Book excerpt: European Union (EU) legislation gives EU citizens the right to receive healthcare services in all EU Member States, as well as the right to work in a different EU Member State. This is particularly important in order to ensure the health and well-being of EU citizens and to offer healthcare workers the opportunity to relocate where labour market needs exist. According to the available data, the mobility of patients and healthcare professionals across EU borders is quite low. However, studies show a strong willingness of healthcare personnel to work in a different Member States and of patients to access services in other countries. Multiple and interrelating factors contribute to the low mobility levels of patients, including: differences in the organisation and delivery of health services; differences in reimbursement procedures; tensions between healthcare authorities on the reimbursement of treatments; lack of information concerning the services available abroad; and lengthy procedures for the authorisation of services or their reimbursement. With regards to healthcare professionals, the main factors are differences in qualifications and job titles, and in some cases lengthy recognition of qualifications. Access to cross-border healthcare is particularly important for citizens living in cross-border regions, where access to healthcare in the neighbour country might be the best option in terms of proximity of care, access to healthcare services lacking the home country or to better quality services. Similarly, healthcare professionals can have access to more job opportunities and in some cases better working conditions; most importantly they can respond to the labour market needs of the neighbouring country. The case of the Torne Valley (or Tornio-Haparanda Region), situated in the Northern part of Finland and Sweden, is used to illustrate the obstacles of different healthcare systems in cross-border context. The Torne Valley region is characterised by a long history of cooperation, also in healthcare. However, some obstacles persist to cross-border mobility, both in terms of access to healthcare for patients and the mobility of health professionals. Differences in coverage and reimbursement of services, imbalances in the mobility of patients and professionals, lack of skilled personnel in some areas, difficulties in the use of IT and telemedicine; difficulties in sharing information between health centres and inadequate IT infrastructures are some of the issues that the case of the Torne Valley highlights. Some of these problems have been the object of cooperation and have been partly solved while others persist and will need further interventions and initiatives to be effectively addressed. The impact of these obstacles is not easily quantifiable, however, three main issues are worth noting: potential shortages and mismatches in the healthcare labour market; missed opportunities for increasing healthcare quality and healthcare delivery efficiency. These are highlighted in the report.

The information and analysis presented in the study can be of considerable use to policy-makers and those with an interest in key aspects of cross-border health care to accompany or follow this process.

Book excerpt: The 2011 Cross‐border Healthcare Directive seeks to ensure EU patients’ rights to access safe and high‐quality healthcare, including across national borders within the EU. These rights are also intended to facilitate closer cooperation between Member States on eHealth and the treatment of rare diseases. We concluded that although EU actions in cross-border healthcare enhance Member States’ collaboration, the benefits for patients were limited. We found that despite the progress made on providing EU citizens with information on cross-border healthcare, in some areas this information remains difficult to access. We identified weaknesses in the Commission’s strategic planning and project management. We make recommendations focusing on the Commission’s support for national contact points, the deployment of cross-border exchanges of health data, and EU’s actions in the field of rare diseases.

Book excerpt: This study aimed to provide an overview of cross-border patient mobility and the reimbursement system used for planned healthcare treatment in selected EU Regions, in order to complement the data on cross-border healthcare collected for the purposes of Directive 2011/24/EU (reporting requirements under Article 20) and the Social Security Coordination Regulations 883/2004 and 987/2009. The study objectives were to carry out four case studies of cross border patient flows in selected EU Regions, specifically to gather quantitative/qualitative data on: cross border patient flows; types of treatment sought; methodological difficulties associated with data collection; and provide recommendations to improve future data collection. The case studies focused on Meuse Rhein region (DE/BE/NL), Grand Est region of France (FR)-Luxembourg, Lower Austria/Bohemia/Slovakia (AT/CZ/SK), and general patient flow between Poland and Czechia. The methodology involved a literature review, baseline data analysis, stakeholder mapping (over 200 respondents), qualitative focus groups/ interviews, and quantitative data analysis. Overall, the study found that current patient mobility data lacks consistency and is often purely financial in nature; some Member States report no data. Improved data collection requires a multi-level stakeholder approach. The COVID-19 Pandemic highlighted that good data is an essential enabler for well-functioning EU health systems and underpins a legal commitment of Member States and the EU to supporting patient rights in cross- border healthcare. The Directive remains an important enabler for patient mobility for planned care including in the context of post-COVID-19 recovery and resilience. Study recommendations include strategic approaches to improve cross-border patient mobility data collection that involve member states, healthcare insurers, regional and subnational stakeholders; which include collection of data on types of care accessed; and which can be piloted in border regions as recognised innovative laboratories for EU integration through cross-border co-operation.

Book excerpt: The increasing number of refugees, asylum seekers, and irregular migrants poses a challenge for mental health services in Europe. This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.
