The advent of new technologies has presented solutions for yesterday’s barriers to research, allowing us to leap forward in our ability to prevent, diagnose, and treat various cancers. Developments in omics studies has provided new insights into the underlying molecular basis of different cancers and their subtypes, greatly enhancing our understanding of the vast heterogeneity that exists. Progress in our ability to diagnose and detect early-stage cancers has resulted in numerous screening and prevention programs. Novel imaging technologies allow us to study and comprehend cancers with greater clarity than ever before. Although the field, as a whole, has experienced many successes, arduous challenges must be overcome in order to see continued success. The truth of the matter is cancer related deaths continue to rise in number worldwide, especially in lower- and middle-income countries. It is evident that there is still a lot of work to be done. This Frontiers in Oncology Special Issue marks the 1st edition of a collection of selected articles published in the journal over the course of the previous calendar year, highlighting ongoing research and advances being carried out in the different disciplines of cancer research.

Thanks to the incredible work and efforts by the scientific community, we have greatly enhanced our ability to detect breast cancer in its earliest stages, and have been able to develop simple routine screening techniques to ensure that everyone, even those in areas where resources are scarce, can identify early signs of a tumor forming. Our ability to treat breast cancer has greatly improved, becoming more sophisticated and precise. The advent of state-of-the-art imaging instruments, advanced surgical techniques, and immunotherapy means that we are able to treat patients better, being mindful of the plethora of difficulties that arise for breast cancer survivors. Breast cancer, despite tremendous advances in recent decades, remains one of the most common cancer types across the globe, and efforts by the scientific community require continued to support in order to guarantee further advances. In this Special Edition, we present selected articles looking at breast cancer from all angles. Coming from a diverse set of authors, this Special Edition includes manuscripts evaluating nation-wide screening programs, advanced surgical techniques, the future direction of molecular targeting, and more. Book excerpt: Cancer accounts for millions of deaths every year, and the burden of this disease is striking - testing our families, health-care systems, economies, and our scientists. In recent years, the outstanding work of researchers and vast improvements in technology has led to remarkable strides in progress. We are now able to prevent at least one third of cancers and have adapted routine-screening techniques for early detection and effective treatment. Our ability to treat and manage this shape-shifting disease has also transformed, as we have developed sophisticated therapies and adopted more tailored approaches. As a result, survival rates are reaching new highs each year, and the outlook for those affected is improving. However, there are still areas that require our attention. Unfortunately, inequalities are well known in the field. In areas where resources are scarce and outreach is limited, cancer patients do not have access to educational programs, timely diagnosis and quality treatment. Significant knowledge-gaps also exist within cancer research, with many minority populations being underrepresented in clinical trials and underreported within the literature. Considering that scientific progress relies on the publication and dissemination of research, the lack of access to primary literature also falters, with many breakthroughs hidden behind paywalls. This not only affects clinicians and researchers, reinforcing a negative feedback-loop for researchers already struggling to obtain sufficient funding, but inhibits the next generation of curious students. Each year, February 4th marks World Cancer Day; a movement dedicated to channeling awareness, education, and unity into collective initiatives and global action against one of medicine’s toughest challenges. The theme of 2021, “I Am and I Will” was one of power, encouraging commitment and togetherness; a sentiment resonating in today’s turbulent world. In honor of this day, Frontiers in Oncology has invited a retrospective of articles from our Specialty Chief Editors, highlighting current, international challenges in their corresponding fields of oncology. Our goal is to empower continuous discussion between communities and across borders, drawing attention to the disparities faced in the field. Our achievements should be shared to maximize impact and facilitate opportunities worldwide. We know that cancer does not discriminate. So, neither should we. We also take this opportunity to thank the wider community for their continued efforts in allowing for accelerated scientific developments, and most importantly for working with us on our mission to make science open. With a record number of papers to choose from, the papers enclosed in this Special Edition highlight some of the recent advances across the different disciplines. With an array of papers looking at everything from translational research to association studies and clinical trial results, Frontiers in Oncology continues to publish high-quality research from researchers around the world. Many of the recently posted Research Topics in the journal focus on early diagnosis and prevention, as well as the importance of survivorship studies and outcomes of cancer treatment. Consideration of such aspects of cancer research and treatment is pivotal in the global effort towards increased survival rates and improved quality of life. Book excerpt: Since the late 1960s, the survival rate in children and adolescents diagnosed with cancer has steadily improved, with a corresponding decline in the cancer-specific death rate. Although the improvements in survival are encouraging, they have come at the cost of acute, chronic, and late adverse effects precipitated by the toxicities associated with the individual or combined use of different types of treatment (e.g., surgery, radiation, chemotherapy). In some cases, the impairments resulting from cancer and its treatment are severe enough to qualify a child for U.S. Social Security Administration disability benefits. At the request of Social Security Administration, Childhood Cancer and Functional Impacts Across the Care Continuum provides current information and findings and conclusions regarding the diagnosis, treatment, and prognosis of selected childhood cancers, including different types of malignant solid tumors, and the effect of those cancers on childrenâ (TM)s health and functional capacity, including the relative levels of functional limitation typically associated with the cancers and their treatment. This report also provides a summary of selected treatments currently being studied in clinical trials and identifies any limitations on the availability of these treatments, such as whether treatments are available only in certain geographic areas.

Cancer continues to be a major and growing problem in conflict affected areas too. The incidence of cancer is expected to rise significantly in those countries compared to the rest of the world. Conflict, massive migration and displacement has put a tremendous pressure on all health care systems and health economy which halted improvement in many cancer care in majority of the countries. Countries affected by conflict like Syria, Ukraine, Iraq, Yemen, Latin America and others has led to destruction of the whole countries healthcare infrastructure including health care facilities, diagnostic facilities, and unavailability of drugs and loss of health care professionals due to death, migration or disabilities. This lead to inequitable access to care, lack of prevention, loss of screening programs, delay in diagnosis, loss of follow up and in many cases no anticancer therapy or palliative care to offer. Historically, most of humanitarian aid was focused on basic needs and first aid, leaving NCDs including cancer care with trivial support if any. Recently with the Russian invasion of Ukraine and the negative global economic crisis, refugees all over the globe will have devastating impact on cancer care screening, early detection, treatment and palliative care.

Book excerpt: Advances in anti-cancer chemotherapy over recent years have led to improved efficacy in curing or controlling many cancers. Some chemotherapy-related side-effects are well recognized and include: nausea, vomiting, bone marrow suppression, peripheral neuropathy, cardiac and skeletal muscle dysfunction and renal impairment. However, it is becoming clearer that some chemotherapy-related adverse effects may persist even in long term cancer survivors. Problems such as cognitive, cardiovascular and gastrointestinal dysfunction, and neuropathy may lead to substantial long term morbidity. Despite improvements in treatments to counteract acute chemotherapy-induced adverse effects, they are often incompletely effective. Furthermore, counter-measures for some acute side-effects and many potential longer term sequelae of anti-cancer chemotherapy have not been developed. Thus, new insights into prevalence and mechanisms of cancer chemotherapy-related side effects are needed and new approaches to improving tolerance and reduce sequelae of cancer chemotherapy are urgently needed. The present Research Topic focuses on adverse effects and sequelae of chemotherapy and strategies to counteract them.
