Book excerpt: Excerpt from The Electro-Therapeutics of Gynaecology, Vol. 1 The necessity for a practical guide for the application of electricity to gynaecology, according to the modem and most approved methods of Apostoli and his followers, was so forcibly urged upon me by many who have read my various contributions to the journals that I was persuaded to undertake the task, the outcome of which is the present work. The numerous claims upon my time by an active practice, as well as my editorial duties, have rendered the task particularly laborious, and I trust my critics will make due allowance for shortcomings, the existence of which I fully realize. It has been my aim to simplify the electro-physics so that it can be understood by anyone, no matter how limited his previous knowledge of the subject may be, at the risk of its being considered elementary, knowing that this is one of the stumbling-blocks in the path of the physician or student who desires to acquire the information necessary to intelligently utilize this agent. This chapter has, therefore, been made short, containing only just what is actually necessary, and leaving the special physics of the different currents to be considered in the separate chapters devoted to each. By this arrangement, it is believed much confusion has been avoided, and a clearer appreciation of the different qualities of the several electrical currents will be facilitated. No attempt has been made to make this part of the work scientific, but rather to make it practical for beginners. The chapter on apparatus contains a description of only such instruments as I have had a personal acquaintance with, either directly or indirectly, which explains why those of my own design predominate. There has been no intentional exclusion of others which may possess equal advantages. The same may be said of the battery apparatus mentioned. Plates VIII and IX are, therefore, half-tone reproductions of actual photographs, and it is hoped that this method of depicting the real situation of these points will lead to greater ease in their location than has been the case in the past. The services of a professional model were also invoked in the more realistic delineation of the therapeutic procedures shown in Plates III to VII. It is the case of a woman of about fifty, who, twenty four years ago, while pregnant, burst a vessel in the brain, and had a well-marked crossed paralysis. When she came to me, the facial paralysis had disappeared, but the left limb was paretic and the left arm was practically useless, as her hand was not entirely under her control, and would open and close involuntarily. The contour of the arm was changed, probably on account of the unusual character of the exercise her muscles had in making these athetosic movements. Sensibility was intact. The essential lesion is a new growth resulting from the irritation caused by the presence of the tubercle bacillus. The process begins in the deeper layer of the cutis. The nodules displace the bundles of fibrous tissue; and as they increase in size they grow upward through the skin, destroying its component elements by pressure, so breaking through the papillary layer, and emerging on the surface, where they are covered only by the epithelium, more or less translucent according to its thickness.
