In spite of the low fuel failure rate in currently operating water cooled nuclear power reactors, there is a continued high level of interest in fuel failures, owing to the problems and inconvenience caused by fuel failures and to the generally accepted goal of achieving a zero failure rate. This requires a detailed knowledge of existing failure mechanisms, their root causes and remedies. A total of 21 papers addressing those issues were presented and discussed at the meeting. The main conclusions and recommendations are summarized in this publication.

Book excerpt: Failure of a fuel element in a nuclear reactor core is determined by a gas tagging failure detection system and method. Failures are catalogued and characterized after the event so that samples of the reactor's cover gas are taken at regular intervals and analyzed by mass spectroscopy. Employing a first set of systematic heuristic rules which are applied in a transformed node space allows the number of node combinations which must be processed within a barycentric algorithm to be substantially reduced. A second set of heuristic rules treats the tag nodes of the most recent one or two leakers as "background" gases, further reducing the number of trial node combinations. Lastly, a "fuzzy" set theory formalism minimizes experimental uncertainties in the identification of the most likely volumes of tag gases. This approach allows for the identification of virtually any number of sequential leaks and up to five simultaneous gas leaks from fuel elements.

Gas releases and sodium-fuel contact events are analyzed on EBR-II by continuous on-line cover gas and sodium monitoring techniques and by the laboratory analysis of periodic samples. On FFTF hardware, electronics and computer software required for FFM have been developed, designed and tested and are in place awaiting plant startup.
