Book excerpt: Pediatric Inflammatory Bowel Disease (IBD) has the potential to affect growth, pubertal and psychosocial development. Limited knowledge and adherence can be problematic for both children with IBD and their caregivers. Inflammatory bowel disease experts recommend annual physical exams, gastroenterologist appointments, vaccinations, eye screenings, and laboratory work. At the time of implementation, the project site did not have a standardized process for health maintenance education for IBD patients/caregivers. The primary purpose of this study was to examine the impact of a live online educational intervention on caregivers of pediatric IBD patients’ level of knowledge and health maintenance adherence. Secondary aims included: (1) to compare various demographic characteristic with IBD knowledge scores and (2) to evaluate the educational session. IBD knowledge was measured using the IBD-KNOW tool before the intervention and one-week and six-weeks after the intervention. Caregivers were asked about adherence to and plans for scheduling health maintenance appointments pre-intervention and six-weeks later, respectively. Evaluation of the education was measured one-week after the session. Caregivers (N = 24), most college-educated, participated in one 30-minute education session. Results indicated a significant 13.7% (p =.002) increase in mean knowledge scores from pre-intervention to the six-week follow up. All caregivers (n = 14) reported they planned to schedule the recommended appointments (i.e., eye exam, blook work, vaccinations, and provider appointments) during the upcoming year. However, during the prior year, five of 24 caregivers reported the child did not have an eye exam. Results revealed a significantly higher knowledge level at baseline for those with a family history of IBD (t = 2.25, p = .035). Findings indicated no significant difference between knowledge scores in caregivers of children with Crohn’s disease versus Ulcerative colitis. Caregivers (n = 21) were overwhelmingly satisfied with the education; found it helpful and understandable. Online education may help to improve IBD-related knowledge, health maintenance plans, and prevent disease related complications. Future research could provide education directly to IBD patients in their teen-years and examine the impact on knowledge and adherence.

Book excerpt: OBJECTIVES: Inflammatory bowel disease (IBD) is a chronic condition with increasing pediatric incidence. Transition from pediatric to adult care is associated both with disruption of links with familiar caregivers and differences in health care delivery. This study explored the impact of the transfer from pediatric to adult care on health services utilization for pediatric-onset IBD patients. APPROACH: A population-based retrospective cohort study identified all children diagnosed with IBD 1994-2008 and treated by pediatric gastroenterologists from within Ontario health administrative data. Self-controlled case series analyses compared health services utilization in the 2 years before and after transfer to adult gastroenterologists, with a 6-month washout period at transfer. Outcome evaluated included IBD related/specific hospitalization, emergency department (ED) utilization, outpatient visits, and laboratory utilization. Relative incidence (RI) in the post-transfer was compared to pre-transfer periods using Poisson regression analysis controlling for transfer starting age. Analyses were stratified by IBD types: Crohn's disease (CD) and ulcerative colitis (UC). RESULTS: 536 patients were included in the study (388 CD, 148 UC). ED utilization rate was higher after transfer for both CD (RI 2.12, 95% CI 1.53-2.93) and UC (RI 2.34, 95% CI 1.09-5.03). Other increases in health care provision included outpatients visits (CD: RI 1.56, 95% CI 1.42-1.72; UC: RI 1.48, 95% CI 1.24-1.76), and laboratory investigations (CD: RI 1.43, 95% CI 1.26-1.63; UC: 1.38, 95% CI 1.13-1.68). There was no statistically significant change in hospitalization (CD: RI 0.70, 95% CI 0.42-1.18; UC: RI 2.41, 95% CI 0.62-9.40). Sensitivity analysis revealed similar results when only the first year post-transfer period was assessed. CONCLUSIONS: In the largest study to date examining the transfer from pediatric to adult IBD care, health services utilization increased significantly in the two years after transfer for measures of access to health care providers. Understanding causation and rigorously evaluated transition programs may help to better manage the resources and meet the needs of pediatric-onset IBD patients.

