After he positioned the mini-pumper at a hydrant, he began to unroll the four-inch supply hose. Soon after unrolling the hose he collapsed. A crew member, just arriving at this location, initially thought the victim was checking something under the mini-pumper. The crew member asked the victim what he was doing, but quickly noted he was unresponsive with shallow respirations. After notifying dispatch of the situation, the crew member began cardiopulmonary resuscitation (CPR). Approximately 63 minutes later, despite CPR and advanced life support (ALS) administered on the scene and at the hospital, the victim died.

Book excerpt: On June 23, 2005, a 33-year-old career female Fire Fighter/Emergency Medical Technician (FF/EMT) reported to her duty station. During the daylight portion of her shift, she responded to two calls: a lightning strike on a house and a fire apparatus breakdown. After dinner, she retired to her bunkroom. Early the next morning, at 0205 hours, her crew (Rescue 121) and Brush 121 were dispatched to a medical call, but she did not respond. A crew member checked on her and found her unresponsive. After calling the brush crew back to the station, the crew member assessed her and radioed Dispatch, who dispatched a neighboring fire department (FD). Cardiopulmonary resuscitation (CPR) and advanced life support (ALS) treatment were performed by crew members until paramedics from the neighboring FD arrived. From the FF/ EMT's clinical condition it was obvious she had expired much earlier in the night.

To this end, data were collected on firefighter fatalities between 1990 and 2000. (For further information, see the "Methodology" section or the Appendix.) Using this analysis, better targeted prevention strategies can be developed in keeping with the USFA's goal to reduce firefighter deaths 25 percent by 2005. In contrast to the annual USFA firefighter fatality reports, this analysis allowed for comparisons over time to determine any changes in firefighter mortality, with a depth of scrutiny not present in earlier analyses.

Upon returning to the station, his crew members found him unresponsive, not breathing, pulseless, and cool to the touch. Due to his cool skin, fire fighters determined he had been dead for at least 1 hour, and no resuscitation measures were initiated. The death certificate, completed by the Assistant Medical Examiner, listed "arteriosclerotic cardiovascular disease" as the immediate cause of death, and the autopsy found significant coronary artery disease.
