (AIDS) continue to be major global health priorities. Although forced displacement is believed to fuel HIV/AIDS epidemic in countries affected by conflict, factors associated with forced displacement and HIV risk in conflict-affected populations are not well understood. The aims of this study were therefore (1) to describe factors that influence risk for HIV infection in a Utah-based Burundi refugee population and (2) explore relationship between Knowledge, Attitudes, and Behavior/Practices (KABP) in order design targeted HIV/AIDS prevention interventions for the population. Results were controlled for known confounders. Attitude towards HIV/AIDS (b = 0.5071, p > 0.001) was a significant predictor of HIV/AIDS knowledge. Behavior/ practices were strongly associated with knowledge, attitudes, self-efficacy, gender, and household size. Overall, men were more knowledgeable than women and had higher self-efficacy. Women had more positive attitudes and scored higher behavior/practices.

Book excerpt: The past 25 years have seen a major paradigm shift in the field of violence prevention, from the assumption that violence is inevitable to the recognition that violence is preventable. Part of this shift has occurred in thinking about why violence occurs, and where intervention points might lie. In exploring the occurrence of violence, researchers have recognized the tendency for violent acts to cluster, to spread from place to place, and to mutate from one type to another. Furthermore, violent acts are often preceded or followed by other violent acts. In the field of public health, such a process has also been seen in the infectious disease model, in which an agent or vector initiates a specific biological pathway leading to symptoms of disease and infectivity. The agent transmits from individual to individual, and levels of the disease in the population above the baseline constitute an epidemic. Although violence does not have a readily observable biological agent as an initiator, it can follow similar epidemiological pathways. On April 30-May 1, 2012, the Institute of Medicine (IOM) Forum on Global Violence Prevention convened a workshop to explore the contagious nature of violence. Part of the Forum's mandate is to engage in multisectoral, multidirectional dialogue that explores crosscutting, evidence-based approaches to violence prevention, and the Forum has convened four workshops to this point exploring various elements of violence prevention. The workshops are designed to examine such approaches from multiple perspectives and at multiple levels of society. In particular, the workshop on the contagion of violence focused on exploring the epidemiology of the contagion, describing possible processes and mechanisms by which violence is transmitted, examining how contextual factors mitigate or exacerbate the issue. Contagion of Violence: Workshop Summary covers the major topics that arose during the 2-day workshop. It is organized by important elements of the infectious disease model so as to present the contagion of violence in a larger context and in a more compelling and comprehensive way.

The country seems to be at an early epidemic phase with low HIV prevalence, but there are a number of underlying vulnerability factors that could lead to the conditions for epidemic expansion, including drug trafficking, the post-conflict situation with displacement of populations, a fledgling health care system, and a low level of knowledge and awareness about HIV/AIDS. As in other parts of central and south Asia, the most important proximate determinants of the scale and distribution of an HIV epidemic in Afghanistan will be the size and characteristics of high risk networks involving injecting drug users (IDUs), female sex workers (FSWs) and men who have sex with men (MSM) who are at high risk. Assessments from elsewhere in central Asia indicate an explosive growth in injecting drug use and commercial sex work throughout the region, concurrent epidemics of sexually transmitted infections (STIs), and economic and political migration. As yet, little information is known about the size, distribution, and characteristics of IDU and sex worker sub-populations in Afghanistan. Therefore, the World Bank (WB) agreed with the Ministry of Public Health (MOPH) to contract with the University of Manitoba (UM) to conduct an assessment of these three key, high risk populations in three cities of Afghanistan (Mazar-i-Sharif, Jalalabad, and Kabul).

Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways.
