Book excerpt: Chromosomal rearrangements which result in localized increases of genetic material are frequent in breast cancer, and occur consistently in certain genomic regions. The resulting increase in expression of genes contained within these amplifications contributes to the malignant phenotype. Such amplified genes, such as Her2-Neu, provide targets for diagnosis and for the development of inhibitory drugs. The purpose of this study is to use novel genomic technologies to find new genes in breast cancer that are both highly amplified and are suitable targets by virtue of the fact that they are membrane-associated (receptors, membrane antigens, secretory proteins). The aims of the study are (1) To specify intervals of genomic amplification (amplicons) in primary breast cancer cell lines using genomic microarrays; (2) To prepare a database of membrane- associated genes, selected by differential hybridization of RNA prepared from fractionated microsomes; (3) To use the database to select membrane-associated genes that are located within amplicons, and measure their expression in the primary cell line using cDNA arrays, in order to select those that are upregulated. These genes will provide new insights and reagents for diagnosis and treatment of breast cancer.

Book excerpt: The experiments carried out under this Innovator award allowed us to set up a genetic system to identify genes through loss of function (RNAi) or gain of function (over-expression) functional screens that are capable of influencing cells to acquire tumorlike properties. In this way we have identified many novel tumor suppressors and oncogenes. Many of these are found to be altered in breast cancer and other cancer types. Importantly, we developed the ability to identify genes whose inhibition is lethal in cancer cells but not in normal cells. We discovered that many genes cancer cells depend on are not oncogenes but genes never implicated or altered in cancers. This introduced the concept of non-oncogene addiction and opened many researchers to the idea that there are many different potential anti-tumor drug targets that are only going to be found through functional screens for cancer cell vulnerabilities. In addition we devised and tested a novel peptide display technology using synthetic biology to probe the auto-immune response of cancer patients to look for early diagnosis biomarkers. We have synthesized the human peptidome and developed methodology to determine what portions of human proteins are being recognized by auto-anibodies on a genomic scale.

Book excerpt: Proteins localized to the cell membrane or secreted show great promise as therapeutic targets and diagnostic markers because of their easy accessibility. However, determining protein localization by traditional methods is a difficult process. A "feature" of membrane-bound and secreted (MAS) proteins can be exploited to determine their membrane-bound status on a large scale. Because the mRNA transcripts of MAS proteins are translated in polysomes bound to the endoplasmic reticulum (ER), they can be separated from their heavier cytosolic counterparts by sucrose gradient centrifugation. At the end of year two, we have reproducibly separated the RNA of MCF7 cells into two fractions and hybridized them to Affymetrix microarrays. Using a training set of 881 MAS and cytoplasmic proteins, as annotated from SWIS-PROT, we show that genes with a membrane to cytoplasmic expression ratio over 1.08 are very likely to have MAS localization, with 97% specificity for those genes expressed above a threshold level. Applying these criteria to the remaining unknown and tentative localized genes on the microarray led to the identification of 810 predicted MAS genes. Combined with breast cancer expression and amplicon data, this could allow for the identification of potential novel membrane-bound and secreted drug targets and markers.

Book excerpt: The broad goal of this project is to develop genome-wide RNAi approaches in mammals and to apply these to the discovery of new therapeutic targets for cancer. Specifically, we will generate a library of short hairpin RNA expression constructs (shRNA) that ultimately correspond to every gene in the human genome. These will be made available as a public resource and used internally to screen for genes that are essential to the survival of breast cancer cells but which are dispensable for the survival of normal cells. A subset of these might prove suitable as therapeutic targets for breast cancer therapy. During the first year of funding, two things have become clear. First, although they were not in place at the time of submitting this application, we have largely developed the technologies necessary to pursue the above goal. Second, funding in the Innovator award falls far short of that necessary to achieve the goal. Relevant to the last point, we have been able to leverage the Innovator award with several other funding sources to crate a program, which is capable of meeting the proposed goal.

Following a recent case involving premature use of omics-based tests in cancer clinical trials at Duke University, the NCI requested that the IOM establish a committee to recommend ways to strengthen omics-based test development and evaluation. This report identifies best practices to enhance development, evaluation, and translation of omics-based tests while simultaneously reinforcing steps to ensure that these tests are appropriately assessed for scientific validity before they are used to guide patient treatment in clinical trials.

Book excerpt: Motivated by the explosion of molecular data on humans-particularly data associated with individual patients-and the sense that there are large, as-yet-untapped opportunities to use this data to improve health outcomes, Toward Precision Medicine explores the feasibility and need for "a new taxonomy of human disease based on molecular biology" and develops a potential framework for creating one. The book says that a new data network that integrates emerging research on the molecular makeup of diseases with clinical data on individual patients could drive the development of a more accurate classification of diseases and ultimately enhance diagnosis and treatment. The "new taxonomy" that emerges would define diseases by their underlying molecular causes and other factors in addition to their traditional physical signs and symptoms. The book adds that the new data network could also improve biomedical research by enabling scientists to access patients' information during treatment while still protecting their rights. This would allow the marriage of molecular research and clinical data at the point of care, as opposed to research information continuing to reside primarily in academia. Toward Precision Medicine notes that moving toward individualized medicine requires that researchers and health care providers have access to very large sets of health- and disease-related data linked to individual patients. These data are also critical for developing the information commons, the knowledge network of disease, and ultimately the new taxonomy.

Multiple consensus guidelines now recommend molecular testing as the standard of care for most metastatic tumors. To aid in the advancement of this rapidly changing field, we intend this Special Issue of JPM to focus on technical developments in the genomic profiling of cancer, detail promising somatic alterations that either are, or have a high likelihood of being, relevant in the near future, and to address issues related to the pricing and value of these tests.The last few years have seen the cost of molecular testing decrease by orders of magnitude. In 2018, we saw the first “site-agnostic” drug approvals in cancer (for microsatellite unstable cancer (PD-1 inhibitors) and NTRK-fusions (TRK inhibitors)). Research on targetable mutations, determination of genetic “signatures” that can use multiple individual genes/pathways, development of targeted therapy, and insight into the value of new technology remains at the cutting edge of research in this field. We are soliciting papers that present new technologies to assess predictive biomarkers in cancer, original research (pre-clinical or clinical) that demonstrates promise for particular targeted therapies in cancer, and articles that explore the clinical and financial impacts of this paradigmatic shift in cancer diagnostics and treatment.
