Book excerpt: In 2015, building on the advances of the Millennium Development Goals, the United Nations adopted Sustainable Development Goals that include an explicit commitment to achieve universal health coverage by 2030. However, enormous gaps remain between what is achievable in human health and where global health stands today, and progress has been both incomplete and unevenly distributed. In order to meet this goal, a deliberate and comprehensive effort is needed to improve the quality of health care services globally. Crossing the Global Quality Chasm: Improving Health Care Worldwide focuses on one particular shortfall in health care affecting global populations: defects in the quality of care. This study reviews the available evidence on the quality of care worldwide and makes recommendations to improve health care quality globally while expanding access to preventive and therapeutic services, with a focus in low-resource areas. Crossing the Global Quality Chasm emphasizes the organization and delivery of safe and effective care at the patient/provider interface. This study explores issues of access to services and commodities, effectiveness, safety, efficiency, and equity. Book excerpt: In Thailand the Ministry of Public Health has launched many public health insurance schemes in order to provide health care for all citizens. These schemes have varying purposes, target populations, and methods of reimbursement for health facilities. In some schemes health facilities benefit financially, while in other schemes health facilities incur losses. Three major public health insurance shemes, which represent more than half of Tahiland's total population, were investigated in this study. These were the Civil Servant Medical Benefit Scheme (CSMBS), the Health Card Scheme (HCS), and the Social Welfare Scheme(SWS). The research reported here attempts to reveal how insurance scheme membership might be related to the quality of hospital care provided to patients. The sutdy was conducted in two large public hospitals drawing on data from caesarean section and asthma patients. Clinical processes and outcomes, as well as non-clinical processes and outcomes of hospital services were assessed. Data were both quantitative and qualitative, and were collected from patient questionnaires, in-depth semi-structured interviews of patients, direct observation and medical records.

Book excerpt: The purposes of this study are to: (1) assess the changes in the health insurance coverage before and after the introduction of the 30-baht health scheme, (2) examine service utilization of the community hospitals, and (3) compare the patterns of hospital utilization between different population subgroups in one of the Northern provinces of Thailand. An adapted framework based on the framework by Aday, Flemming, and Andersen (1984) and the framework from Classifying Topics and Issues in Health Services Research by Aday, Begley, Lairson, and Slater (1998) was used to analyze service utilization pattern. The hospital dataset was obtained from the six community hospitals. The descriptive findings for changes in health insurance coverage indicated: (1) a decline in hospital use by uninsured and Health Card beneficiaries; (2) an increased hospital use by the 30-baht beneficiaries; (3) the remaining number of uninsured individuals after the implementation of the 30-baht health. Hospital use increased after the scheme was implemented. Admission rate/1000, in-patient day/1000, and out-patient visits/1000 increased 26, 14, and 38 percent for the older age group and 23, 9, 21 percent for the female group, respectively. The shift of in-patient care to the less costly out-patient setting appeared in most of the community hospitals in this study. The patterns of service utilization varied according to age and gender over time. The pattern of service utilization of the younger adults was not consistent compared across male and female populations; however, the pattern of service was consistent for both the older males and the older females.
