Book excerpt: "This guideline updates and expands the original Centers for Disease Control and Prevention (CDC) Guideline for Prevention of Catheter-associated Urinary Tract Infections (CAUTI) published in 1981. Several developments necessitated revision of the 1981 guideline, including new research and technological advancements for preventing CAUTI, increasing need to address patients in non-acute care settings and patients requiring long-term urinary catheterization, and greater emphasis on prevention initiatives as well as better defined goals and metrics for outcomes and process measures. In addition to updating the previous guideline, this revised guideline reviews the available evidence on CAUTI prevention for patients requiring chronic indwelling catheters and individuals who can be managed with alternative methods of urinary drainage (e.g., intermittent catheterization). The revised guideline also includes specific recommendations for implementation, performance measurement, and surveillance. Although the general principles of CAUTI prevention have not changed from the previous version, the revised guideline provides clarification and more specific guidance based on a defined, systematic review of the literature through July 2007. For areas where knowledge gaps exist, recommendations for further research are listed. Finally, the revised guideline outlines high-priority recommendations for CAUTI prevention in order to offer guidance for implementation."--Page 8.

Book excerpt: Urinary tract infections associated with catheter insertions are the most prevalent hospital associated infection. The expenses incurred in treating Cather Associated Urinary Tract Infections (CAUTI) are not covered by the Center for Medicare and Medicaid Services (CMS) since October 2008. According to the Center for Disease Control and Prevention (CDC), almost twenty-five percent of hospitalized patients receive urinary catheters during their hospital stay and approximately seventy-five percent of urinary tract infections are associated with urinary catheters. The need for catheter usage and the chance of developing a CAUTI associated with the prolonged use of catheters should be reviewed. Because of this reason, catheters should only be used if indicated and should be removed as early as possible. This literature review is based on the Patient, Intervention, Control, and Outcome (PICO) question: Does following CAUTI guidelines reduce hospital-acquired CAUTIs? Current research studies shows that approach to managing and avoiding CAUTIs should be based on practicing guidelines and these guidelines should be adhered to (Grabe M, 2011). Hence, the goal of this project is to develop a CAUTI prevention team to prevent CAUTIs by increasing staff awareness regarding the importance of following these guidelines. The first phase of implementation of guidelines include introduction to bladder bundle care and surveillance rounds by CAUTI team along with in-services, which would be continued for three months. The second phase would include a survey to evaluate the outcome of the project; evaluate infection rates by Audit is necessary after the implementation of the new plan. Through written policies and procedures, the project would be established and made available for staff members. This paper discusses the current status based on literature review on the problem, theory, evidence, implementation, and evaluation of the plan.

Book excerpt: Catheter associated urinary tract infection (CAUTI) is the most common health care associated infection that is very preventable if appropriate measure are followed. It is responsible for about 40% of health care associated infections (HAI) in acute care settings and about 80% of the cause is directly linked to the presence of indwelling catheters. According to the American Association of Critical Care Nurses ( 2011), CAUTI adds to the hospital cost and no additional payment is made by the Centers for Medicare and Medicaid services to the facility as this condition is considered to be very preventable. Use of catheter for a prolonged period of time has been identified as the main cause of CAUTI hence the need to remove catheter immediately it is no longer needed. CAUTI increases the average length of stay by 5-6 days and this has a significant effect on the outcome of the patient. Yearly in hospitals around the country, 450,000 incidence of CAUTI is treated and the cost is greater than $400 million (Centers for Disease Control and Prevention (CDC),2012. This condition can be caused by lack of about knowledge among nursing staff in regards to infection control practices (Fraczyk, Tibbs, Brennan, and Godfrey, 2011). The goal of this research is to increase awareness of CAUTI in this author's place of employment and using evidence-based practice and CDC guidelines for prevention. Extended use of catheters without clinical justification can cause bacteremia, sepsis, cystitis, epididymitis, pyelonephritis, and in rare occasions endocarditis and even death (Stickler and Feneley, 2010). Assessing the patient for continuous need of catheter and removing it immediately it is no longer necessary is an important to prevent CAUTI. The implementation plan to prevent CAUTI was developed by this author after reviewing scientific literatures.

