Book excerpt: Introduction : L'imagerie médicale est au coeur de la prise en charge des patients par sa contribution au diagnostic, au suivi thérapeutique et au traitement de nombreuses pathologies. Afin d'optimiser la pertinence des examens d'imagerie dans un souci constant d'une meilleure qualité de soin, la SFR a élaboré le GBU puis une 2ème version destinée aux médecins généralistes : l'ADERIM. L'objectif principal de notre étude est d'évaluer l'utilisation de ce nouvel outil l'ADERIM auprès des radiologues et de recueillir leur avis dans une perspective d'amélioration et de diffusion du guide. Méthodologie : Étude qualitative par focus groupes réalisée auprès de 23 radiologues afin d'évaluer un outil d'aide à la prescription des examens de radiologie. L'analyse thématique des données recueillies lors des entretiens a été réalisée par la technique de SWOT. Résultats : Les radiologues ont d'abord eu un avis positif du guide. Ils apprécient sa présentation et le trouvent facile et rapide d'utilisation. Cependant, ils soulignent des faiblesses comme des motifs de consultation manquants, une hétérogénéité dans la précision des items, une validité externe et une diffusion du guide insuffisantes. Certaines perspectives d'amélioration ont été proposées telles que l'optimisation de l'ergonomie, l'amélioration de la qualité de l'information des items, le renforcement de la validité externe, la création d'une application et surtout l'amélioration de sa promotion. Des facteurs menaçant son utilisation ont été soulignés, comme un temps de consultation des médecins généralistes limité ou encore l'hétérogénéité de l'accessibilité à l'imagerie selon les territoires de santé. Conclusion : Notre étude a permis de dégager de nombreuses perspectives d'amélioration de l'ADERIM facilitant son utilisation afin d'optimiser la pertinence des actes. S'inspirer d'autres SADM et créer une application intégrant un algorithme décisionnel pourrait être une innovation intéressante. Afin de généraliser nos résultats, une analyse quantitative est réalisée auprès d'un grand nombre de radiologues de France permettant d'évaluer leurs principales attentes.

One assumes that progress is concerned primarily with recent developments whilst history deals with matters of the past. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

Book excerpt: In June 1998 the Fourth International Workshop on Digital Mammography was held in Nijmegen, The Netherlands, where it was hosted by the department of Radiology of the University Hospital Nijmegen. This series of meetings was initiated at the 1993 SPIE Biomedical Image Processing Conference in San Jose, USA, where a number of sessions were entirely devoted to mammographic image analysis. At very successful subsequent workshops held in York, UK (1994) and Chicago, USA (1996), the scope of the conference was broadened, establishing a platform for presentation and discussion of new developments in digital mammog raphy. Topics that are addressed at these meetings are computer-aided diagnosis, image processing, detector development, system design, observer performance and clinical evaluation. The goal is to bring researchers from universities, breast cancer experts, and engineers together, to exchange information and present new scientific developments in this rapidly evolving field. This book contains all the scientific papers and posters presented at the work shop in Nijmegen. Contributions came from as many as 20 different countries and 190 participants attended the meeting. At a technical exhibit companies demon strated new products and work in progress. Abstracts of all papers were reviewed by members of the scientific committee. Many of the accepted papers had excellent quality, but due to limited space not all of them could be included as full papers in these proceedings. Papers that were rated high by the reviewers are included as long or short papers, others appear as extended abstracts in the last chapter.
