Book excerpt: This report focuses on the results of the 2009-10 and 2010-11 financial audits of HSC bodies in Northern Ireland. It notes the following: all health bodies were able to "break even" with regards the budget set for both 2009-10 and 2010-11 with the exception of the Northern Trust which required additional funding for both years and the Belfast Trust which required additional funding for 2009-10; management costs in the health and social care sector in 2010-11 were £125 million which is £17.9 million above the management costs incurred in 2006-07; and that compliance with prompt payment targets continues to present a considerable challenge to health bodies. Key targets have not been achieved for inpatient treatment, outpatient appointments, diagnostic tests or emergency care during 2010-11. At the end of March 2011 over 17,000 patients were waiting more than 13 weeks for inpatient treatment compared to over 3,000 at March 2010; 32,000 patients were waiting longer than the target of nine weeks for a first outpatient appointment; and 12,000 patients were waiting longer than nine weeks for a diagnostic service or more than 13 weeks for a day case endoscopy. The number of patients waiting more than 12 hours in an emergency care department showed an improvement, with 850 waiting in 2010-11 compared to 1,239 twelve months earlier. The target is that no patient should wait for this period of time. The report also draws attention to the report on the Belfast Trust's 2010-11 accounts; the 2010-11 reports on each of the five non-regional trusts; and the reduction in Single Tender Actions across the health and social care sector from £9.8 million in 2006-07 to £5.4 million in 2010-11

Book excerpt: The Department of Health has until recently been focusing on speed of response as a measure of performance of the ambulance service, rather than on clinical outcomes for patients. The ambulance service achieves high levels of public satisfaction but there are wide variations in ambulance trusts' efficiency and the NAO concludes that the system has not delivered the best value for money to date. The 8-minute response target, intended for the most seriously ill patients, is one of the most demanding in the world. However, its application has skewed ambulance trusts' approach to performance measurement and management. The target is also applied to a much wider group of patients than intended. The report identifies various inefficiencies in the system, with cost per incident varying between £176 and £251. There is scope across the urgent and emergency care system to make more of different ways of responding to patients, such as clinical advice to callers over the phone and taking patients to minor injuries units rather than A&E departments. If all 11 trusts adopted the best practice currently being used in at least one trust, the NHS could save £165 million a year. The elements of the emergency care system are not yet fully integrated and this leads to delays in turnaround times at hospital A&Es. Over one-fifth of patient handovers take longer than the recommended 15 minutes. The new clinical quality indicators introduced from April offer the potential for a fuller measurement of performance based on outcomes.

Life expectancy varies significantly across the UK - from 75.9 in Scotland to 78.6 in England for men, and from 80.4 in Scotland to 82.6 in England for women. Spending on health services in the UK more than doubled in cash terms in the last decade. In 2010-11, despite devoting a higher proportion of total public spending to health, England spent the least on health per person. NHS staff has also increased over the last decade. Scotland has the most GPs per person (80 per 100,000 people in 2009 compared with 70 in England and 65 per 100,000 in both Wales and Northern Ireland). Scotland also has the most medical hospital staff and nursing, midwifery and health visiting staff per person. Comparable data on the efficiency and quality of healthcare are patchy. In 2008-09, average hospital lengths of stay varied from 4.3 days in England to 6.3 days in Wales. Hospital waiting times have fallen in all four nations in recent years, although there are notable variations in how long patients wait for common procedures. In 2009-10 waiting times tended to be lower in England and Wales. There have been significant improvements in levels of healthcare associated infections with rates of MRSA infection dropping by a third or more in all nations. The NAO considers that there would be value in the health departments in the four nations carrying out further work to investigate the variations in performance and identify how they can learn from each other

Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. Book excerpt: At the request of the Government of Liberia (GoL), the IMF Fiscal Affairs Department (FAD) led an external assessment of the central government’s public financial management (PFM) systems based on the Public Expenditure and Financial Accountability (PEFA) methodology. The assessment was undertaken in close collaboration with the Ministry of Finance’s (MoF) PFM Reform Coordination Unit (RCU), with the participation of staff of the African Development Bank and the World Bank, and with financial support from the European Union and Sida. The assessment examines progress since the PEFA assessment of 2007 and provides a renewed baseline for monitoring progress in PFM reform and for supporting the GoL in refining, where necessary, the current PFM reform strategy. The assessment snapshot date was April 23, 2012. The report was reviewed by the GoL, the PEFA Secretariat, a donor reference group, and FAD, the latter being at the same time responsible for quality assurance.
