Book excerpt: The endometrium goes through cyclical changes under the influence of hormones, and immune effectors in the endometrium change their populations cyclically as well. Dynamic changes in immune effectors determine endometrial cytokine and chemokine milieus and, consequently, the endometrial immune response. Endometrial immune responses have been shown to be important for trophoblast invasion and early pregnancy. Indeed, dysregulated endometrial inflammatory immune responses are associated with reproductive failures, such as recurrent pregnancy losses, repeated implantation failure, and gynecological diseases with oncogenic potential. Hence, the investigation of endometrial gene expression may predict infertility, gynecological and oncological conditions. This book presents the state of the art in endometrial gene expression as well as recent developmental findings relating to reproduction and reproductive disorders. It begins with a review of the genetic regulation of urogenital tract formation and molecular mechanism underlying the physiology of menstrual cycle before moving on to the current and emerging technological advances in molecular biology. All rights not granted by the above license are retained by the author. Abstract: Human endometrium undergoes cyclic tissue breakdown and regeneration throughout a woman of reproductive life. It has been speculated that there is a subpopulation of stem/progenitor cells residing in the endometrium responsible to its remarkable capability. One key feature that distinguishes the adult stem cells is their quiescent state within the stem cell niche. Based on this unique stem cell property, slow-cycled cells from human endometrial tissues were examined and characterized. The first aim of this study was to examine label retaining cells (LRCs) in long-term engrafted human endometrial tissues using a mouse xenotransplantation model in vivo. After various initial approaches, the maximum initial labeling of LRCs was determined and, only stromal LRCs were detected after a 12-week chase. Mesenchymal stem cell (MSCs) and stem cell-like phenotypic appearances were detected in a small proportion of stromal LRCs. The second objective was to establish a novel method for identifying slow-cycled cells from cultivated endometrial epithelial and stromal cells in vitro. The method identified cells that retained fluorescence label after long-term culture. These cells were termed fluorescence retaining cells (FRCs). The variables and the different initial approaches leading to the establishment of the protocol for the identification of FRC were optimized. The third objective further characterized the endometrial stromal FRCs. Endometrial stromal FRCs were enriched with cells having higher colony forming and self-renewal abilities when compared with non-FRCs. At subsequent passage (P2) the adult stem cell characteristics of cells derived from FRCs was more prominent and differentiated into mesenchymal lineages. Clonally derived stromal FRCs expressed higher self-renewal and pluripotent genes (BMI-1, NANGOG, OCT4, and SOX2). They also expressed endometrial mesenchymal stem cell phenotypic surface markers: CD146/PDGFRβ and W5C5, suggesting these cells might be of MSC origin. The fourth objective was to examine the role of epithelial-stromal interactions on regulation of endometrial stem cells. Menstrual cycle day 2 epithelial conditioned medium significantly enhanced the colony forming ability of endometrial stromal cells. Protein expression analysis of the early menstrual phase conditioned medium showed two cytokines: angiogenin and interleukin 8, potentially involved in the regulation of endometrial stromal stem/progenitor cells. The fifth objective was to examine the reconstitution ability of the endometrial clonogenic cells in vivo using a mouse xenotransplantation model. Endometrial clonogenic stromal cells together with epithelial cells reconstituted muscle-like tissue. Although no endometrial-like tissue was detected, the findings provide the first in vivo evidence that stromal stem/progenitor cells are present within clonogenic cells. Overall, the thesis reports the existence of slow-cycled cells from human endometrium. By using various label retention techniques in vitro and in vivo a subpopulation of slow-cycled stromal cells which exhibit characteristics properties of adult stem cells was demonstrated for the first time. Indoleamine2,3-dioxygenase enzyme (IDO) contributes to regulation of immune suppression in endometrium by tryptophan depletion and kynurenine generation. RAGE (receptor binding advanced glycation end-products) activation is another mechanism providing immunoregulatory factors such as cytokines. Understanding of the mechanisms leading to endometrial abnormalities might bring us closer to novel cancer therapeutic options for or impaired receptivity of endometrium. This requires proper models suitable for research. Here we present the patient-derived primary endometrial cells cultured as a model for studying immunoregulatory events within endometrial microenvironment.MethodsThe cells were isolated from cancerous and non-cancerous endometrial tissue during the surgical intervention. The in vitro cultured cells were cultured in the optimized medium supplemented with factors promoting growth of epithelial cancer cells. The cell proliferation rate was determined along with morphology. Immunofluorescence was used to test the expression of epithelial marker (CytK8) and MUC1 (abundant in endometrium). The presence of immune regulatory proteins, like IDO and RAGE were determined by IF and Western Blotting. In addition, kynurenine accumulation in the medium from cultured cells was analyzed by HPLC.ResultsThe human primary endometrial cells isolated from cancerous tissue showed higher propagation capability compared to the control tissue (as expressed by the number of passages carried and amount of the achieved cell number). The size and morphology were quite diverse showing a mixture of epithelial-like and fibroblastic cells. Abundant staining with CytK8 antibodies (often overexpressed on the cancer epithelial cells) was seen in all cultures confirming substantial population of the positive cells in the isolates. MUC1 protein was also seen in cultures independently of disease status. Finally, the immune regulatory proteins, i.e. IDO and RAGE were also detected by IF and WB, although at different level dependent on culture. Importantly, the cells isolated from cancerous tissue released higher amount of KYN into the medium, indicating the enhanced tryptophan metabolism.ConclusionThe in vitro expanded primary human endometrial cells might be used as a model to study local immunoregulatory proteins IDO and RAGE involved in biology of endometrium.

The value of existing and emerging systems biology technologies for biomarker development is addressed in several chapters on genetics, microarrays, proteomics and metabolomics. Although tests with high sensitivity and acceptable specificity have been developed, sometimes validated in independent populations and seem promising, more research is needed to translate these data into clinical benefit for patients and coordinate efforts internationally to standarize analysis, reports and operating procedures. The gold standard to diagnose endometriosis is currently through laparoscopic inspection with histological confirmation, a surgical procedure with rare but significant potential risks for the patients. A non-invasive test for endometriosis would be critical for the early detection of endometriosis of symptomatic women with pelvic pain and/or subfertility with normal ultrasound. This would include nearly all cases of minimal-mild endometriosis, some cases of moderate-severe endometriosis without a clearly visible ovarian endometrioma and cases with pelvic adhesions and/or other pelvic pathology, who might benefit from surgery to improve pelvic pain and/or subfertility. Such a test would also be useful in symptomatic women with ultrasound imaging suspicious for endometriosis, since it may be difficult to differentiate an ovarian endometrioma from other ovarian cysts and since the quality of ultrasound imaging is highly variable worldwide.