According to Centers for Disease Control (CDC), about 15-25 % of patients in hospitals get catheterized at some point during their hospital stay (CDC, 2010). This puts many hospitalized patients at risk for CAUTI which causes increased morbidity, mortality, length of hospital stay, and also increase health care cost (CDC, 2010). Two main causes of CAUTI are unnecessary urinary catheter use and prolonged use of already inserted urinary catheter (CDC, 2012). The risk for CAUTI can be reduced significantly by avoiding these causes. CAUTI can also be caused by a break in aseptic technique during urinary catheter insertion, improper care for urinary catheters that are already inserted, and inability to maintain closed drainage system. CAUTI is mostly preventable. Centers for Disease Control's guideline for CAUTI prevention states that, limiting the use of urinary catheter to only when it is absolutely needed and removing unnecessary catheters as soon as possible make a difference in preventing CAUTI (CDC, 2010). Other means of prevention include staff education on proper aseptic technique during urinary catheter insertion, maintaining closed drainage system, and using electronic surveillance for proper documentation and reminding staff about catheter removal (Welden, 2013). The purpose of this CAUTI prevention project is to show the effect of CAUTI, its causes, ways to prevent it, and how to get stake holders involved in the project by using fifteen critically appraised articles to support this writer's ideas for the CAUTI Prevention project.

The illness cluster is common among the elderly. Moreover, infections occur among female more often than among male with the ratio of 2:1. UTI is an inflammation in the urinary tract that causes morbidity resulting in long-term medical care for the patients. The causal agent is bacterial infections with evidence of bacteria in urine; while severe infections spread to affect the kidneys. The risk factors of UTI include urinary tract obstruction, neurogenic bladder dysfunction, improper use of urinary catheters and external urine collection devices. Due to asymptomatic nature of early infections, diagnosis becomes difficult until the symptoms manifest at the stage, when the infection is causing complications. Jean Watson published the book The Philosophy and Science of Caring in 1979 basing on the principle of caring for patients and developing relationships with them (Watson, 1979). She assessed the science of caring from the perspective of human-to-human interpersonal relationship. The nursing care requires care factors, transpersonal caring relationship and the personal moments that recognize human-to-human interactions. The purpose of Watson's theory is the consideration of patient caring as a holistic process, where the body, the mind, and the soul must be nurtured to complete the cycle of healing (Smith and Parker, 2001). The patient vulnerability is the time when an individual is most in need for care in their lives, which is the major factor justifying Watson's approach. Catheter-associated urinary tract infections (CAUTI) are considered to be the main complication of UTI health challenge. CAUTI results from improper use of catheters triggering the need to insert catheters that will solve the challenge of high risk of microbial infections. Catheterization is a health care professional activity. Therefore, the high risk of associated infection from the use of catheters suggests that high percentages in CAUTI are classified as hospital acquired infections. Therefore, the problem suggests the need for a control policy and personnel training to prevent infections.

Intervention to address problems will be nurses assessing for the appropriateness of use of catheters.

Urinary indwelling catheter is often used in hospitalized patients for various reasons but it is often times the cause of urinary tract infection (UTI). Efforts to prevent CAUTI are imperative as it has become a common hospital acquired infection (HAI) which is costly to both patients and the hospital. The Centers for Medicare and Medicaid Services (CMS) now refuses reimbursement to hospital for HAI which CAUTI is one of them. CAUTI is common and it is a preventable problem. The purpose of this research project is to provide evidence-based strategies to prevent incidence of CAUTI in the hospital. A guideline for prevention of CAUTI has been updated to provide strategies to reduce inappropriate catheter usage. Criteria for appropriate urinary indwelling catheter indication and reminders for aseptic urinary catheter insertion and maintenance has been established (Gould, C et al, 2009).Other studies have indicated that the duration of the indwelling urinary Catheter (IUC) increases the risk of CAUTI in hospitalized patient in acute care settings(Yin-Yin, c. et all,2013). Early removal of IUC is among important interventions in preventing UTI. Therefore a reminder system is advocated for nurses to prompt discontinuation of urinary catheter as soon as it is no longer needed. Other evidence-based strategies were researched such as engaging the healthcare workers through education especially in the emergency departments and intensive care units where initial urinary catheters are often inserted in critically ill patients as a measure to change the current practice and develop solution proposals.
