The International Network Health Policy and Reform aims to narrow the gap between health services research and health policy. Network partners are research institutions and health policy experts from 20 industrialized countries.

These objectives may be conflicting and require a balance. It is unlikely that competition is aligned with all of these objectives at the same time. The conditions for competition to be a useful instrument vary across countries, health care subsectors and time. There is no golden rule or unique set of conditions that can be met to ensure that competition will always improve the attainment of health system goals. Introducing, increasing or changing competition in health services is a delicate policy exercise.^The need for an appropriate regulatory framework should be analysed, and relevant institutions and mechanisms put in place. Accreditation of providers and the detailed design of payment systems are of specific importance. Sound policy evaluation studies are also needed to assess and judge the impact of competition, because policy design and policy outcomes are likely to vary from one context to another. Such evaluation should form the basis for changes in regulation to meet defined policy goals. Key elements to consider when introducing, changing or increasing competition are ensuring market transparency, with availability of information on quality and prices, careful monitoring of access and equity effects, promoting health literacy, and enforcing competition rules to prevent the creation, strengthening and abuse of dominant positions. Competition among health care providers is distinct from patient choice.^The value of patient choice has gained important status in several countries as a principle underpinning the health system, and as an instrument for making the allocation of health system resources responsive to patient preferences. Patient choice may be combined with different degrees of competition among health care providers; between public providers only, between public and private providers, and with different restrictions for entry to the market. Patient choice works best in situations where patients can easily assess the quality of the services provided. There is a permanent need to build empirical evidence in a way that is useful for policy makers, as context-specific realities will change and with them the effects of introducing or increasing competition among health care providers.

Book excerpt: In this paper we outline the tools that have been developed to model and analyze competition and regulation in health care markets, and describe particular papers that apply them to policy-relevant questions. We focus particularly on the I.O. models and empirical methods and analyses that researchers have formulated to address policy-relevant questions, although we also provide an overview of the institutional facts and findings that inform them. We divide the chapter into two broad sections: (i) papers considering competition and price-setting among insurers and providers and (ii) papers focused specifically on insurance and market design. The former set of papers is largely concerned with models of oligopolistic competition; it is often focused on the US commercial insurance market where prices are market-determined rather than being set administratively. The latter focuses on insurance market design with an emphasis on issues raised by asymmetric information, leading to adverse selection and moral hazard. In addition, we discuss the literature on consumer choice frictions in this market and the significant implications of those frictions for I.O. questions.

Book excerpt: We review Greek public sector healthcare policies and health-related outcomes since 2010.We find that excess spending was successfully curtailed, elements of the institutional framework were modernized, and health outcomes have been relatively favorable. However, especially prior to Covid-19, public healthcare spending had been compressed to potentially unsustainable levels, with widening inequalities and large unmet needs, especially among the poor. Higher public spending and advancing structural healthcare reforms are needed to improve the efficiency and equity of the Greek healthcare system, including strengthening primary healthcare, reducing out-of-pocket payments, and eliminating remaining insurance gaps.
