Book excerpt: This presentation describes various aspects of the regulation of tissue oxygenation, including the roles of the circulatory system, respiratory system, and blood, the carrier of oxygen within these components of the cardiorespiratory system. The respiratory system takes oxygen from the atmosphere and transports it by diffusion from the air in the alveoli to the blood flowing through the pulmonary capillaries. The cardiovascular system then moves the oxygenated blood from the heart to the microcirculation of the various organs by convection, where oxygen is released from hemoglobin in the red blood cells and moves to the parenchymal cells of each tissue by diffusion. Oxygen that has diffused into cells is then utilized in the mitochondria to produce adenosine triphosphate (ATP), the energy currency of all cells. The mitochondria are able to produce ATP until the oxygen tension or PO2 on the cell surface falls to a critical level of about 4–5 mm Hg. Thus, in order to meet the energetic needs of cells, it is important to maintain a continuous supply of oxygen to the mitochondria at or above the critical PO2 . In order to accomplish this desired outcome, the cardiorespiratory system, including the blood, must be capable of regulation to ensure survival of all tissues under a wide range of circumstances. The purpose of this presentation is to provide basic information about the operation and regulation of the cardiovascular and respiratory systems, as well as the properties of the blood and parenchymal cells, so that a fundamental understanding of the regulation of tissue oxygenation is achieved.

Therapy must proceed from profound knowledge of the multi variant physiological events in order to influence shock, sepsis and organ failure. Although numerous possibilities for therapy have arisen from pharmaceutical research in recent years, they are beyond the scope of this book and are not discussed here. To gain a better understanding of the pathophysiological events it was necessary to examine and to describe different models that simulate and reproduce these events. Here we describe the causative agents (shock) and the consequences (sepsis, organ failure) in two main sections, divided on the basis of their pathophysiology.

The 1992 meeting took place in Obermayerhofen, a small but charming castle in the soft green hills of Eastern Styria in the South-Eastern part of Austria. The castle was built in 1130, restored and turned into a hotel in 1977. The renaissance court yard and the generous and exclusive decors make it one of the most beautiful castles in Austria. The frank beauty and hospitality of this area acted as a catalyst to crystalize the thoughts of this interdisciplinary group of scientists as they discussed new findings in the cytokine area.

Apparently our society associates "learning" only with known, long-gathered information. This symposium, I believe, will generate new information about shock through the integration of knowledge of many investi gators, who have come to share a common meeting ground. Hopefully, the worth of this and similar endeavors will gain the recognition and, more important, the support of the public. In the next decade or so, I believe that shock research workers will be channeling their efforts more and more into investigating the repair of damaged cells. The individual cell and its products must be scrutinized just as carefully as we examine the whole organism or its collections of cells. Shock is an exciting area of research, but it involves years of painstaking work. Moreover, the society which will reap its fruits must be persuaded to share the burden of its support. John A. Through their extensive experience, the authors’ consider that the lifesaving feature of MB is still underestimated. For this reason they outline key concepts of MB, mentioning essential experimental data, yet focusing mainly on its application in the care of critically ill patients. In an easy-to-read format, the 18 chapters were organized in a didactically structured order, distributed in four parts. The first part introduces the general conceptual aspects and physiology of the endothelium. The second part presents the particularities of endothelial dysfunction in different types of shock. Methylene blue is discussed in detail in the third part. In the fourth part, particular situations of endothelial dysfunction, such as heart surgery and organ transplantation, are described. Additionally, the two complementary subjects of acid-base balance and the therapeutic use of inhaled nitric oxide are mentioned. Finally, looking for therapeutic alternatives for what we classify as "vasoplegic endothelial dysfunction", MB remains promising as an adjuvant to sympathomimetic amines. The combination of three recently proposed concepts may be useful in obtaining better results against the high mortality rates in critically ill patients. These three concepts are "wide-spectrum vasopressors", vasopressor economy strategies, and microcirculation protection against the harmful effects of using high concentrations of amines. This book ́s translational approach will appeal to a variety of readers within the health specialties of medicine, biomedicine, physiotherapy, nursing, odontology, and basic science.
