Au Royaume-Uni, elles ont été généralisées dans les années 2000 et jouent un rôle central dans l'accès aux soins. En Belgique, elles sont en plein essor avec des initiatives locales sur l'ensemble du territoire. Aux Etats-Unis et en France, elles ne font pas l'objet d'une réglementation spécifique. Les équipes françaises rapportent un manque d'harmonisation des pratiques, de formation, de moyens et de communication auprès du grand public et des partenaires. Dans une seconde partie, nous avons étudié leur efficacité. Elles diminuent le nombre d'admissions en hôpital psychiatrique, réduisent la durée des séjours, engendrent une plus forte satisfaction des usagers que les soins hospitaliers et sont moins coûteuses. En France, les éléments recueillis suggèrent aussi une réduction du taux d'admission et une satisfaction des usagers plus élevée. Il parait nécessaire d'approfondir l'évaluation des équipes françaises. Enfin, nous exposons les résultats d'une étude observationnelle portant sur une équipe de crise bordelaise, EPIC, créée en 2016. L'objectif principal était d'analyser la réponse d'EPIC aux demandes de prise en charge. Nous souhaitions aussi évaluer les caractéristiques de la population prise en charge, son délai d'accès aux soins, le taux de recours à l'hospitalisation et le devenir des patients pris en charge. Il ressort de ce travail qu'EPIC apporte trois types de réponse : information et conseil aux usagers, évaluation et orientation, et prise en charge de la crise. La population présente des caractéristiques similaires à celle des patients hospitalisés. Le délai d'accès aux soins était significativement supérieur lorsque l'indication de prise en charge EPIC était retenue, et le taux d'hospitalisation était de 33,33%. Les patients étaient majoritairement orientés vers un CMP (41,67%) ou hospitalisés sans consentement (20,83%).
