Book excerpt: Le suivi de grossesse a été organisé par des plans de périnatalité qui ont mis l'accent sur la sécurité de la mère et de l'enfant. Plusieurs professionnels sont compétents pour ce suivi. L'offre médicale est inégale entre les milieux urbains et ruraux en termes, notamment, de densité de professionnels et de nombre de maternités. Nous avons montré que cette inégalité engendre des différences dans le parcours des patientes. La sage-femme, libérale ou hospitalière, a une place prépondérante en milieu rural tandis qu'en milieu urbain, cette place est occupée par le gynécologue. Nous avons pu constater que la qualité de la prise en charge des grossesses et l'état de santé des nouveau-nés à la naissance ne sont pas différents en fonction des lieux d'accouchement. La surconsommation médicale en milieu urbain entraîne une moins bonne efficience du système de santé dans ce milieu. Nous avons remarqué que l’activité des sages-femmes en milieu rural semblait différente de celle des sages-femmes en milieu urbain. Ce constat demanderait à être approfondi et pourrait faire l’objet d’une étude spécifique.

Cette étude comparative entre 100 patientes dites urbaines et 100 patientes dites rurales tente de savoir si cet éloignement est préjudiciable au bon déroulement de la grossesse et du travail et tente de définir les caractéristiques des femmes des deux milieux. Après analyse des résultats, il semblerait que les deux groupes de patientes bénéficient de la même qualité de suivi mais qu'elles accouchent différemment.