Book excerpt: Objective: To summarize rates of supplement adherence in youth with inflammatory bowel disease (IBD) and to identify predictors and outcomes of supplemental adherence. Methods: 49 adolescents (ages 11-18) participated. Youth completed six monthly assessments of adherence and supplement knowledge. Youth and parents completed questionnaires. Medical record reviews provided medical and laboratory data. Results: Mean supplement adherence rates ranged from 32% to 44%. Predictors of supplement adherence included higher knowledge (for multivitamin, iron, and calcium adherence) higher family involvement (for iron and calcium adherence), and greater inflammation (via a hematological index) for multivitamin adherence. Few relationships between supplement adherence and inflammation, growth, or nutritional outcomes were found. Conclusion: Supplement adherence is problematic in pediatric IBD, and predictors of adherence tended to be psychosocial rather than biological variables. Future research with larger sample sizes and assessments of supplement adherence that do not rely solely on patient self-report are important next steps.

Book excerpt: Patient activation may improve medication adherence by equipping patients with skills and knowledge about their disease. There is evidence of a positive relationship between activation and quality of life in IBD patients. However, the impact of a tailored patient activation intervention (PAI) among this group of patients has not been investigated. Activated patients are motivated to be partners in managing their health and would most likely remain in remission for longer periods (in the case of IBD patients). The purpose of this study was to evaluate the effectiveness of a Patient Activation Intervention (PAI) tailored to the needs of individual IBD patients in improving patient activation levels (primary outcome) as well as medication adherence, and satisfaction with medical care. A pre-test, post-test, control group design was employed. IBD patients, who met the inclusion criteria, were selected from medical records via convenience sampling, were consented and completed a baseline survey. They were then categorized into one of four patient activation stages based on responses to the 13-item patient activation measure (PAM-13) at baseline. During their office visit, intervention group patients (N=23) were given a tailored PAI based on their stage which consisted of an information booklet and focused discussion with the gastroenterologist, while the control group (N=27) received usual care. For those who completed both surveys (N=20 intervention group and N=21 control group), baseline scores were compared to 1-month post-intervention scores for changes in patient activation score/stage, medication adherence, and satisfaction with medical care using Chi square and T tests. The majority of participants were Caucasian (88%), female (64%), college graduates (56%), and had Crohn’s disease (59.2%). Averaging across both groups, females had a significantly higher (p=0.04) mean activation score (Mean=70.9, SD=15.4) than males (Mean=60.9, SD=10.7) at baseline and this trend was the same post-intervention (75.6 females vs 64.4 males; p=0.03). The mean activation score difference pre- vs post-intervention for the intervention group (Mean=6.8, SD=12.0) was higher than that of the control group (Mean=1.9, SD=12.5; p=0.21). There was no significant difference in medication adherence or satisfaction scores pre- vs post-intervention for either group. Tailored PAIs have the potential to provide customized medical care to patients (especially those with chronic conditions) and to involve them in their disease management, leading to improved health outcomes.

Purpose: The purpose of this study was to determine the impact of post-transfer health care setting (academic vs. community) on emergent health resource utilization. Methods: This was a population-based retrospective cohort study using health care administrative data from Ontario, Canada. Patients with PO-IBD were identified and health resource utilization during a 2-year pre-transfer period, transfer of care period and 2-year post-transfer period was analyzed. Comparator groups included academic vs. community vs. loss-to-follow-up. The primary outcome comprised Emergency Department (ED) utilization. Secondary outcomes included hospitalizations, surgeries, ambulatory visits, endoscopic and radiological investigations. Results: There were no significant differences found in ED use, ambulatory care visits (aside from the lost to follow-up group), hospitalizations, endoscopic or radiological procedures. Conclusion: Post-transfer healthcare setting does not impact emergent health resource utilization in the post-transfer period.
