Book excerpt: The dearth of reliable data on the levels and causes of mortality in poorer regions of the world continues to plague efforts to build a solid evidence base for health policy, planning, monitoring, and evaluation. As a partial solution to this problem, verbal autopsy has become the primary source of information about causes of death in populations lacking vital registration and medical certification. The purpose of this manual is to disseminate new standard data collection and cause-of-death assignment resources for verbal autopsy, and to provide some general guidelines for their use. The manual includes verbal autopsy questionnaires for three age groups, cause-of-death certification and coding guidelines for applying the International statistical classification of diseases and related health problems (ICD-10) to verbal autopsy and a cause-of-death list for verbal autopsy with corresponding ICD-10 codes. These resources are the consensus products of a three-year effort by an expert group led by WHO, consisting of researchers, data users, and other stakeholders under the sponsorship of the Health Metrics Network. They are intended to serve the needs of various users and producers of mortality information, including researchers, policy-makers, program managers, and evaluators.

The document provides guidance on possible analysis and visualization of the indicators, including considerations for interpreting and using the data for decisionmaking. An annex on data quality considerations for MNCAH managers provides suggestions for reviewing and interpreting routine health facility data through a quality lens. Accompanying this guidance are a series of presentations and exercises, including a facilitator guide, that can be used in workshops to strengthen capacity of analysis, interpretation, and use of data by MNCAH managers. The target audience for the guidance and accompanying materials are ministry of health staff working on MNCAH programmes and monitoring and evaluation activities at national and subnational levels; health workers; and partner organizations involved in supporting MNCAH programmes and monitoring. Technical Discussions on Covering every birth and death: Improving civil registration and vital statistics (CRVS), were conducted in WHO Regional Office for South-East Asia, New Delhi, on 16-17 June 2014. To provide unified direction and political support to prioritize CRVS, the Regional Office has developed a regional strategy for strengthening the role of the health sector in improving CRVS. The regional strategy and the recommendations arising out of the technical discussions were submitted to the Sixty-Seventh Session of the Regional Committee in Dhaka, Bangladesh, on 10-12 September 2014. During the two-day technical discussions, participants provided feedback on the regional strategy for strengthening the role of the health sector in improving CRVS based on which the strategy has been finalized. It was agreed that the strategy would focus on five strategic areas as listed below: Strategic Area 1: Legal and organizational framework for CRVS; Strategic Area 2: Political commitment and intersectoral collaboration for national capacity building, partnership, advocacy and outreach; Strategic Area 3: Birth and death registration--completeness and coverage; Strategic Area 4: Recording cause of death, ensuring completeness and quality; Strategic Area 5: Creating demand for health and vital statistics, enabling service delivery and planning through use in: (a) evidence-based decisionmaking and (b) linkage to other activities. Though the multisectoral processes required to strengthen the CRVS system as a whole can be complex and take time, the health sector can move immediately to accelerate those parts of the CRVS systems that are the sole responsibility of health. Further, it was also agreed that the health sector can play an instrumental role to support the creation of demand for registration, where possible, through linking with health services such as immunization and universal health coverage."--Page 4 of cover

Book excerpt: The Republic of Indonesia, home to over 240 million people, is the world's fourth most populous nation. Ethnically, culturally, and economically diverse, the Indonesian people are broadly dispersed across an archipelago of more than 13,000 islands. Rapid urbanization has given rise to one megacity (Jakarta) and to 10 other major metropolitan areas. And yet about half of Indonesians make their homes in rural areas of the country. Indonesia, a signatory to the United Nations Millennium Declaration, has committed to achieving the Millennium Development Goals (MDGs). However, recent estimates suggest that Indonesia will not achieve by the target date of 2015 MDG 4 - reduction by two-thirds of the 1990 under - 5 infant mortality rate (number of children under age 5 who die per 1,000 live births) - and MDG 5 - reduction by three-quarters of the 1990 maternal mortality ratio (number of maternal deaths within 28 days of childbirth in a given year per 100,000 live births). Although much has been achieved, complex and indeed difficult challenges will have to be overcome before maternal and infant mortality are brought into the MDG-prescribed range. Reducing Maternal and Neonatal Mortality in Indonesia is a joint study by the U.S. National Academy of Sciences and the Indonesian Academy of Sciences that evaluates the quality and consistency of the existing data on maternal and neonatal mortality; devises a strategy to achieve the Millennium Development Goals related to maternal mortality, fetal mortality (stillbirths), and neonatal mortality; and identifies the highest priority interventions and proposes steps toward development of an effective implementation plan. According to the UN Human Development Index (HDI), in 2012 Indonesia ranked 121st out of 185 countries in human development. However, over the last 20 years the rate of improvement in Indonesia\'s HDI ranking has exceeded the world average. This progress may be attributable in part to the fact that Indonesia has put considerable effort into meeting the MDGs. This report is intended to be a contribution toward achieving the Millennium Development Goals.
