With non- compliance of proper hand washing technique from staff, nosocomial infections may occur. Nosocomial infections are infections that have developed in a patient while under medical care. These infections were not there or had been developing before the patient had been admitted into the healthcare facility, but the infection did arise within the seventy two hour time frame after being admitted into the healthcare facility. These infections that have developed in the hospital can be detrimental to a patient's recovery and many of these infections are produced by microorganisms that have become resilient to antibiotic therapy. Research has indicated that in many hospitals across the United States, shockingly hand washing compliance from healthcare staff fell below fifty percent. However, these numbers are not entirely valid due to the fact that many of the statistics are gathered by auditors which work in the healthcare facility and the workers are conscious of their existence. These figures were compared against nosocomial infections in the hospital setting and validated that with simple hand washing technique, many infections could have been avoided. Hospital acquired infections are detrimental in that they delay a patient's recovery process, extend hospital stay, development of resistance to antibiotic therapy, financial burden to the patient and to the healthcare system, disability, and even death. Hand hygiene is the most effective and basic way to prevent the rise of nosocomial infections. The way to reduce the occurrences of nosocomial infections is to provide education from current evidence based practice, skill training, mandatory in-services, and compliance with behavioral change from staff (Mehta, Y., Gupta, A., Todi, S., Myatra, S., Samaddar, D., Patil, V., Bhattacharya, P., and Ramasubban, S., 2014). This paper will discuss many options that healthcare workers may follow in order to be compliant with proper hand hygiene technique.

While ensuring consistency with the Guidelines recommendations, individual adaptation according to local regulations, settings, needs, and resources is desirable. This extensive review includes in one document sufficient technical information to support training materials and help plan implementation strategies. The document comprises six parts.

Research demonstrates that healthcare workers who comply with hand hygiene according to WHO guidelines prevent a greater percentage of these infections, making hand hygiene the single most important practice in reducing the number of nosocomial infections. Yet, despite the availability of this guideline, hand hygiene compliance remains suboptimal among healthcare workers. Many healthcare professionals' associate noncompliance to lack of education, demanding workload, skin irritation, lack of support from leaders and inadequate supplies of hand hygiene products. Implementing a multimodal hand hygiene compliance program based on WHO guideline including education, direct observation, regular update, auditing and performance feedback will reduce nosocomial infections. Other factors such as attitude of healthcare workers, strong support from management, adequate access to hand hygiene supplies, addressing religious and cultural issues among healthcare workers showed a significant reduction in nosocomial infection rate due to improved hand hygiene practices. Hand hygiene is a major component of patient and staff safety. Effective infection control program and patient safety require that healthcare workers be familiar with appropriate hand hygiene recommendations and consistently adhere to the guideline. Evidence based practice are in support of healthcare workers compliance and adherence to hand hygiene guideline to prevent nosocomial infection. Hand hygiene compliance according to WHO guideline will improve quality of care, improve patient safety and reduce healthcare costs.

A quantitative cross-sectional survey using primary data collected via a WHO validated self-administered questionnaire has been distributed to students at the Solwezi College of Nursing. 167/206 participants were recruited via stratified random sampling. Descriptive statistics highlighted potential relationships between demographic/training factors and students hand hygiene knowledge which reduces HAI. Moreover, potential relationships were analysed using Fishers exact test and a multinomial logistic regression. The low compliance to effective hand hygiene has continued to fuel the high prevalence of Hospital Acquired Infections in Africa. The large number of nursing students has a potentially high impact at reducing the HAI public health problem in Zambia, however, there is insufficient information on the nursing student’s demographic/training factors affecting their hand hygiene knowledge which reduces the HAI risk.

