Book excerpt: While reports show that approximately 25% of HIV-infected women do not receive recommended gynecological care and that gynecological disorders are more common and severe in HIV-infected women, there has been limited research on patterns of utilization of gynecological care among this population. This cross-sectional study was conducted to explore the association between some predisposing, enabling and need variables and the receipt of gynecologic care in HIV-infected women receiving medical care in the United States using data from the 1996-1998 HIV Cost and Services Utilization Study (HCSUS). A sample of 718 HIV-infected women 18 years and older that completed the first follow-up interview in the HCSUS study were analyzed using the STATA statistical software and the svy module to account for the complex sample design. Approximately 54% of the participants were African Americans, 25% were Whites and an estimated 63% had public insurance while 22% were uninsured. Results showed that 31% of women in the study reported lack of receipt of gynecological care. In multivariate logistic regression, race/ethnicity (Blacks: OR = 1.9, 95%CI = 1.23-2.96, p=0.005; Others: OR = 2.33, 95%CI = 1.4-3.88, p=0.002), education (High school: OR 1.80, 95%CI=1.0-3.25, p=0.05), social support (5 or more friends: OR = 1.88, 95%CI = 1.12-3.18, p=0.018) and age (45+years: OR = 0.45, 95%CI = 0.27-0.76, p=0.004) were significantly associated with receipt of gynecological care. In summary, findings from this study suggest that minorities, women with at least a high school education or diploma and women with more social support were about two times more likely to receive gynecologic care and women 45 years and older were less likely to receive gynecologic care. It is therefore recommended that public health interventional programs target those groups of HIV-infected women particularly at risk for lack of receipt of gynecological care as identified in this study and incorporate factors such as social support and enhanced education tailored to reach the older and less educated women. More studies are needed to study receipt of preventive services in HIV-infected women.-- Abstract.
