Book excerpt: This manual presents guidelines used by the staff of the Afghan Refugee Health Programme in providing health services to the Afghan refugees in Pakistan. It is intended to be used as a reference tool by refugee workers and serves as a description of the standards of care established for the programme. The author begins by giving an overview of the situation of Afghan refugees in Pakistan and describes the socio-economic characteristics of the refugee population. The major health needs of the refugees in Pakistan are examined and a summary of the health status and services is given. Information is also provided on infant and child mortality, the nutritional status of refugees and the immunization programme. The overall health programme for Afghan refugees in Pakistan is a collaborative effort of the Government of Pakistan, UNHCR and voluntary agencies. The role played by each of these parties in the administration of the programme is described. The second part of the manual presents an analysis of the scope of the health services offered, ranging from the role of the basic health units in refugee villages, to staffing needs and data collection. The importance of reorienting the programme towards primary health care is emphasized. Attention is drawn to the organization of emergency disaster assistance, procedures to be followed, the assessment of the emergency and the planning of the assistance. The third section of the manual consists of a compilation of guidelines covering specific health concerns, namely, tuberculosis control, malaria, the programme for immunization, supplementary feeding, diarrhoeal disease control, sanitation, etc.

Today, the concept of "the refugee" as distinct from other migrants looms large. Immigration laws have developed to reinforce a conceptual dichotomy between those viewed as voluntary, often economically motivated, migrants who can be legitimately excluded by potential host states, and those viewed as forced, often politically motivated, refugees who should be let in. In Crossing, Rebecca Hamlin argues against advocacy positions that cling to this distinction. Everything we know about people who decide to move suggests that border crossing is far more complicated than any binary, or even a continuum, can encompass. The decision to leave home is almost always multi-causal and often involves many stops and hazards along the way--a reality not captured by a system that categorizes a majority of border-crossers as undeserving, and the rare few as vulnerable and needy. Drawing on cases of various "border crises" across Europe, North America, South America, and the Middle East, Hamlin outlines major inconsistencies and faulty assumptions upon which the binary relies, and explains its endurance and appeal by tracing its origins to the birth of the modern state and the rise of colonial empire. The migrant/refugee binary is not just an innocuous shorthand, indeed its power stems from the way in which is it painted as objective, neutral, and apolitical. In truth, the binary is a dangerous legal fiction, politically constructed with the ultimate goal of making harsh border control measures more ethically palatable to the public. This book is a challenge to all those invested in the rights and study of migrants, to interrogate their own assumptions and move towards more equitable advocacy for all border crossers.

A professor at Kabul University and scholar of Afghanistan affairs at the time of the 1978 coup d'état, Kakar vividly describes the events surrounding the Soviet invasion in 1979 and the encounter between the military superpower and the poorly armed Afghans. The events that followed are carefully detailed, with eyewitness accounts and authoritative documentation that provide an unparalleled view of this historical moment. Because of his prominence Kakar was at first treated with deference by the Marxist government and was not imprisoned, although he openly criticized the regime. As Afghans and the rest of the world try to make sense of Afghanistan's recent past, Kakar's voice will be one of those most listened to.
