Although research in stroke, hematology, and related fields has made enormous progress in the last decade, stroke still remains one of the leading causes of death today. To achieve an interdisciplinary approach to current research and practice in the management of thrombosis of cerebrovascular disorders, scientists and clinicans worldwide met in Lübeck, Germany, in May 1993 for an International Symposium on Hemostasis and Stroke. This book represents the proceedings of the symposium. It also summarizes the results of recent stroke prevention trials, including critical reviews on current recommendations of acetylsalicyclic acid and the usefulness of ticlopidine. In addition, animal models of experimental cerebral ischemia are viewed under their possible application for research in embolic stroke.

Book INFLAMMATORY AND HEMOSTASIS BLOOD BIOMARKERS PREDICT THE CLINICAL OUTCOME OF STROKE PATIENTS TREATED BY THROMBOLYSIS THROMBECTOMY

Download or read book INFLAMMATORY AND HEMOSTASIS BLOOD BIOMARKERS PREDICT THE CLINICAL OUTCOME OF STROKE PATIENTS TREATED BY THROMBOLYSIS THROMBECTOMY written by S andrine Delbosc and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Introduction In ischemic stroke, the lack of blood flow triggers local inflammation and vascular injury, which further promotes tissue remodeling and angiogenesis. The aim of the present study was to investigate whether biomarkers of inflammation, hemostasis, vascular injury, and tissue remodeling measured in the plasma of stroke patients could stratify stroke patients concerning their clinical outcome. MethodsWe included 153 patients consecutively admitted for ischemic stroke treated by thrombolysis with or without thrombectomy. Blood was sampled at admission and 21 biomarkers related to inflammation (MPO, Osteopontin, CCL2/MCP-1, Adiponectin, Resistin, CD40L), hemostasis (uPA/Urokinase, alpha 2-macroglobulin, Serpin C1/Antithrombin-III, Serpin E1/PAI-1, Kallikrein 6/Neurosin, CD40L), endothelium activation (VCAM-1, ICAM-1 , CD31/PECAM-1, CD40L, Neuropilin-1), and tissue remodeling (Osteopontin, MMP-2, MMP-3, MMP-9, MMP-13, Cathepsin S, Cystatin C, Neuropilin-1) were assayed by a quantitative Luminex multiplex technic. ResultsPatients with higher NIHSS at admission presented higher levels of the inflammatory biomarker MPO, the hemostasis biomarker uPA/Urokinase and the endothelial activation biomarker VCAM. Logistic regression analyses correlated levels of uPA/Urokinase to NIHSS at arrival (p=0.0037), suggesting the major role of hemostasis in initial stroke gravitymRS>1 at 3 months was correlated to higher levels of MPO, the endothelial activation biomarkers ICAM-1, VCAM-1 and the tissue remodeling biomarkers Osteopontin, Cathepsine S, and lower levels of MMP-13. Logistic regression analyses correlated mRS 0-1 at three months to levels of Cathepsine S (p=0.0201) and VCAM-1 (p=0.0048). The area under ROC curve of both biomarkers was 0.75. Symptomatic hemorrhagic transformation, defined according to ECASS-3 criteria, was correlated to MMP-9 (p=0.0382)ConclusionsAs expected, initial NIHSS was related to early hemostasis response. Importantly, initial inflammatory response, endothelial activation and tissue remodeling biomarkers significantly impact the three-month outcome and the hemorrhagic transformation.

Book Evidence based Management of Stroke

Download or read book Evidence based Management of Stroke written by José Biller, and published by tfm Publishing Limited. This book was released on 2011-06-01 with total page 376 pages. Available in PDF, EPUB and Kindle. Book excerpt: Winner of a HIGHLY COMMENDED AWARD in the Neurology category of the 2012 BMA Medical Book Competition. More than five million people die from stroke each year. Stroke is also the leading cause of adult disability, affecting over 20 million people worldwide, with two-thirds of stroke victims left permanently disabled. We now know that speed of intervention is critical and patient outcomes are determined by a very narrow therapeutic window, during which physicians need to be able to make their choice of treatment rapidly, based on the best evidence available. The purpose of this book is to review that evidence, for both medical and surgical therapies as well as preventative strategies, drawing on the expertise of leading international authorities to distinguish the issues for which there is a consensus on best practice from those for which the evidence remains inconclusive. For example, for patients with acute ischemic stroke the first choice is intravenous thrombolytic therapy, but does a treatment window of 0-3 hours deliver outcomes significantly better than one of 3-4.5 hours? Arterial hypertension is the single most important modifiable risk factor for ischemic stroke, but what are the effects of blood pressure reduction among acute stroke patients? How effective is blood pressure lowering in primary intracerebral hemorrhage? What are the arguments in favour of continuation versus temporary discontinuation of antihypertensive therapy immediately after acute ischemic stroke? Endovascular procedures (carotid angioplasty/stenting) are increasingly important but what do the data tell us about their safety? Or durability? How do they compare with carotid endarterectomy? The management of unruptured intracranial aneurysms is rife with controversies; clip, coil, wait and watch? The authors examine the evidence for and against. Other sections examine the impact of various risk factors, and special populations. A section on stroke after cardiovascular surgeries reviews the incidence and management of stroke following a whole range of procedures. Another examines special risks during pregnancy. Lastly, the book reviews our understanding of childhood stroke and presents current best practice recommendations for this most challenging group of patients.

Book THE ASSOCIATION OF HEMOSTASIS SYSTEM GENES WITH THE DEVELOPMENT OF ISCHEMIC STROKE IN MEN WITH EARLY ONSET OF THE DISEASE

Download or read book THE ASSOCIATION OF HEMOSTASIS SYSTEM GENES WITH THE DEVELOPMENT OF ISCHEMIC STROKE IN MEN WITH EARLY ONSET OF THE DISEASE written by Ekaterina Kimelfeld and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Background and AimsThe etiology and pathogenesis of the ischemic stroke (IS) in young people is differ from those in older age group. The results of research devoted to the effect of the polymorphisms of hemostasis system genes on the risk of development an IS are ambiguous. The aim of our research is to study an influence of polymorphisms hemostasis system genes on the risk of ischemic stroke (IS) in patients of the Slavic population under the age of 50 years.MethodSeventy three males (aged 23-49 years) with IS and 186 healthy males (aged 19-49 years) were examined. The diagnosis of stroke was confirmed by neuroimaging (CT or MRI) in all patients. Polymorphic alleles of GP1BA, F2, F5 were studied by a real time polymerase chain reaction using the TaqMan technology. ResultsAn analysis of the GP1BA -5T/C polymorphism showed that it was associated with the risk of IS in young men. The -5C allele and the genotypes -5T/C and -5C/C (p = 0,02; OR=1,84; CI [1,13; 2,99]) indicated an elevated the risk of IS. However, no significant association between the F5 G1691A polymorphism and the F2 G20210A polymorphism and the risk of IS was observed (p u02c3 0.05).ConclusionOur findings suggest that the GP1BA -5T/C polymorphism is associated with the risk of IS in u201cyoungu201d men, whereas the F5 G1691A and the F2 G20210A polymorphisms are not associated with the risk of IS in this group .
