Book excerpt: Introduction : Le tri réalisé par l'infirmière organisatrice d'accueil (IOA) est une mission indispensable à une prise en charge médico-chirurgicale correcte devant un flux croissant de patients consultant au service d'accueil des urgences. Le but de l'étude est de juger de la pertinence et de la bonne application des échelles de tri en étudiant les patients initialement installés en box et redirigés secondairement à la SAUV. Matériel et méthode : De juillet à décembre 2015, tous les patients installés en box et redirigés à la salle d'accueil des urgences vitales (SAUV) ont été inclus dans l'étude sans restriction concernant l'âge, le sexe ou l'ethnie. Une étude qualitative et quantitative a été réalisée dans cette population. Des critères secondaires tels que les paramètres vitaux, les constantes biologiques et notamment le taux de lactate ont été étudiés. Résultats : 216 patients ont été inclus sur les 15073 admissions sur la période étudiée. 18 patients ont été admis via le SMUR et 53 par leurs propres moyens. 65% des motifs d'admission sont représentées par une plainte bien identifiée. 34% des admissions sont représentées par une détresse vitale. 203 patients seront hospitalisés. 79 patients avaient des paramètres vitaux justifiant initialement un transfert à la SAUV. Conclusion : L'IOA a un rôle fondamental au sein des urgences et la bonne application des échelles de tri est indispensable. Il semble toutefois nécessaire de bien s'attarder sur les paramètres vitaux relevés à l'admission des patients.

Matériel et méthode : il s’agissait d’une étude cas-témoin, monocentrique comparant les patients admis directement au SAUV (SAUV1) aux patients admis secondairement (SAUV2) sur les données d’accueil (motif, paramètres vitaux), motifs de transfert, résultats biologiques. Nous avons inclus 241 patients et comparé deux groupes. 162 patients étaient admis directement en salle d’accueil des urgences vitales (Groupe SAUV 1) et 79 patients ont été admis initialement au service des urgences polyvalentes adultes avant d’être secondairement transférés en salle d’accueil des urgences vitales. Book excerpt: The admission of a patient in the intensive care room of emergency (ICRE) in our hospital and its organization are ill-defined. Despite 39,000 admissions per year in the emergency service, ICRE seems underexploited. The objective is to quantify the daily number of patients who need to be admited to ICRE. Materials and Methods: Prospective study of patients admitted to the emergency department from 3 to 17.3.2014. An evaluation grid with severity criteria (critical vitals and reasons for admission) was established from existing sorting scales filled for all patients. Results: 1460 medical records, 1316 patients included (90.14%). Average age = 49.8 year old. Sex ratio M / F = 1.17. 14 patients admitted to ICRE. Average age = 69.6 year old, sex ratio M / F = 1. Admission Reasons: 3 cardiac arrests, 3 polytrauma, 4 neurological failures, 3 cardiac failures, 1 respiratory failure. The patients were dispatched to recovery unit (5), cardiac intensive care (2), deceased (2), short hospitalization unit (3), hospital unit (1) released (1). 100 patients (7.6%) were admitted to other units despite fullfilling ICRE's admission criteria. Causes of admission: 29 respiratory failures, 21 cardiovascular failures, 20 neurological failures, 12 hemodynamic failures and 7 traumas. Patients were referred to short hospitalization unit (31), medical services (31) or surgery (6), cardiac intensive care (11), recovery unit (3) released (15), deceased (1). Discussion: Our study confirms the underutilization of ICRE, it is only used for extreme gravity patients and therein is used as a recovery unit. This underutilization leads to overcrowding and poor efficiency of other units of the service. Conclusion: The appointment of a responsible physician and standardization of patient admissions to ICRE would improve both management of potentially unstable patients and the effectiveness of the entire service.

Book excerpt: L’accroissement majeur du nombre de consultations aux urgences ces dernières années implique que la Salle d’Accueil des Urgences Vitales ( S.A.U.V.) soit conforme aux Recommandations de 2002 établies par les Sociétés Savantes, afin d’assurer une bonne prise en charge des patients en situation d’ urgence vitale ou potentielle. Une étude rétrospective a été menée aux Urgences du Centre Hospitalier d’Armentières du 1er Janvier au 30 juin 2007, incluant 479 patients répondant aux critères d’admission dans la SAUV.L’objectif de cette étude était d’évaluer l’activité de cette salle de « déchoquage », et d’identifier les patients qui y étaient admis, les gestes et examens effectués, leur orientation et leur devenir. La SAUV représentait 3 ,7% de l’activité globale et 11,3% des hospitalisations du service. Les patients avaient un âge moyen de 61,5 ans, appartenaient majoritairement aux classes 4 et 5 de la CCMU dans 54% des cas, et présentaient pour 81% d’entre eux des pathologies médicales, essentiellement cardiovasculaires. Si quasiment tous avaient bénéficié d’examens biologiques et radiologiques, une tomodensitométrie était réalisée dans 23% des cas. Au total, 32,8% avaient bénéficié de gestes de réanimation. Les patients étaient majoritairement orientés vers la cardiologie ou les USIC ( 28%), mais aussi l’UHCD dans 19% des cas. La réanimation ou les USC concernaient 19,4% des patients. La durée moyenne de passage dans la SAUV s’élevait à 4 heures et 45 minutes. Seul un quart des patients des classes 4 et 5 de la CCMU avaient été médicalisés par le SMUR avant leur entrée dans la SAUV, d’où la nécessité de mettre en place une grille de triage qui permettrait d’apprécier efficacement le degré de gravité des patients et d’ optimiser leur prise en charge initiale dès l’arrivée aux urgences. Cette étude met en évidence qu’outre sa fonction de prise en charge des urgences vitales, si la SAUV du Centre Hospitalier d’Armentières remplit sa fonction première dans près de 54% des cas, elle assure également un rôle de stabilisation, d’évaluation, et de surveillance rapprochée des patients.Dans les années à venir, une restructuration profonde du Service des Urgences du C.H.A. et une augmentation des moyens matériels et humains avec une IOA 24H/24 à l’accueil permettront une sectorisation des filières de soins, et une utilisation de la SAUV spécifiquement dédiée à la prise en charge des urgences vitales
