Hospitals are at the forefront of policy discussions in Brazil. The discussions reflect their promise as centers of technological innovation and medical advances as well as widespread concern about their cost and quality. Brazilian hospitals are important to many people for many different reasons. What makes hospitals important is easy to understand. What makes hospitals deliver quality care efficiently or not is much harder to grasp. Can Brazil improve the performance of its hospitals? It presents new evidence on the limited coverage of existing programs and emphasizes the challenges caused by high informality for achieving universal social protection for old age income, for health, for unemployment risks and for anti poverty safety nets. It identifies interaction effects between SP programs and the behavioral responses of workers, firms and social protection providers, which can further undermine efforts to expand coverage, summarizing evidence from recent work across the region. It argues for a re-design of financing to eliminate cross subsidies between members of contributory programs and subsidies that effectively tax income from formal employment. Instead, it advocates well-targeted, tax-funded tapered subsidies to provide incentives to the savings efforts of low income workers, coupled with an effective safety net for the extreme poor who have no capacity to contribute to financing their own social protection arrangements. It also argues for the consolidation of programs and harmonization of benefits packages across different insurers. The book develops an overall conceptual framework and presents in-depth analysis of the main SP sectors of pensions, health, unemployment insurance and safety net transfers.

Yet the complex and fascinating history of the Caribbean, borne of the ways European colonialism combined with slavery, indentureship, migrant labour and plantation agriculture, led to the emergence of new social and cultural forms which are especially evident the area of health and medicine. The history of medical care in the Caribbean is also a history of the transfer of cultural practices from Africa and Asia, the process of creolization in the African and Asian diasporas, the perseverance of indigenous and popular medicine, and the emergence of distinct forms of western medical professionalism, science, and practice. This collection, which covers the French, Hispanic, Dutch, and British Caribbean, explores the cultural and social domains of medical experience and considers the dynamics and tensions of power. The chapters emphasize contestations over forms of medicalization and the controls of public health and address the politics of professionalization, not simply as an expression of colonial power but also of the power of a local elite against colonial or neo-colonial control. They pay particular attention to the significance of race and gender, focusing on such topics as conflicts over medical professionalization, control of women’s bodies and childbirth, and competition between ‘European’ and ‘Indigenous’ healers and healing practices. Employing a broad range of subjects and methodological approaches, this collection constitutes the first edited volume on the history of health and medicine in the circum-Caribbean region and is therefore required reading for anyone interested in the history of colonial and post-colonial medicine.

The countries' response has been a momentous achievement. Now, a third big step is being taken with the publication of this book. It underlines WHO's unwavering commitment to supporting the implementation of the Code and provides a wide range of detailed examples from the countries themselves of how they are tackling the many complex issues involved. It provides not just numerous insights into progress but also gives other countries valuable guidance and recommendations on how they, too, can implement the Code. Countries are encouraged to learn from the shared experiences, domestic solutions and multi-lateral cooperation described in this book, and move ahead to support and advance the Code's aspirational principles.
