Book excerpt: Low inter-rater reliability can jeopardize the integrity of scientific inquiries or have dramatic consequences in practice. In a clinical setting for example, a wrong drug or wrong dosage of the correct drug may be administered to patients at a hospital due to a poor diagnosis. Likewise, exam grades are considered reliable if they are determined only by the candidate's proficiency level in a particular skill, and not by the examiner's scoring method. The study of inter-rater reliability helps researchers address these issues using an approach that is methodologically sound. The 4th edition of this book covers Chance-corrected Agreement Coefficients (CAC) for the analysis of categorical ratings, as well as Intraclass Correlation Coefficients (ICC) for the analysis of quantitative ratings. The 5th edition however, is released in 2 volumes. The present volume 2, focuses on ICC methods whereas volume 1 is devoted to CAC methods. The decision to release 2 volumes was made at the request of numerous readers of the 4th edition who indicated that they are often interested in either CAC techniques or in ICC techniques, but rarely in both at a given point in time. Moreover, the large number of topics covered in this 5th edition could not be squeezed in a single book, without it becoming voluminous. Volume 2 of the Handbook of Inter-Rater Reliability 5th edition contains 2 new chapters not found in the previous editions, and updated versions of 7 chapters taken from the 4th edition. Here is a summary of the main changes from the 4th edition that you will find in this book: Chapter 2 is new to the 5th edition and covers various ways of setting up your rating dataset before analysis. Chapter 3 is introductory and an update of chapter 7 in the 4th edition. In addition to providing an overview of the book content similar to that of the 4th edition, this chapter introduces the new multivariate intraclass correlation not covered in previous editions. Chapter 4 covers intraclass correlation coefficients in one-factor models and has a separate section devoted to sample size calculations. Two approaches to sample size calculations are now offered: the statistical power approach and the confidence interval approach. Chapter 5 covers intraclass correlation coefficients under the random factorial design, which is based on a two-way Analysis of Variance model where the rater and subject factors are both random. Section 5.4 on sample size calculations has been expanded substantially. Researchers can now choose between the statistical power approach based on the Minimum Detectable Difference (MDD) and the confidence interval approach based on the target interval length. Chapter 6 covers intraclass correlation coefficients under the mixed factorial design, which is based on a two-way Analysis of Variance model where the rater factor is fixed and the subject factor random. The treatment of sample size calculations has been expanded substantially. Chapter 7 is new and covers Finn's coefficient of reliability as an alternative to the traditional intraclass correlations when they are not be applicable. Chapter 8 entitled "Measures of Association and Concordance" covers various association and concordance measures often used by researchers. It includes a discussion of Lin's concordance correlation coefficient and its statistical properties. Chapter 9 is new and covers 3 important topics: the benchmarking of ICC estimates, a graphical approach for exploring the influence of individual raters in low-agreement inter-rater reliability experiments, and the multivariate intraclass correlation. I wanted this book to be sufficiently detailed for practitioners to gain more insight into the topics, which would not be possible if the book was limited to a high-level coverage of technical concepts.

Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.

