La mise en place d'une formation à ce sujet au cours de l'Internat de Médecine Générale permet-elle d'améliorer ce dépistage chez les professionnels concernés ? Matériel et Méthode : une étude observationnelle prospective monocentrique avant/après a été réalisée de novembre 2017 à mars 2019 auprès des Internes en Médecine Générale (IMG) de la Faculté de Médecine de Strasbourg inscrits à la formation. Les participants ont été invités par mail à répondre à un premier questionnaire avant la formation, puis à un second deux mois après la formation. Résultats : 66 IMG étaient inscrits à la formation. Parmi ceux-ci, 52 ont répondu au premier questionnaire, et 47 ont répondu au deuxième questionnaire. La formation a permis la mise en place d'un dépistage systématique des violences lors d'une première consultation chez 42% (n=15) des participants, alors qu'aucun (n=44) ne le faisaient avant, p=0,0005. Conclusion : la formation des IMG au repérage et à la prise en charge des victimes de violences conjugales permet une réelle amélioration des pratiques, et lève un certain nombre de freins empêchant d'aborder le sujet en consultation. Le développement de ce type de formation dans les différentes facultés de médecine semble important pour améliorer la prise en charge des victimes de violences conjugales.

Book excerpt: CONTEXTE : Chaque année, en France, 9 à 10 % des femmes déclarent être victimes de violences conjugales. La HAS et le Ministère des affaires sociales, de la santé et des droits des femmes recommandent un dépistage systématique des violences conjugales par tous les professionnels de santé de premiers recours pour améliorer leur prise en charge comme indiqué dans les 4e et 5e plans interministériels de prévention et de lutte contre les violences faites aux femmes. OBJECTIFS : Réaliser un état des lieux de la formation initiale des futurs professionnels de santé de Meurthe et Moselle et des connaissances apportées par celle-ci, estimer leur intérêt pour la problématique des violences conjugales et identifier les besoins en formation. MÉTHODE : Une enquête transversale, descriptive, quantitative, auprès des étudiants des filières ergothérapie, kinésithérapie, maïeutique, médecine, odontologie, pharmacie option officine et soins infirmiers a été réalisée sous la forme d'un questionnaire de 23 items. RÉSULTATS : Il y a eu 367 étudiants répondants, nous permettant d'obtenir un taux de réponse de 39%. Malgré les recommandations officielles, la thématique des violences conjugales n'est pas abordée dans toutes les filières de santé étudiées. Le taux de réponses correctes aux questions générales sur la prise en charge des violences conjugales est faible mais significativement plus important lors de l'existence d'une formation. La confiance des étudiants en leur capacité de prise en charge est statistiquement améliorée lors de l'existence d'une formation en effectifs réduits et répétée dans le cursus. Les étudiants, quelles que soient leurs futures catégories professionnelles, expriment majoritairement une volonté à être formés le plus tôt possible à ce sujet. La connaissance du réseau local, la législation et les pratiques de dépistage et d'information des victimes restent des sujets à approfondir au cours de la formation. CONCLUSION : Le développement de formations sur le dépistage et la prise en charge des femmes victimes de violences est primordial pour tous les professionnels de santé de premier recours afin qu'ils prennent conscience de l'ampleur de ce problème de santé publique et puissent dépister et prendre en charge au mieux ces patientes. L'intégration de telles formations à la formation initiale des professionnels de santé parait aujourd'hui indispensable.

Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."