Book excerpt: Based on documented studies, the noncompliance of hand washing by medical professionals in the health care setting indicates that as many 99,000 patients die annually from hospital-associated infections. Nosocomial infections are commonly transmitted when hospital officials do not practice correct hygiene regularly. Hand washing frequently is the single most important measure to reduce the risk of transmitting disease from one person to another or from one site to another on the same patient. Washing hands should be performed after contact with a patient, between patients and after contact with blood, body fluids, secretions, excretions, and equipment or articles contaminated by them as an important component of infection control and isolation precautions. Research has indicated a definitive problem with the noncompliance of hand washing protocol exemplified by health care professionals. Health care professionals have reported several barriers to appropriate hand hygiene including skin irritation, inaccessible supplies, interference with worker-patient relation, patient needs perceived as priority, wearing gloves, forgetfulness, ignorance of guidelines, insufficient time, high workload and understaffing, and lack of scientific information demonstrating impact of improved hand hygiene on hospital infection rates. The rate of nosocomial infections can be reduced by full-scale infection control programs whose expense would be recovered with the reduction of cost calculated in treating nosocomial infections. The increase of portable and accessible sanitizers, the discontinuation of artificial nails, and promotion of educational material have been shown to decrease the number of hospital-associated infections each year. Implementing screening tools and implementing the above interventions can prevent disease progression and death. These interventions can potentially decrease the length of stay and health care costs.

HAI's are responsible for prolonged hospital stays due to complications such as MRSA leading to costly treatments. Most of the deadly pathogens are carried by the healthcare from one patient to another or from one place to another if proper hand hygiene is not observed. However compliance of washing hands remains low or poor among healthcare workers. In published studies suggest that, on average, compliance with hand hygiene is around 40% (WHO Guidelines on hand hygiene in health care). Education about hand hygiene remains the main factor to reinforce the prevention of hospital-acquired infections. The process of education is based on Health Belief Model a conceptual framework that describes a person's behavior as an expression of health beliefs. This is where one focuses on individual's perception of risks and benefits that can change in health related behavior such as washing hands. There are several components of the model including susceptibility, severity, benefits and barriers. An education program to motivate health workers should be initiated to improve washing hands to prevent HAI. Providing regular training on the importance of hand hygiene using " My 5 Moments for hand hygiene" approach and the correct procedures of hand rubbing and hand washing to all health workers. (WHO, 2005) By providing the tools to support health care workers and others responsible for improving patient safety at the national and local levels WHO Patient Safety hopes to see compliance increase in each country of the world from its current Impact of Hand Washing Education on Hospital Acquired Infection baseline. The goal is to promote hand hygiene and increase awareness of Hospital Acquired Infections through education Evidenced Based programs.

To prevent the chances of omission and noncompliance of hand hygiene exist in a busy environment like the hospital, presence of a flashing light drew the attention and thus increasing the compliance rate to a high percentage was the success on the intervention of attention theory. Which is another example of the evidence based practice research. Being in contact with patients directly almost all the time, it is necessary that nurse follows infection control practices such as hand hygiene regularly and mandatorily to protect the patient and self. The big question that this project focuses on is whether, nurses and other healthcare personnel that practices proper hand washing and sanitation techniques to prevent the infection while administering patient care in hospitals. Result in a reduction in the possibility of the patients being infected. When compared with those who do not follow the appropriate preventative methods such as washing or sanitizing hands before and after providing care?

Book excerpt: Nosocomial infections are real and serious threats to all patients who are admitted to the hospital. Microorganisms are transmitted to healthcare workers' hands. The most effective way to reduce this transmission is to perform hand hygiene before and after visiting each patient's bedside. The evidence based studies revealed that healthcare workers' knowledge regarding the hand hygiene practice exists, but the compliance regarding this simple process is intentionally low and nosocomial infections are still a real threat to the community. Many healthcare organizations are spending money on hand hygiene products to make it easily available to healthcare workers and education to improve compliance. A closer look at the rationale behind unacceptable and unsafe low compliance, hand hygiene, and barriers show that increased hand hygiene is not regularly followed. Mini research conducted by the writer in a 15 be unit included the study of doctors, nurses, nursing assistants, therapists, therapy assistants, and dietitians who are involved in direct patient care (Appendix A). Beliefs, attitudes, knowledge, and reasoning among healthcare workers are main barriers for noncompliance. Behavioral changes among healthcare workers improved the compliance. The writer who conducted this study shows that frequent observations, friendly reminders, direct questionnaires for the reasoning of noncompliance, patient education and attitude changes, and increased anxiety among healthcare workers about job security improves hand hygiene skills and helps it become a habit. In conclusion, in order to effectively improve hand hygiene compliance among healthcare workers to reduce nosocomial infections, patient and worker education, speaking up to unsafe care, and friendly reminders must be reinforced (Appendix E).