Led by UNESCO, the World Bank, the Center for Universal Education at the Brookings Institution and UNICEF, the initiative aims to promote feasible, accurate and useful measurement of childrenâs development and learning at the start of primary school, and of the quality of their pre-primary learning environments. Items are designed for children between the ages of 4 and 6 years. Following the premise that many existing tools include similar items, the leading organizationsâ core team worked with a consortium of experts, non-governmental organizations (NGOs) and multilaterals to build upon current measurement tools to create a common set of items organized into modules for measuring: 1) early childhood development and learning, and 2) the quality of pre-primary learning environments. The MELQO core team and experts also collaborated to outline a process for context-specific adaptation of the measurement modules resulting from lessons learned from field-testing in several countries in 2015 and 2016. The modules are designed to be implemented at scale, with an emphasis on feasibility for low- and middle-income countries (LMICs). Book excerpt: The internal validity of a study reflects the extent to which the design and conduct of the study have prevented bias(es). One of the key steps in a systematic review is assessment of a study's internal validity, or potential for bias. This assessment serves to: (1) identify the strengths and limitations of the included studies; (2) investigate, and potentially explain heterogeneity in findings across different studies included in a systematic review; and (3) grade the strength of evidence for a given question. The risk of bias assessment directly informs one of four key domains considered when assessing the strength of evidence. With the increase in the number of published systematic reviews and development of systematic review methodology over the past 15 years, close attention has been paid to the methods for assessing internal validity. Until recently this has been referred to as “quality assessment” or “assessment of methodological quality.” In this context “quality” refers to “the confidence that the trial design, conduct, and analysis has minimized or avoided biases in its treatment comparisons.” To facilitate the assessment of methodological quality, a plethora of tools has emerged. Some of these tools were developed for specific study designs (e.g., randomized controlled trials (RCTs), cohort studies, case-control studies), while others were intended to be applied to a range of designs. The tools often incorporate characteristics that may be associated with bias; however, many tools also contain elements related to reporting (e.g., was the study population described) and design (e.g., was a sample size calculation performed) that are not related to bias. The Cochrane Collaboration recently developed a tool to assess the potential risk of bias in RCTs. The Risk of Bias (ROB) tool was developed to address some of the shortcomings of existing quality assessment instruments, including over-reliance on reporting rather than methods. Several systematic reviews have catalogued and critiqued the numerous tools available to assess methodological quality, or risk of bias of primary studies. In summary, few existing tools have undergone extensive inter-rater reliability or validity testing. Moreover, the focus of much of the tool development or testing that has been done has been on criterion or face validity. Therefore it is unknown whether, or to what extent, the summary assessments based on these tools differentiate between studies with biased and unbiased results (i.e., studies that may over- or underestimate treatment effects). There is a clear need for inter-rater reliability testing of different tools in order to enhance consistency in their application and interpretation across different systematic reviews. Further, validity testing is essential to ensure that the tools being used can identify studies with biased results. Finally, there is a need to determine inter-rater reliability and validity in order to support the uptake and use of individual tools that are recommended by the systematic review community, and specifically the ROB tool within the Evidence-based Practice Center (EPC) Program. In this project we focused on two tools that are commonly used in systematic reviews. The Cochrane ROB tool was designed for RCTs and is the instrument recommended by The Cochrane Collaboration for use in systematic reviews of RCTs. The Newcastle-Ottawa Scale is commonly used for nonrandomized studies, specifically cohort and case-control studies.

Book excerpt: The primary objective of this book is to show practitioners simple step-by-step approaches for organizing rating data, creating SAS datasets, and using appropriate SAS procedures, or special SAS macro programs to compute various inter-rater reliability coefficients. The author always starts with a brief and non-mathematical description of the agreement coefficients used in this book, before showing how they are calculated with SAS. The non-mathematical description of these coefficients is done using simple numeric examples to show their functionality. The author offers practical SAS solutions for 2 raters as well as for 3 raters and more. The FREQ procedure of SAS offers the calculation of Cohen's Kappa as an option, when the number of raters is limited to 2. The introduction of this feature is without doubt a very welcome addition to the system. But in addition to offering only Kappa as the only agreement coefficient, the use of FREQ to compute Kappa is full of pitfalls that could easily lead a careless practitioner to wrong results. For example, if one rater does not use one category that another rater has used, SAS does not compute any Kappa at all. This problem is referred to in chapter 1 as the unbalanced-table issue. Even more seriously, if both raters use the same number of different categories, SAS will produce "very wrong" results, because the FREQ procedure will be matching wrong categories to determine agreement. This issue is referred to in chapter 1 as the "Diagonal Issue." There are actually a few other potentially serious problems with weighted Kappa that the author has identified. They are all clearly documented in this book, and a plan for resolving each of them is proposed.
