Book excerpt: Foodborne diseases (FBD) are an important externality of agriculture and food systems, but only recently have they risen up the development agenda as the result of growing awareness of the health and economic burdens of FBD and how they relate to food systems with a focus on low-income and middle-income countries (LMIC), particularly African nations. The health burden of FBD is comparable with that of malaria, and over 90% falls on people in LMIC, with an economic burden of more than US$100 million per year. FBD have many other, less well-estimated effects on nutrition, gender, equity, and the environment. While understanding of food safety in domestic markets of LMICs has advanced greatly, risk management is in its infancy. This Research Topic will bring together leading regional perspectives on food safety in LMICs.

They are frequently traded in local, unregulated markets and this can pose significant health risks. This volume presents an accessible overview of these issues in the context of food safety, zoonoses and public health, while at the same time maintaining fair and equitable livelihoods for poorer people across the continent. The book includes a review of the key issues and 25 case studies of the meat, milk, egg and fish food sectors drawn from a wide range of countries in East, West and Southern Africa, as part of the "Safe Food, Fair Food" project. It describes a realistic analysis of food safety risk by developing a methodology of ‘participatory food safety risk assessment’, involving small-scale producers and consumers in the process of data collection in a data-poor environment often found in developing countries. This approach aims to ensure market access for poor producers, while adopting a realistic and pragmatic strategy for reducing the risk of food-borne diseases for consumers.

Most of the foods that carry the highest pathogen risk are produced by smallholder farmers, marketed through the informal sector, and sold in wet markets. Given the significant role of informal markets in African and Asian food systems, attention is invested in understanding (1) how the people that participate in informal markets are exposed to risk, and (2) how they manage risk. We conduct a participatory risk analysis with a gender lens in 20 livestock and fish value chains to study whether gender-based differences influence risk of food-borne disease. We find that socially constructed gender roles are more important determinants of health risk than biological differences between men and women. Variations in risk exposure between men and women are mainly due to gender-based differences in occupational exposure, and secondarily to differences in consumption patterns. Women are important but under-recognized risk managers in the realms of food production, processing, selling, preparation, and consumption. Understanding the influence of gender on risk exposure and management is essential for improving food safety in informal markets.

Book excerpt: Increased consumption of meat, milk, eggs, and fish among poor consumers in developing countries has the potential to improve nutrition as well as drive pro-poor economic development. However, animal-source foods are a major source of food-borne disease. In addition to the health impacts, concerns about food safety can reduce consumption of nutritious foods and reduce market access for smallholders. Researchers from the International Livestock Research Institute and partners have developed and piloted an institutional innovation -- a training, certification, and branding scheme for informal value chain actors -- that has the potential to improve the safety of animal-source foods sold in informal markets. To support further research and, eventually, delivery at scale, this paper develops a theory of change for how the intervention is expected to contribute to better nutrition and health outcomes for consumers. The outcomes along the pathway from intervention to impact are identified, along with the underlying causal assumptions. For each assumption, the existing evidence is summarized and assessed. The results show that for some parts of the impact pathway, outcomes and causal links are well defined and supported by evidence, while for others, the program logic needs to be refined and more evidence gathered to validate hypothesized causal relationships in specific contexts. Addressing these gaps through research and through piloting interventions with development partners can increase the likelihood of achieving expected outcomes and contribute to learning about how to improve the performance of informal markets in developing countries.

Book excerpt: Ensuring the safety of food and the quality and safety of medicines in a country is an important role of government, made more complicated by global manufacturing and international trade. By recent estimates, unsafe food kills over 400,000 people a year, a third of them children under 5, mostly in low- and middle-income countries; every year poor quality medicines cause about 70,000 excess deaths from childhood pneumonia and roughly 8,500 to 20,000 malaria deaths in sub-Saharan Africa alone. The Federal Drug Administration (FDA) Office of Global Policy and Strategy is charged with improving capacity of the agency's foreign counterpart offices and increasing understanding of the importance of regulatory systems for public health, development, and trade. At the request of the FDA, this study sets out a strategy to support good quality, wholesome food and safe, effective medical products around the world. Its goal is to build on the momentum for strengthening regulatory systems and to set a course for sustainability and continued progress. The 2012 report Ensuring Safe Food and Medical Products Through Stronger Regulatory Systems Abroad outlined strategies to secure international supply chains, emphasized capacity building and support for surveillance in low- and middle-income countries, and explored ways to facilitate work sharing among food and medical product regulatory agencies. This new study assess progress made and the current regulatory landscape.

However, the conditions under which the informal sector operates raise concerns relating to the safety and quality of food sold. This chapter makes reference to case studies conducted by the International Livestock Research Institute,Äôs Safe Food Fair Food project to facilitate the implementation of food safety in Sub-Saharan Africa. The purpose is to illustrate the relevance and applicability of a food chain approach in the implementation of food safety in the informal sector. Results show that some milk gets contaminated during milking, processing, cooling and handling. Such practices as adulteration and pooling were also reported to contaminate milk. The chapter concludes that it is the responsibility of all stakeholders to ensure the safety of milk along the chain.

International disputes over sanitary and phytosanitary (SPS) measures influence the ability of developing countries to compete in export markets. These countries need to evaluate their interest in the SPS agreement under the World Trade Organisation (WTO); participate more fully in international agencies responsible for harmonisation; develop the capacity to assess equivalence for process standards, which are increasingly used for fresh food products; and resist the imposition of inappropriate standards.
