Book excerpt: The WHO Guidelines on Hand Hygiene in Health Care provide health-care workers (HCWs), hospital administrators and health authorities with a thorough review of evidence on hand hygiene in health care and specific recommendations to improve practices and reduce transmission of pathogenic microorganisms to patients and HCWs. The present Guidelines are intended to be implemented in any situation in which health care is delivered either to a patient or to a specific group in a population. Therefore, this concept applies to all settings where health care is permanently or occasionally performed, such as home care by birth attendants. Definitions of health-care settings are proposed in Appendix 1. These Guidelines and the associated WHO Multimodal Hand Hygiene Improvement Strategy and an Implementation Toolkit (http://www.who.int/gpsc/en/) are designed to offer health-care facilities in Member States a conceptual framework and practical tools for the application of recommendations in practice at the bedside. While ensuring consistency with the Guidelines recommendations, individual adaptation according to local regulations, settings, needs, and resources is desirable. This extensive review includes in one document sufficient technical information to support training materials and help plan implementation strategies. The document comprises six parts.

These infections can be acquired anywhere healthcare is delivered, including inpatient acute care hospitals, outpatient settings such as ambulatory surgical centers and end-stage renal disease facilities, and long-term care facilities such as nursing homes and rehabilitation centers. Healthcare-Associated Infections may are categorized as bacteria, fungi and viruses. Furthermore, these infectious pathogens present serious threats to all parties involved as they can be deadly to patients, as well as costly to the healthcare system. Healthcare Associated Infections are preventable. Consequently, the importance of hand hygiene as a regulated and enforced practice in hospitals and healthcare facilities emphasizes the relationship between two variables. The dependent variable of the risk of healthcare associated infections, which is related to the independent variable of hand hygiene. Indeed, It is a well-known fact that the key to eradicate HAIs is through proper hand hygiene. However, hand hygiene compliance rate among healthcare workers remains low making the fight against HAIs very challenging. The proposed suggestion to this issue is that alcohol based hand- rubs (ABHR) are utilized as hand decontaminants to prevent the spread of these infections. Research has shown that ABHR are just as effective as hand washing when used properly. For this reason, ABHR should be made readily available in this facility which will facilitate hand hygiene thus increasing compliance rate, and eradicate healthcare-associated infection.

Book excerpt: Nursing assistants provide the majority of care to patients in nursing-home settings and have the potential to decrease infection rates through improved hand hygiene. The purpose of this quantitative, cross-sectional, pretest and posttest design project was to determine if reinforced educational interventions regarding handwashing increased the knowledge and improved practices of handwashing in Certified Nursing Assistants (CNA) as evidenced by a decrease in the facilities' infection rates. The three-week project was conducted in an Alzheimer's long-term care facility located in a suburban area of Southern California. A demographic survey and the 10 item questionnaire using the World Health Organization (WHO), Hand Hygiene Knowledge Questionnaire for Healthcare Workers was given to 21 participants (n=21) involved in the pre-test, and 19 participants involved in the post-test (n=19). A visual reinforcement of hand hygiene practices was given via a five-minute education video and the use of Glo-germ gel with ultraviolet light. A paired sample t-test was computed to compare the pre and post knowledge from the nursing assistants in terms of hand hygiene practices. The mean overall score of the pre-test was 58.95, and the post-test mean score was 60.53. The results showed no statistical significance found (t (18) = -.358, p>.05). No significant difference was noted in the knowledge levels of the participants from the study. The use of the Glo-germ gel and ultraviolet light provided awareness on the employees as evidenced by them washing their hands thoroughly and longer. The facility has not seen an increase in infection rates since the project was conducted.
