In this successful international collaboration cancer chemotherapy has been one of the most productive areas. Pioneers such as YOSHIDA, UMEZAWA, SHEAR, and GOLDIN established firm links of mutual trust and respect in the period after the Second Great War. Japanese drugs, such as mitomycin C and bleomycin have become mainstays of clinical oncology in the U. S. and throughout the world. Many drugs developed in the U. S. have become established in Japanese cancer therapy. Within the cancer chemotherapy field the antitumor antibiotics rank as one of the most important groups. In the U . S. -J apanese collaboration this group of drugs has taken the paramount role. The Japanese, under the leadership of U mezawa, are considered to be among the most innovative and productive in this area which has also had great emphasis in the United States as part of the National Cancer Institute's drug development program and in the pharmaceutical industry. This extended collaboration in general oncology, and chemotherapy in particular, has received increased impetus by and support from the official U . S. -J apan Joint Agreement on cancer research, which was established in 1974 between the National Cancer Institute and the Japanese Society for the Promotion of Science. One of the subsections of this agreement is cancer therapy with emphasis on chemotherapy.

Book excerpt: When Antibiotics I was published in 1967, the teleological view was held by some that" antibiotics" were substances elaborated by certain microorgan isms for the purpose of competing with other microorganisms for survival in mixed ecological environments. However, not only had J. EHRLICH and his associates shown 15 years earlier that chloramphenicol was produced by Strepto myces venezuelae in cultures of sterilized soils but not in parallel cultures of the same soils which were not sterilized, but operationally, the search for anti cancer antibiotics was actively under way (Antibiotics I reporting on numerous such substances), although the concept of antibiosis could not logically justify such undertakings. This editor hesitates to accept the use of the term "antibiotic" for anti microbial agents of non microbiological origins which is sometimes encountered, but neither does he subscribe to the view that antibiotics are in some fundamental manner different from chemotherapeutic substances of other origins. Modes and mechanisms of action of chemotherapeutic compounds are not systematic functions of their origins nor of the taxonomical position of the target organisms. Consequently, in the selection of topics for Antibiotics III (published in 1975), synthetic drugs and natural products of higher plants (alkaloids) were represented, along with antibiotics in the strict sense of the definition. We now present Antibiotics V, for whose assembly the same selection criteria were applied as for Antibiotics Ill. The aggregate length of the contributions rendered it impractical to place the entire text between the covers of one book.

Besides the large number of specialty journals and publications devoted to cancer related fields, nearly every general medical journal contains one or more articles related to cancer treatment and research. Another example of this expanding commitment and interest is reflected in the Internal Medicine subspecialty of Medical Oncology which, since its recognition as a subspecialty in 1973, has become the second most populated subspecialty, second only to cardiology. This burgeoning interest and commitment is obviously appropriate in view of the prevalence and incidence of the various cancers. These diseases constitute, after all, some of the most important and devastating problems of civilized man. It has been particularly gratifying to those involved with cancer research and therapy to observe the increasing interest in these diseases being translated into real improvements in patient care - improvements in length of survival, improvements in quality of survival, and improvements in palliative care. One need only look at Hodgkin's disease to observe the high rate of cure now routinely obtained whereas, in the past, many patients' disease continued to progress with fatal consequences. Some of these improvements came about through better staging techniques, and other improvements, as will be discussed in the chapter on Hodgkin's, came about from the application of early chemotherapy. Further research in Hodgkin's disease is still going on in order to improve results and decrease treatment related complications.
