Book A Guide To Practical Human Reliability Assessment

Download or read book A Guide To Practical Human Reliability Assessment written by B. Kirwan and published by CRC Press. This book was released on 2017-12-14 with total page 608 pages. Available in PDF, EPUB and Kindle. Book excerpt: Human error is here to stay. This perhaps obvious statement has a profound implication for society when faced with the types of hazardous system accidents that have occurred over the past three decades. Such accidents have been strongly influenced by human error, yet many system designs in existence or being planned and built do not take human error into consideration.; "A Guide to Practical Human Reliability Assessment" is a practical and pragmatic guide to the techniques and approaches of human reliability assessment HRA. lt offers the reader explanatory and practical methods which have been applied and have worked in high technology and high risk assessments - particularly but not exclusively to potentially hazardous industries such as exist in process control, nuclear power, chemical and petrochemical industries. A Guide to Practical Human Reliability Assessment offers the practitioner a comprehensive tool-kit of different approaches along with guidance on selecting different methods for different applications. It covers the risk assessment and the HRA process, as well as methods of task analysis, error identification, quantification, representation of errors in the risk analysis, followed by error reduction analysis, quality assurance and documentation. There are also a number of detailed case studies from nuclear, chemical, offshore, and marine HRA'S, exemplfying the image of techniques and the impact of HRA in existing and design-stage systems.

Book Human Reliability

Download or read book Human Reliability written by K.S. Park and published by Elsevier. This book was released on 2014-06-28 with total page 355 pages. Available in PDF, EPUB and Kindle. Book excerpt: The objectives of Human Reliability are to build reliability into the job, into the machine, and into the environment, and to let man perform naturally. In this book the author shows how these objectives can be achieved by concentrating on human reliability issues during the design stage. This is done by illustrating the relationships between various design features and some aspect of human performance, e.g. human errors. The book is designed as a practical guide to the daily performance of tasks in Human Reliability as well as a general reference and tutorial introduction to the field. It is therefore both practical and theoretical: the first four chapters focus on principles and ramifications relevant to human error prevention; the latter four are primarily concerned with human reliability analysis and prediction methodology. Throughout the book there are extensive references, numerous ready-to-use recommendations, formulas and mathematical models, and computer programs for human reliability work for analyzing, predicting and preventing human errors in a variety of situations. Though some of the material requires undergraduate training in engineering, the more difficult mathematical expositions can be omitted without loss of continuity, but are available for the reader who needs a more complete understanding of the relevant theory.

Book Human Reliability Analysis

Download or read book Human Reliability Analysis written by Erik Hollnagel and published by . This book was released on 1993 with total page 376 pages. Available in PDF, EPUB and Kindle. Book excerpt: The prevalence of human erroneous actions as the major cause of accidents in man-machine systems has created a need for better descriptions of human performance, both for accident analysis and system design purposes. Models and methods are therefore required to assess human reliability, identify potential erroneous actions, and specify ways of preventing them from happening. This book discusses how modelling of cognition is applied to the analysis of human reliability and performance in complex technical domains. It provides a critique of existing approaches to modelling of cognition, and offers an alternative which recognises that the control of human actions is determined by the context as well as cognitive functions. This approach produces an improved qualitative analysis of human performance as a basis for later quantitative reliability assessment. Human Reliability Analysis will be essential reading for practitioners of human reliability analysis as well as students of cognitive psychology and ergonomics at advanced undergraduate and graduate level. Computers and People Series: this series is concerned with all aspects of person-computer relationships, including interaction, interfacing, modelling and artificial intelligence. The volumes are interdisciplinary, communicating results derived in one area of study to workers in another. Applied, experimental, theoretical and tutorial studies are included.

Book Atheana User s Guide Final Report

Download or read book Atheana User s Guide Final Report written by John Forester and published by CreateSpace. This book was released on 2014-07-25 with total page 134 pages. Available in PDF, EPUB and Kindle. Book excerpt: This manuscript provides a user's guide for the human reliability analysis (HRA) method known as “A Technique for Human Event Analysis” (ATHEANA), which the U.S. Nuclear Regulatory Commission (NRC) documented in NUREG-1624, Rev. 1, dated May 2000. As the first publication of its kind, this user's guide describes both the quantitative and qualitative ATHEANA analysis approaches, fully describing the revised quantification approach and presenting a simpler description of the other ATHEANA elements needed to perform an HRA as part of a probabilistic risk assessment (PRA). Toward that end, this user's guide strives to present the steps for applying ATHEANA in a straightforward and succinct manner, so that HRA experts can easily and effectively apply the technique. Consequently, although the authors relied on NUREG-1624 as a primary resource for its development, the NRC is publishing this user's guide as a standalone document, such that it can be used by analysts to apply the ATHEANA technique without the need to use NUREG-1624.

Book The Quantitative Failure of Human Reliability Analysis

Download or read book The Quantitative Failure of Human Reliability Analysis written by and published by . This book was released on 1995 with total page 8 pages. Available in PDF, EPUB and Kindle. Book excerpt: This philosophical treatise argues the merits of Human Reliability Analysis (HRA) in the context of the nuclear power industry. Actually, the author attacks historic and current HRA as having failed in informing policy makers who make decisions based on risk that humans contribute to systems performance. He argues for an HRA based on Bayesian (fact-based) inferential statistics, which advocates a systems analysis process that employs cogent heuristics when using opinion, and tempers itself with a rational debate over the weight given subjective and empirical probabilities.

Book Human Factors and Reliability Engineering for Safety and Security in Critical Infrastructures

Download or read book Human Factors and Reliability Engineering for Safety and Security in Critical Infrastructures written by Fabio De Felice and published by Springer. This book was released on 2017-09-29 with total page 263 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book collects a high-quality selection of contemporary research and case studies on the complexity resulting from human/reliability management in industrial plants and critical infrastructures. It includes: Human-error management issues—considering how to reduce human errors as much as possible. Reliability management issues—considering the ability of a system or component to function under certain conditions for a specified period of time. Thus, the book analyses globally the problem regarding the human and reliability management to reduce human errors as much as possible and to ensure safety and security in critical infrastructures. Accidents continue to be the major concern in “critical infrastructures”, and human factors have been proved to be the prime causes to accidents. Clearly, human dynamics are a challenging management function to guarantee reliability, safety and costs reduction in critical infrastructures. The book is enriched by figures, examples and extensive case studies and is a valuable reference resource for those with involved in disaster and emergency planning as well as researchers interested both in theoretical and practical aspects.
