There is no pain, swelling or pulsation in the region of the former aneu rism, but there is a little hard tumor and slight tenderness on pressure in the region a little anterior to the temporo-maxillary articulation. Book excerpt: Excerpt from Gaillard's Medical Journal, 1894, Vol. 59 The liver is more influenced by respiratory movements than the other abdominal organs on account of the position of its convex sur face within the dome of the diaphragm. From its position of rest, a, that occupied at the close of passive expiration, according to Sibson it descends and advances nearly two inches in deep inspiration, the lower ribs and cartilage at the same time ascending and advancing. The downward movement of the liver in relation to the ribs amounts to about three inches. In forced expiration the reverse movements take place and the liver is elevated above its position of rest. A few experiments in percussion, the subjects being in the knee chest position, have shown an ascent of liver dullness in the axillary line during expiration varying from one to two inches. Besides these variations in the position of the organ the liver suffers a change of shape during respiration. During a deep inspiration, ac cording to Sibson', it is compressed and shortened from above down ward and widened from side to side. Conversely, on forced expiration the liver, compressed by the lower ribs, would be lengthened from above downward and narrowed from side to side. According to Symington'2 its transverse diameter is diminished and its vertical ex tent on the right side increased by distension of the stomach, and at the same time it undergoes a rotation around a vertical axis passing through the inferior vena cava, the left and the anterior part of the right lobe being most displaced. A distension of the stomach and of the colon may also tilt the anterior surface of the liver upward and backward. The anterior surface of the liver is generally in apposition with the abdominal wall in the recumbent position. When the sub ject stands the anterior edge recedes. It is thought that this change ofposition would be farther increased by bending the body forward. Such partial rotations around ahorizontal as well as a vertical axis might increase in certain cases the separation of the external and the internal wound-track. I would especially allude to one performed by Professor Berry Haycraftl. He has found that if a drop of blood is introduced under suitable precautions into a deep nar row jar of castor oil, and before the drop, which falls slowly through this oil, has reached the bottom of the vesssel, the jar is inverted and the drop made to retrace its steps without having touched the glass, this process being repeated again and again, the drop of blood, having never come in contact with an ordinary solid, remains fluid for an indefinite period. This experiment may perhaps appear to some of you even more conclusive than mine, inasmuch as no coagulation whatsoever occurs in the drop of blood under these circumstances. Certainly it seems to me that it confirms in an absolutely unmistakable manner the view to which I had been previ ously led. Book excerpt: Excerpt from Gaillard's Medical Journal and the American Medical Weekly (Consolidated), 1883, Vol. 35 Quinine caused insanity. - The father of a Washington lawyer guilty of escapades has recentlyr given the following explanation of' the erratic victim: Thinking it a safe thing to do, my son has been in the habit for months of carrying quinine in his pocket, and taking it in small but frequent doses, and the result is an elated, sanguine state of mind, quite beyond the bounds of reason. His memory is not yet impaired, and the marked improvement already consequent upon being deprived of the drug, gives his friends reason to expect (complete restora tion in a short time'. Meanwhile, let this case be one of many others to teach us all that, excellent as quinine may be in many cases, it'is, like chloral or opium or alcohol, exceedingly dangerous to trifle with. Mind 'of the physician, on reading this, is whether quinine has ever been reported as causing, insanity. There are at least two such cases on record, both of them reported in the Journal of Nervous and Mental Disease, July, 1881. In both, insanity always fol lowed. Upon the use of quinine, and, from the_ char acter of the two reported cases, it seems certain that in some predisposed cases a brief, curable insanity may follow upon the use of quinine.
