Soft chancre or chancroid is a virulent ulcer. It usually begins within twenty-four or thirty-six hours after exposure, first as a red spot, but rapidly developing into an ulcer covered with thick yellowish pus. The period of development is about three or four days. iSometimes a week elapses from the time of exposure to the development of the sore, and occasionally a period of incubation is as long as ten days. A sore appearing within a few days, or a week, or even as late as ten days after the exposure is usually regarded as a chancroid. But in practice this is not a safe rule, for the reason that many venereal sores are of a mixed character. A hard or syphilitic chancre contracted two or three weeks ago, makes its appearance to-day. A soft chancre or chancroid contracted two or three days ago, makes its appearance to-day. The inoculations of both poisons take place at the one and same spot, the result is a mixed chancre; or if two sores appear the origin of one may be syphilitic, the other chancroidal. It is therefore difficult, if not impossible, in many cases to determine the character of the disease from the period of incubation or from the appearance or local characteristics of the chancre. A mixed chancre is a syphilitic chancre (a hard chancre), while its appearance may be precisely like that of the soft chancre or chancroid. The only safe plan is to regard all venereal sores as suspicious. But while this is true, treatment for syphilis should not be commenced before the appearance of secondary symptoms, for unless such symptoms appear it is impossible to determine that syphilis really exists in any case. Concern for the health of merchant mariners has, from the beginning, been a part of our nation's history. In the 1700's, legislation mandated that a Medicine Chest be carried on each American Flag vessel of more than 150 tons, provided it had a crew of ten or more. By 1798, a loose network of marine hospitals, mainly in port cities, was established by Congress to care for sick and disabled American merchant seamen. Called the Marine Hospital Service, later the Public Health and Marine Hospital Service, and finally the Public Health Service, these federal entities continued to provide healthcare to merchant seamen until 1981. The Ship's Medicine Chest and Medical Aid at Sea has been a part of much of this maritime history. This edition has evolved through many previous editions. The Public Health Service published the first Medicine Chest in 1881 under the title, Handbook for the Ship's Medicine Chest.

As early as 1921, an international convention regarding this matter was presented to govern ments of maritime countries for ratification. The First World Health Assembly, in 1948, recommended that WHO should establish, with the International Labour Of fice, a Joint Committee on the hygiene of seafarers. The first session of this Com mittee, held in 1949, defined the problems affecting the health of seamen and made a number of recommendations. In the opinion of this Joint Committee, the health of seamen called for interna tional attention for a nu mb er of reasons. By the nature of his work, the seafarer is obliged to travel from country to country and is exposed to great variations of cli mate and also to any disease that may be prevalent in the port of call. He may there fore become a carrier of disease, so that the protection of his health is of importance not only to himself and the other members of the crew but also to the populations of other countries he visits. Yet, on account ofthe nature ofhis employment, it is diffi cult to provide the seafarer with the same standard of health care that is gene rally available to other sections of the population.
