The book reports findings of a study conducted by the author which provide statistical hinges to the qualitative findings of an earlier study by Pfeiffer and others on the same theme. Having used data from 43 Sub-Saharan African countries collected on a number of variables over a period of 8 years (2002 - 2009), the author applied correlations, regressions and factor analysis to analyse the data. The findings show that the integration of HIV/AIDS services into Primary Health Care has substantially contributed to the reduction in the HIV prevalence rate in Mozambique. He concludes by projecting some possible policy actions to maximize any perceived benefits of the integration program in Mozambique and other countries in similar situations.

Escalating costs and poorly constructed strategies are weakening the efficiency and effectiveness of service delivery in health care. This has a negative impact on value creation and internal processes as critical elements such as human capital, information and organisational capital are not aligned with strategies and roll-out plans for HIV/Aids interventions. This research study therefore questioned the extent to which these strategies have impacted on the roll-out plans for HIV/Aids interventions, and investigated if the utilisation of public-private partnerships (PPP) resulted in applying fiscal reponsible mechanisms in health care reforms (effectively, efficiently, economically and equitably (4Es)). Trends in the new public management (NPM) movement inspired a shift towards business-like reforms and saw PPP as a mechanism that improved efficiency and effectiveness in service delivery as it offered the promise to strengthen the capacity of government policy. The study aimed to put forward value-creating strategies and develop a best practice model that strengthened government's policy capacity by providing efficient, effective, economical and quitable service in health care and thereby improving strategies that impact on the roll-out plans for HIV/Aids. This comparative study comprised four international case studies (developed and developing countries) which presented benchmarks against which the performance of the national case study was measured. A better understanding of the influence which different ideologies had on the architecture of international and global governance structures was gained as it highlighted and compared the key issues that influenced strategies for HIV/Aids intervention between the developed and developing countries. Results of the study indicated that there are conflicting views between government departments in how to achieve value-for-money outcomes and their application of risk allocation. The conflicting views widened the gap between public and private governance structures and relations. The focus of the PPP definition as applied in the national context of health care is not percieved as being health-specific or effective as it excludes some forms of interactions occurring in the health sector. PPP goals emphasised efficiency, affordability and value-for-money approaches, while health care goals emphasised the interest of the patient and public health.

Book excerpt: In the hard-hit countries by HIV/AIDS, the epidemic has eroded the development gains made in past decades. The spread of HIV is heterogeneous in Sub-Saharan African courtiers, with different peaks which vary geographically and in terms of their distribution among social or economic groups. Thus, there is a pressing need to promote researches and policy making not only at biomedical and behavioural level but also at the underlying social, cultural and economic causes of the epidemic. Although HIV prevention policies and interventions should be based on evidences that show how the disease spreads in the community, however, very few studies have focused on the susceptibility of pastoralists, and therefore there is a great need to acknowledge and document the risk environments which contribute to the spread of HIV/AIDS in pastoral areas to curb the spread of the epidemic. Based on the research in Shinile district of Somali region in Ethiopia, this study aims to identify the risk for the spread of HIV among Shinile pastoralists by looking at the susceptibility factors in the community. In the fieldwork in Ethiopia interviews with 20 people and 3 focus group discussions (FGDs) in the community were conducted. As a tool for identifying susceptibility factors, the study adapted the framework by Tony Barnett and Alan Whiteside and analyzes the research results in terms of identified factors. The research shows that although there is lack of reliable data on the prevalence of the epidemic in the district, HIV/AIDS is a clear and present danger to Shinile pastoral communities. The awareness on HIV/AIDS level in the community are low, the communities have not accepted the presence and the threat associated with the epidemic. The study reveals that migration to urban centers particularly to the neighboring centers with high HIV/AIDS prevalence is likely to be one of the factors which contribute to the spread of the disease to the low risk population. The low awareness and misconceptions on HIV/AIDS compounded by the low access to preventive health services also poses a major risk. In addition, there is a general absence of self- protection among the community, a situation perpetuated by religious and traditional norms. The study also shows that women faces a heightened risk of HIV infection as they have low awareness level due to the ‘women stay home culture’ among pastoralists which limited their access to HIV/AIDS information. Moreover, gender related norms which put women at risk like polygamous marriage, marriage by inheritance, early marriage and female genital mutilation (FGM) are also widely practiced. The striking finding of the research is that being part of pastoral societies, Shinile communities have no experience of violent conflicts among their clan or the neighboring pastoral societies which is known to be common among pastoralists in general. Traditional patterns of sexual networking like wife sharing and multiple sexual partners (pre-marriage and in-marriage) which are promoted in some pastoral societies are not widely acknowledged among Shinile pastoral communities. Finally, the study presents both short term and long term recommendations which contribute towards prevention of HIV spread among the study area. The short term include i) better understanding of the status of the epidemic ii) organise targeted awareness raising campaigns iii) community empowerment to challenge norms and culture on gender issues iv) enhance the involvement of non-governmental organisations (NGOs) and community based organisations( CBOs) in response to the epidemic and v) improve access to preventive health services. The long term recommendations are: i) Policy review on development of pastoral areas and ii) Strengthening and diversifying of pastoral livelihoods.

Book excerpt: This paper draws on two reviews commissioned by the UK Department for International Development in 2006-2007 that explore progress in linking HIV prevention and maternity services in sub-Saharan Africa. Although pilot and demonstration projects have been successful, progress in scaling up PMTCT has been slow, reaching just 11% of pregnant HIV positive women in much of Africa, less than half the percentage of coverage achieved by antiretroviral treatment programmes for adults in need. Despite ongoing efforts to promote comprehensive approaches, significant policy, financing and institutional barriers, and weak co-ordination and leadership, continue to hamper progress. Maternal health services face human and financial resource shortages which affect their capacity to integrate HIV prevention. Both HIV and maternal health programmes often receive targeted financial and technical assistance that does not take the other into account. However, proposals in 2007 from a number of countries to the Global Fund to Fight AIDS, TB and Malaria incorporate sexual and reproductive health programming that will have an impact on HIV, including certain maternity services. Moreover, Botswana, Kenya and Rwanda have shown that progress can be made where national commitment and increased resources are enabling maternal and newborn care to address HIV.
