Book excerpt: A patient, after receiving a total hip arthroplasty, was seen for physical therapy treatment for 8 sessions from 8/01/15 to 8/05/15 in an acute care hospital. Treatment was provided by a student physical therapist under the supervision of a licensed physical therapist. The patient was evaluated at the initial encounter with the Well's Clinical Prediction Rule to assess for the presence of deep vein thrombosis; the Numeric Pain Rating Scale was used to assess pain; the Physical Function ICU Test--Scored was used to assess strength; the Activity Measure for Post-Acute Care mobility short form was used to assess functional mobility; the 10 Meter Walk Test was used to assess gait speed; the Lower Extremity Function Scale was used to assess independence with ADLs; a plan of care was established based on the results of these outcome measures. Main goals for the patient were to improve pain, range of motion, strength, functional mobility, gait speed, and functional independence. Main interventions used were over-ground gait training, restorative training, resistive exercises, bed mobility and transfer training, and functional training. The patient improved in pain levels, strength, range of motion, functional mobility, gait speed, and functional independence. The patient was discharged to home with a home exercise program and a plan to follow-up with home health physical therapy.

This chapter presents a systematic review about conventional physical therapy programs for hip arthroplasty. Based on this review, we proposed therapeutic exercises adapted to a low cost web-platform. In order to reach this objective we will present a brief review of the total hip arthroplasty, telemedicine, telerehabilitation and conventional physical therapy approaches.

The remediation of muscle strength and functional mobility deficits is not achieved through total hip arthroplasty (THA) alone, thus understanding the time course of recovery and the effects of rehabilitation will inform rehabilitation priorities. The overall purpose of this doctoral thesis is to characterize the strength and functional deficits present in end-stage OA and following THA, and to investigate the efficacy of a rehabilitation intervention. The first study examined strength and functional performance in patients with end-stage hip OA compared with healthy adults of similar age. The results of this study indicate that individuals with hip OA had strength deficits and performed poorer on tests of stair climbing, sit-to-stand and walking. They were also less physically active than their healthy peers. These findings indicate that prior to THA, these individuals have diminished strength and functional performance, which should be considered during postoperative rehabilitation planning. The second study examined the time course of strength and functional recovery following THA. The results of this study indicated that patients see a significant decline in muscle strength and functional performance one month following surgery. Twelve months after THA, patients exhibit significant deficits in knee extensor and flexor strength compared with healthy adults. Furthermore, although not statistically significant, differences in functional performance compared to healthy adults remained. The third study examined the efficacy of a comprehensive, multi-component (CMC) rehabilitation program. The results of this study indicated that individuals who participated in the rehabilitation intervention had greater improvements in stair climbing ability, walking ability, balance and stability compared with individuals who did not receive intervention (CON). Further, there tended to be greater improvement in muscle strength by the end of intervention. Importantly, participation in the CMC intervention did not compromise safety, as there were no incidences of hip dislocation, injury or falls in either group. Overall, the results of these studies indicated strength and functional deficits present in end-stage hip OA may not be completely remediated by THA surgery alone. An 8-week multi-component rehabilitation program may enhance recovery after THA.

The main goals for the patient were to decrease pain severity, improve basic functional mobility, increase gait speed, and advance health related quality of life. The principal interventions used were resistance training, over-ground gait training, task-specific training, and functional training. The severity of the patient's pain improved significantly [75%], and, to a lesser degree, basic functional mobility and gait speed improved as well. The patient was discharged to an extended care facility for further physical therapy treatment.

The main interventions used were: functional, task specific strength training; gait training on level, uneven surfaces and stairs; and balance training in different environments. The patient improved pain levels, strength, balance, gait speed, ambulation distance, functional independence, and community mobility.

While THR is tremendously successful in terms of pain relief for patients, outcome studies report some residual impairment and functional limitations in these patients post operatively. Despite the rising number of hip replacements performed, the optimum rehabilitation after total hip replacement lacks consensus throughout the literature and no evidence based guidelines exist worldwide on this topic. Physiotherapy exercises and protocols within rehabilitation programs are largely based on level 4 evidence. There is little evidence to support the effectiveness of physiotherapy post total hip replacement, and little to support physiotherapy supervised exercise programs in favour of home exercise programs. This thesis aimed to inform best practice in rehabilitation following hip replacement. First, by systematically reviewing the current evidence for total hip replacement rehabilitation. Secondly by a prospective study to determine the benefit of supervised and unsupervised rehabilitation interventions on patient outcomes, namely functional and quality of life measures (WOMAC and SF-36 scores); post total hip replacement surgery in the adult population. Finally, to contribute to the evidence supporting clinical decisions in Australia for total hip replacement rehabilitation. Our systematic review aimed to synthesise the current literature for rehabilitative exercise effectiveness on strength, gait, function and quality of life after a total hip replacement including comparison in a home based or supervised setting. Five randomised controlled trials were included comprising 234 participants. Meta-analysis showed that physiotherapy can improve abductor muscle strength of the operated leg, as well as gait speed and cadence. Function and quality of life measures were unable to be measured due to the broad variation in outcome measures used in the studies. There was no difference between supervised and unsupervised exercise programs. Further well designed research is needed to compare the efficacy of home based to supervised programs. Two key topics for study are providing accurate assessments pre- and post-THA and investigating factors that could affect functional outcomes after THA. MANUSCRIPT I - Performance-Based Tests and Self-Reported Questionnaires Provide Distinct Information for the Preoperative Evaluation of Total Hip Arthroplasty Patients. BACKGROUND: Performance-based tests (PBT) such as the 2-minute walk test (2MWT) and the timed get-up-and-go test (TUG) have been advocated for the use as an essential part of preoperative THA evaluation. OBJECTIVES: The objectives of this study were to assess the relationship between self-reported questionnaires (Western Ontario McMasters Universities Osteoarthritis Index [WOMAC] and the Short Form-36 [SF-36]) and PBT (2MWT and TUG) in patients scheduled for THA, and to identify clinical variables associated with the 2MWT and the TUG. METHODS: We collected data from a cohort of patients scheduled for primary THA. Each patient was asked to complete the WOMAC and SF-36 and perform the 2 tests (2MWT, TUG) at the time of a preoperative evaluation. Pearson's correlation coefficients and Spearman's rho were used to determine the association between the self-reported questionnaires and PBT. Multivariable linear regression models were created to evaluate independent associations between clinical variables and preoperative 2MWT and TUG results. RESULTS: The correlation between preoperative self-reported questionnaires and 2MWT and TUG tests was mild to moderate, ranging from 0.16 to 0.58. The use of a walking aid, female sex and the presence of other painful lower extremity joints were significantly associated with 2MWT (r 2 =0.51, p=0.017), while age and additional painful lower extremity joints were significantly associated with TUG (r 2 =0.10, p=0.017). DISCUSSION: Our study demonstrates that there are mild to moderate correlations between preoperative self-reported questionnaires and PBT. This indicates that PBT capture additional dimensions of function that are not measured by the WOMAC and SF-36 and might be important patient-centered outcomes during post-operative evaluations.
