A concerning issue is increased nurse to patient ratio; not only does this effect patient outcomes as well as nurse outcomes. This paper will discuss the topic of nurse to patient ratio and the impact it has on delivery of care. Recent research has shown that an increased nurse to patient ratio impacts patient care and outcomes. Studies have concluded increased nurse to patient ratio can cause mortalities, never events, readmissions, and decreased patient and nurse satisfaction. According to the National Institute of Health Also increasing the nurse's workload by one patient increases the likelihood of patient death by 7 percent (2014). Decreasing nurse to patient ratio will improve patient care as well as prevent complications associated with nurse sensitive outcomes. The Department of Professional Employees reports that increased nurse to patient ratio Besides the occupational hazards caused by understaffing, numerous studies show a correlation between inadequate nurse staffing and poor patient outcomes. High nurse-to-patient ratios are associated with an increase in medical errors, as well as patient infections, bedsores, pneumonia, MRSA, cardiac arrest, and accidental death (2014). An implementation plan was created that will decrease nurse to patient ratio from 6:1 to 4:1 on telemetry units. An evaluation plan was created to educate stakeholders and gather baseline data to measure the effectiveness of implementing this change. This research paper will discuss solution, implementation plan, theory, evaluation plan, disseminating evidence, and a review of literature.

Based on different peer reviewed studies it is noted that the higher the patient load per nurse the increased risk of negative patient outcomes during hospitalization and once they are discharged home. This proposed project consists of a problem description, solution, implementation, plan evaluation, dissemination plan and review of literature. Implementing a nurse patient ratio for the medical/surgical units will increase patient quality care, nurse satisfaction and decrease negative patient outcomes (Shekelle, 2013). A new nursepatient ratio policy is recommended for the three medical-surgical units at Chandler Regional Medical Center. Implications of the research are that not all units were interviewed about the benefit of having a nurse patient ratio policy.

With an endless scarcity of nurses, nurse-patient ratios are often inadequate, causing concern for the quality of care patients receive, and the risk to patient safety. As nurses acquire responsibility for more patients, the amount of time dedicated to care decreases, causing errors in nursing judgment, increased number of adverse patient events, and a decline in the quality of care delivered to patients. A solution to this problem within the nursing profession involves developing mandated maximum nurse-patient ratios within the acute care setting, based on the acuity of care required for patients within each nursing unit. Once support and commitment from organizational leaders and nursing staff participants has been secured, mandated maximum nurse-patient ratios should slowly be introduced to nursing practice within the organization, essentially one unit at a time. After implementation has taken place, the organization must obtain input from nursing staff, patient and databases to determine the effectiveness of the change to nursing practice. Assessment tools such as surveys, questionnaires and organizational datasets can be used to retrieve this information, but each variable within the project must be considered when evaluating the success of the implementation project. Following evaluation, the results of the implementation project should be presented, not only to administrative leaders within the organization, but to members of the greater nursing community as well. Evidence-based nursing practice relies on the willingness of one person or organization to take initiative and propose changes to improve patient safety and quality of care. These transformations can then be expanded to the greater nursing communities, ultimately upholding the image of professionalism and dedication to safe, quality patient care.

As the shortage continues, nurses are expected to take on overwhelming patient workloads. Nurse to patient ratios are becoming high and it is having negative a negative impact on both patients and nurses. As, nurses are assigned to more patients the deliverance of care is at a lower quality. Patients are more likely to experience negative incidences during a hospital stay or become re-hospitalized due to issues the patients were previously hospitalized for. Re-hospitalization can occur when the patients are not treated to the best ability or are not thoroughly educated on post hospital care. The time a nurse has to spend with each patient on teaching and care is cut when the workload is at an overwhelming high. It is shown that hospitalized patient experience more falls and the risk of hospital acquired infections are increased because the nurse cannot provide an adequate amount of attention. Patient safety and delivering quality care is a priority to nurses and must be taken seriously. Nurses cannot provide satisfactory care when there is an overwhelming patient workload. As workloads increase nursing job satisfaction is decreasing. The stress and risk of burnout become easily experienced. The retention of nurses is a priority as many are leaving the profession due to inadequate work environments. Patients are hospitalized with more comorbidities than ever and acuity is at an all time high. Nurse to patient ratios must be adjusted for the intense level of care. With mandated patient ratios patient outcomes are better and nurse job satisfaction is higher.

For nurses who work in the emergency department, would a 1:4 nurse-to-patient ratio, verse the current 1:6 nurse-to-patient ratio reduce error and poor patient outcomes while increase the safety of patients and nurse satisfaction during a three month trial period? In a growing emergency department, a new trial assignment protocol will be devised to ensure adequate patient care, increase patient outcomes with increased patient safety as well as increasing nurse satisfaction. In just one of the many examples, research shows an increase of one RN per patient day was associated with a 9% reduction in patient mortality in critical care areas, as there is a consistent relationship between higher ratios and hospital-related death (Shekelle, 2013). The trial will assess the nurses' needs to aid in sufficient patient care, and attempt to secure the safety of patients. The proposal includes an implementation plan involving an additional staff position during the times of higher census and acuity while placing another nurse on-call for further coverage during mass codes, traumas, and to cover nurse absences. Staff will be educated on the process and supportive research. Evaluation will be done with data collection of patient complaints, nurse and patient surveys, and compilations of incident reports, medication errors, and investigation of poor patient outcomes. Results of the study will be presented to staff, administration and stakeholders, to determine whether the trial is effective in decreasing patient risk for adverse outcomes, and increasing nurse satisfaction for permanent protocol initiations.
