Book excerpt: Although distant metastases are the most dreaded situation in the evolution of cancer of every organ, the medical literature has surprisingly given little attention to the anatomical relationship between the primary tumor and metastasic sites. Only risk factors, treatment possibilities, and survival results are extensively examined. Stimulated by the occurrence in his practice of some puzzling and unexpected metastases, the author reviewed more than 12,000 references. He looked for anatomical relationships highlighting the relation between the location of the primary tumors and the particular patterns of metastasis observed. It would seem that the `pathways and flows' are apparently a more decisive factor in the implantation of the metastases than the `seed and soil' properties of the cancer cells and the metastatic site. Aided by his colleague Dr. T. Geukens, M.D., the author includes original anatomical drawings, illustrating the sometimes unexpected pathways the cancer cells follow in order to reach the organs where they will become lodged and give rise to metastatic tumors. The subject has apparently not been exhausted in the literature and several ideas are given for further research.

Only malignant tumour cells and infections have the capacity to metastasise. Cancer cells can "break away" from a primary tumour, penetrate into lymphatic and blood vessels, circulate through the bloodstream, and grow in a distant focus (metastasise) in normal tissues elsewhere in the body. Metastasis is considered a hallmark of malignancy. All tumours can metastasise albeit to varying degrees, barring a few exceptions (eg. Glioma and Basal cell carcinoma never metastasize). When cancer cells spread to form a new tumour, it is called a secondary, or metastatic tumour, and its cells are like those in the original tumour. This means, for example, that if breast cancer spreads (metastasizes) to the lung, the secondary tumour is made up of abnormal breast cells (not abnormal lung cells). The disease in the lung is then called metastatic breast cancer (not lung cancer). Only malignant tumour cells and infections have the capacity to metastasize. This book presents the latest research in the field from around the world.

Book excerpt: Surgical resection of lung metastases is a widely accepted procedure but long-term results are disappointing with a 5-year survival rate of approximately 40%. The main prognostic factor is complete resection and a better survival is reported in patients with a single metastasis and a disease-free survival of more than 3 years. Re-operations are feasible but often patients become inoperable due to insufficient pulmonary reserve and new treatment modalities are looked for. Intravenous (iv) chemotherapy has no major impact on survival due to systemic side-effects when high doses are given. As isolated limb and liver perfusion, isolated lung perfusion (ILuP) has the advantage of selectively delivering an agent into the lung while diverting the venous effluent. This allows the drug to be given in a significantly higher dose compared to iv therapy, while drug levels in the other organs are kept low enough to avoid major complications. ILuP has been shown to be very effective for the treatment of lung metastases in animal models. Phase I clinical studies to determine dose-limiting toxicity and maximum tolerated dose have recently been reported. The principal aim of this book is to discuss the current treatment of lung metastases and the possible contribution of ILuP.

However, even in those countries in which paediatric cancer is attracting increasing interest, the disease is relatively uncommon, and the number of cases seen by any one physician, even in a large general hospital, is often limited to one or two a year. The widely held view by parents and even by many doctors that cancer in childhood is usually untreatable and inevitably fatal is no longer tenable. With improvement in the therapeutic response or in the actual survival rates of children with such lesions as Wilms' tumour, brain tumours, rhabdomyosarcomas, Ewing's sarcoma, retinoblastoma, lym phoma and even leukaemia, there is a real hope of achieving a substantial reduction in the mortality of childhood cancer. Paediatric oncology is, in fact, providing a vigorous stimulus to the much wider field of cancer treatment and research, and is demonstrating the advantages of a multi-disciplinary cooperation in the management of this disease.

There is growing evidence that non-pharmacological therapies provide symptom and pain management in cancer palliation.
