Book excerpt: Over the past three decades, many countries of Latin America and the Caribbean have recognized health as a human right. Since the early 2000s, 46 million more people in the countries studied are covered by health programs with explicit guarantees of affordable care. Reforms have been accompanied by a rise in public spending for health, financed largely from general revenues that prioritized or explicitly target the population without capacity to pay. Political commitment has generally translated into larger budgets as well as passage of legislation that ring-fenced funding for health. Most countries have prioritized cost-effective primary care and adopted purchasing methods that incentivize efficiency and accountability for results, and that give stewards of the health sector greater leverage to steer providers to deliver on public health priorities. Evidence from the analysis of 54 household surveys corroborates that investments in extending coverage are yielding results. Though the poor still have worse health outcomes than the rich, disparities have narrowed considerably - particularly in the early stage of the life course. Countries have reached high levels of coverage and equity in utilization of maternal and child health services; coverage of noncommunicable disease interventions is not as high and service utilization is still skewed toward the better off. Catastrophic health expenditures have declined in most countries; the picture regarding equity, however, is mixed. While the rate of impoverishment owing to health-care expenditures is low and generally declining, 2-4 million people in the countries studied still fall below the poverty line after health spending. Efforts to systematically monitor quality of care in the region are still in their infancy. Nonetheless, a review of the literature reveals important shortcomings in quality of care, as well as substantial differences across subsystems. Improving quality of care and ensuring sustainability of investments in health remain an unfinished agenda.

This report sheds light on the complex relationship between stages of economic development and female economic participation. It documents a shift in women’s perceptions whereby work has become a fundamental part of their identity, highlighting the distinction between jobs and careers. These dynamics are made more complex by the acknowledgment that individuals are part of larger economic units—families. As development progresses and the options available to women expand, the need to balance career and family takes greater importance. New tensions emerge, paradoxically made possible by decades of steady gains. Understanding the new challenges women face as they balance work and family is thus crucial for policy.

In addition to exacerbating the structural challenges of gender inequality, the coronavirus disease (COVID-19) pandemic has highlighted the unfair organization of care within society and the need to put care and sustainability at the centre of the development model. The Economic Commission for Latin America and the Caribbean (ECLAC) has therefore called for faster progress towards economic, climate and gender justice and a transition to a care society1 that prioritizes the sustainability of life and care for the planet and guarantees the rights of people who require or provide care; that takes into account self-care; that works to reduce the precariousness related to the care sector; and that raises awareness of the multiplier effects of the care economy on well-being and its ability to drive a transformative recovery with equality and sustainability.At the various sessions of the Regional Conference on Women in Latin America and the Caribbean, the governments of the region have committed to overcoming inequality through public policies that guarantee women's rights and contribute to women's physical, economic and decision-making autonomy and to achieving gender equality in legal frameworks and in the results of policy implementation; in short, commitments have been made to achieving formal equality and substantive equality. The sixty-first meeting of the Presiding Officers of the Regional Conference on Women in Latin America and the Caribbean will include a high-level panel to address the challenges and opportunities of moving towards a care society for recovery with gender equality and sustainability in Latin America and the Caribbean.

These contributions have focused on the need to reorganize and redistribute care work as a key factor in more egalitarian and inclusive societies.Over the course of more than four decades, the member States of ECLAC, meeting at sessions of the Regional Conference on Women in Latin America and the Caribbean, have adopted the Regional Gender Agenda, which aims to guarantee women's rights and drive progress towards their autonomy, laying the foundations for societies with equality. In that framework, governments of the region have adopted a number of agreements that are essential for designing and implementing care policies. The agreements reaffirm the principles of universality and progressivity in access to quality care services, the importance of co-responsibility between men and women, and among the State, the market, communities, and families, as well as the importance of promoting the financial sustainability of public care policies aimed at achieving gender equality.

Book excerpt: The authors present an overview of gender-based violence (GBV) in Latin America, with special emphasis on good practice interventions to prevent GBV or offer services to its survivors or perpetrators. Intimate partner violence and sexual coercion are the most common forms of GBV, and these are the types of GBV that they analyze. GBV has serious consequences for women's health and well-being, ranging from fatal outcomes, such as homicide, suicide, and AIDS-related deaths, to nonfatal outcomes, such as physical injuries, chronic pain syndrome, gastrointestinal disorders, complications during pregnancy, miscarriage, and low birth-weight of children. GBV also poses significant costs for the economies of developing countries, including lower worker productivity and incomes, and lower rates of accumulation of human and social capital. The authors examine good practice approaches in justice, health, education, and multisectoral approaches. In each sector, they identify good practices for: (1) law and policies; (2) institutional reforms; (3) community-level interventions; and (4) individual behavior change strategies.
