Book excerpt: "It is essential for new graduate nurses to possess professional confidence in order to practice and provide high quality care in the healthcare system today. Unfortunately, many new graduate nurses are entering the field without it. The purpose of this descriptive qualitative study was to explore how new graduate BSN nurses perceive their professional confidence and what contributes to it during their first year of practice in the hospital setting. Data collection involved one individual semi-structured recorded conference interview with each of the five participants through the Zoom application which was transcribed verbatim. Each transcript was analyzed using manual content analysis with a constant comparative technique. Five major themes as well as four facilitators and four inhibitors to the development of professional confidence emerged. The five themes related to new graduate nurses' perception of professional confidence included knowledge and experience, interprofessional collaboration, independence, ongoing learner, and critical thinking. The four facilitators were positive feedback, support systems, asking for help, and perseverance. The four inhibitors were self-doubt, not being confident, being unprepared, and fear. These findings have implications for both undergraduate nursing education programs and nursing practice in the hospital setting. Undergraduate nursing programs should evaluate implementing strategies that better prepare new graduate nurses for their transition into the professional role. Healthcare professionals involved in developing new graduate nurses in practice, specifically nurse managers and preceptors, should be aware that they are entering practice without professional confidence and should focus efforts on what they can do to help promote its development." -- From page v.

Undertaking research to improve the competence of new graduate nurses entering practice is identified as a priority for undergraduate education providers (WHO, 2009). In New Zealand (NZ) the nursing profession is regulated by the Nursing Council of New Zealand (NCNZ). To qualify to practice as a registered nurse (RN), student nurses must complete a Bachelor of Nursing (BN) degree. In this three-year programme of study, student nurses must develop the required competencies to enter the RN scope of practice. This thesis reports on an action research study undertaken in a BN programme in NZ. The aim of the study was to develop a new student clinical competence assessment tool. The key participant groups in the study were the student nurses, and nurse lecturers, from the programme, and registered nurses in the clinical area who were preceptors for student nurses completing clinical placements. The central research question asked: What factors do lecturers, preceptors and students perceive as important to design, and develop a clinical competence assessment tool for student nurses? A total of three action cycles were completed, using a series of focus group interviews to collect data to evaluate the current assessment tool, and collaboratively design a new tool to meet the participants' needs and the required programme standards. The new tool was trialled with the participants, and then evaluated with recommendations to refine the tool for introduction into this programme. The study findings indicated the need for the new tool to authentically assess students' clinical competence, and further guide students' clinical practice development. In addition, the new tool had to be integrated into the BN programme curriculum, and the model of nursing education. The study made recommendations in the areas of clinical learning, educational practice, and research.

Nurse educators must find ways of bridging the theory-to-practice gap. High-fidelity simulation is one mechanism for bridging this gap. This pilot study explored the effect of multi-patient simulation (MPS) on students' self-efficacy and perceived clinical judgment. This quasi-experimental study sought to explore the readiness of BSN students to care for multiple patients through the use of MPS and a self-efficacy survey measuring perceived level of confidence (LOC) in clinical care. Findings were inconclusive regarding whether MPS as a teaching method helped nursing students increase their perceived LOC and improve clinical judgment skills. Students agreed that the design should be part of nursing education and it identified gaps in their knowledge. This study raised an awareness of what students lack in terms of prioritization management when caring for multiple patients.

Book excerpt: Due to an increasingly complex patient population, new graduate nurses are expected to enter the workforce and provide competent safe care in order to promote positive patient outcomes. Pre-licensure nursing education holds the responsibility for providing this population with the critical thinking and clinical self-efficacy necessary for the successful transition from student nurse to practicing Registered Nurse. Unsuccessful transition into this new role has been attributed to a gap between what is learned in school and the reality of nursing practice, the so called "theory-practice gap" leading to poor self-efficacy. In addition, it has been suggested that poor self-efficacy leads to high nurse turnover, medical errors, and nurses leaving the profession entirely. With a looming nursing shortage expected to reach 500,000 Registered Nurses by 2025, ensuring the nursing profession is hiring competent nurses with high self-efficacy is crucial. The purpose of this pilot cohort study was to evaluate the perceived self-efficacy of final semester pre-licensure nursing students before and after completing the required clinical precepted experience. Using a quantitative approach, senior baccalaureate nursing students were surveyed during their final semester, prior to beginning their required clinical precepted experience and again after completing all required clinical hours. Descriptive statistics were used to summarize survey responses while paired samples t-tests were used to evaluate pre and post-test scores. While no statistical significance was found, the variability in student responses to the survey increased, with some students' reporting improved self-efficacy while others remained the same or declined. The interepretation of findings needs caution due to the small sample size. The influence of the theory-practice gap on students' perceived self-efficacy and preparedness to enter the nursing workforce needs to be evaluated further.
